Chute was plugged with large blocks of coal at crusher. A slate bar was being utilized with a sledge hammer to break the material free. Sledge hammer glanced off of bar being used and struck victim in right forearm resulting in cracked ulna bone.

EMPLOYEE WAS TRIMMING A BELT SPLICE AT THE #3 HEAD DRIVE WHEN HE SLIPPED AND THE KNIFE HE WAS USING WENT ACROSS HIS THUMB ON HIS LEFT HAND RESULTING IN A LACERATION REQUIRING STITCHES.
EE (ELECTRICIAN) WAS USING A PIN PULLER TO PULL A PIN ON A 735 REFUSE TRUCK, WHEN THE BOLT ON THE PULLER BROKE AND STRUCK EE ABOVE THE RIGHT EYE, REQUIRING SUTURES. EE RETURNED TO WORK THE NEXT SCHEDULED SHIFT.
Employee was cutting a piece of ventilation curtain with his knife and accidently cut his forefinger.
EE stated he was using a sledge hammer, when he felt a pain in his back. At first this was classified as an ongoing injury. After an investigation, this injury has been determined by worker comp. to be a new injury. EE has been off work due to heart surgery.
While replacing tie rod on 777 rock truck employee was hitting ball stud with a 5 lb. hammer and the bearing busted. A piece of bearing struck him in the lower abdomen.
EE was removing the panel board on a roof bolt machine and while pulling on his wrench it slipped which resulted in a fracture to his little finger on his right hand.

While using a pry bar to place a belly pan back on D-11 dozer the bar slipped striking ee on left ear.
Employee was in process of changing the y-pin on 992G cat loader when the tool that presses the seal in place fell and trapped his thumb between the loader tire and the press tool. This resulted in a mashed thumb and employee had outpatient surgery for damage to thumb on right hand. (Worked two days then took off).
Employee was putting a lift jack on a 992-G loader when the straps holding the jack broke causing the jack to fall and strike him on the right foot.
EE WAS USING A HAMMER TO BUST A PIECE OF ROCK ON TOP OF THE CONTINUOUS MINER. A PIECE OF ROCK FLEW OFF STRIKING EE IN THE GROIN AREA.
