The employee leaned a cume-along against the rib. He thought it was secure, When he walked away, the cume-along fell and struck him on the right shoulder.

EMPLOYEE WAS HOLDING A PIPE WRENCH WITH HIS LEFT HAND PLACED ON A D-D NUT IN AN ATTEMPT TO LOOSEN IT BY SWINGING A SLEDGE HAMMER WITH HIS RT.HAND. ON HIS 3RD ATTEMPT HIS SWING MISSED THE PIPE WRENCH, HITTING HIS LEFT THUMB.
EE WAS TIGHTENING A ROD WITH A 36" PIPE WRENCH. HE SOMEHOW HIT THE ROTATION LEVER IETHER FELL INTO IT OR HIT IT WITH THE PIPE WRENCH CAUSING HIS HAND TO GET PINNED BETWEEN THE WRENCH AND THE DRILL HEAD MOTOER
Employee was in the act of removing an o-ring from a check valve with an o-ring pick. The pick slipped & punctured his rt. middle finger tip. Treated w/first aid on 3/14/05. EE went to the Dr.'s office & was treated for an infection on his rt. hand, middle finger tip. He received a prescription for antibiotics.
THE CRUSHER (CONE) PLUGGED. AFTER L.O.T.O. THE OPERATOR WAS USING A BAR TO DISLODGE THE PLUG. HE NOTICED THAT HIS HAND WAS STINGING. HE TOOK OFF THE GLOVE AND SAW THE INJURY. DOESN'T KNOW HOW IT HAPPENED. (HIT BY ROCK OR THE BAR.) TAKEN TO TOWN FOR FIRST AID. X-RAY'S FOUND FINGER TO BE FRACTURED.
MINER WAS TRIMMING PLASTIC PIPE WITH A UTILITY KNIFE, THE KNIFE SLIPPED CUTTING THE MINERS WRIST. THIS IS NORMALLY DONE WITH A HACKSAW.
EE WAS TIGHTENING A BOLT WITH A 2' DRIVER USING A JACKLEG WHEN THE LEG CAME DOWN TOO FAST. THE EE STEPPED BACK OUT OF THE WAY TRIPPING AS HE DID SO LANDING ON HIS BACK. THIS OCCURRED ON 8-6-03 AND THE EE DID NOT SEEK MEDICAL ATTENTION UNTIL 8/18/03.
WHILE SCALING, LOOSE CAUSED SCALING BAR TO HIT EE SAFETY GLASSES WHICH CUT HIS L EYEBROW.
