A fire consumed the the mine office trailer. Cause unknown. No one was present at the time of the fire. Fire department extinguished fire and notified owner of the property.

Employee states they "Went to the wrong side of the handrail thinking the ladder was on that side and it wasn't but it was too late". Employee fell off of the ANFO rig suffering a right ankle sprain.

Injured states "I forgot my knife so blaster was throwing their closed knife at pillar next to me which hit and opened on impact then bounce (sic) and stabbed (sic) me in the arm." 3 stitches required.

Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.

Employee reported on 7/14/2021 that EE's back was hurting and had been bothering EE since the week of 6/28/2021, when EE was operating various pieces of equipment. Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.

Employee stated that while mounting a haul truck, employee bent over to pick up the coffee that employee had placed on the bumper, lost balance, and fell off the truck.

The employee states EE closed hopper inspection door on sweeper truck when EE felt a pain in EE's elbow. Reportable due to medical treatment on 4.28.2022.

Employee stated that while operating a loader, the bucket struck a toe on the floor and caused elbow to be stuck between the arm rest and the machine controls, resulting in elbow pain. No medical treatment until 04/12/2021.

Employee was walking towards EE's vehicle because the mine shut down due to snowy weather conditions. EE slipped and fell on the snow in the parking lot near EE's vehicle. Physical therapy recommended to start on Feb 15.

Employee stated EE was shoveling on the catwalk of the VSI when EE felt a pain in EE's lower back. Employee received medical treatment on November 6.

Employee stated that as EE was climbing onto EE's loader EE's foot slipped off the ladder rung and EE felt a pull in EE's groin.

Employee stated was hand scaling a pillar when a piece of rock fell from above and struck arm.

On 1/20 during the workplace exam, it was discovered the Gazebo Lake located above the mine had emptied through a geological formation its entire contents into the mine. Date and time of the occurrence is unknown, no weekend work. No injuries from this accident.

Employee was bringing 50 pound anfo bags from inside the truck to the tailgate. While bending over, employee twisted and felt pain in lower back. Employee sought treatment with Chiropractor on 12-30-19. No work restrictions given.

Employee stated that as they were loading a blast hole, a small rock fell from the hole being loaded above and struck them on the neck and shoulder.

Employee stated was cleaning with a loader when a rock rolled around the bucket which EE ran over causing EE to strain EE's back.

Employee stated was using a pry bar to clear material from a chute when the bar slipped and employee caught a finger between the chute and the pry bar.

Employee stated ee was scaling in a man lift when the rock ee was prying on forced the pry bar down and caught finger between the pry bar and the top railing on the man lift basket.

Employee stated EE was driving the man-lift out of the mine when the man-lift hit a hole in the road causing the man-lift to rock and jarred EE's back. No medical treatment until physical therapy was prescribed on March 13.

Employee was pushing a wheel barrel of rock during cleanup and felt pain in neck and shoulder.

Employee reported numbness and tingling in right wrist, diagnosis as carpal tunnel, surgery on 6/22/2018

Employee stated ee was working on a hose on machine, felt a sharp pain in knee while walking around machine. No restrictions on initial medical examination, treatment on 2/21/2018 resulted in injury becoming reportable.

Employee stated stepped out of truck to check a face area, when EE turned around to get back in truck EE felt a pain in calf. No medical treatment was given until physical therapy started on Dec 15.

Employee stated was prying on a rock while hand scaling and felt a pain in back.

Employee reported feeling numbness in hands and wrist after hand scaling. Medical treatment on 12/14/2017 resulted in light duty.

The employee stated employee was picking up a belt roller and felt a pop in employee's back.

The employee was working on damaged hydraulic cylinder, attempted to move cylinder. The cylinder moved easier than anticipated pinching finger between cylinder and frame.

Employee walked to scale printer to clear a paper jam and slipped on the snow. The area around the scale house had been salted however it was still snowing and the path they took to the scale printed has snow accumulated on it.

The employee was operating a haul truck, hit a hole in the road and jarred neck.

The employee stated was installing a roof bolt when EE felt pain in left shoulder.

Employee was shoveling, cleaning work area, felt pain in right shoulder. Employee was put on restricted duty on July 7 after diagnosed with damage to rotator cuff.

The employee experienced back pain over a period of time while operating loader underground. Employee reported the injury on June 7th and requested medical treatment, work restrictions were issued on June 8th 2016.

The employee pick up a tool turned around to take a step, felt knee pop and fell to the ground. (The employee is now receiving Physical Therapy.)

The employee stated was climbing into the truck, EE's foot slipped off the step and struck left shin. Employee went to the doctor on 4-6-16 and was given restricted duty.

The employee was climbing up the ladder on a haul truck and felt pain in EE's knee. Surgery on 9-8-16 made injury reportable.

The employee bent over to pick up a wheel chock and felt pain in his lower back.

The employee was manually scaling and bumped elbow on manlift basket handrail.

The employee was trying to unplug the screening hopper with the use of a 15 foot pipe and sledge hammer. The employee struck the pipe which came back and hit his finger. No medical treatment requested until 11-25-14 at which time he was diagnosed with a fracture.

The employee was installing roof bolts, he turned to grab a bolt and felt a pop in his knee. The employee was on full duty until he required surgery on 2-20-15 at which time he was placed on restricted duty.

The employee was hand scaling, the bar slipped and pinched his finger.

The employee was climbing into the haul truck, as he stepped on the lower step, which is designed to be flexible, it shifted causing additional strain to his left arm.

The employee lifted a chute to clear it and returned the chute down to the normal position. When he removed his safety harness he felt pain in his left shoulder. The employee was given restrictions on 9-11-14.

The employee was removing a 8 inch block, by pushing it with his foot, from under the front wheel of a 773 Haul Truck that had been loaded on a trailer.

The employee stated he bent over to pick up a hitch pin and felt a pain in his low back.

The employee reported shoulder pain after shoveling and having removed a panel during his shift. No specific incident to contribute to the injury.

The employee was moving a chute and felt a pop in his right shoulder.

The employee was changing out screens and strained his right shoulder.

The employee was pushing a squeegee when the squeegee stopped suddenly causing a sprain to the wrist.

The employee was scaling, a rock fell pushing the scale bar down pinching his finger between the bar and the top railing of the man basket.
