We have experienced a non injury unintentional roof fall outby the inactive section. The roof fall occurred on 2 South Main in the #6 entry (intake) in an intersection at cross cut #14. laminated shale gave way from deteriorating. The approximate size of the fall is 5 feet thick, 35 feet wide and 20 feet long.
This mine has experienced a non-injury un-intentional roof fall outby the working section. The fall occurred on the intake of Six North which leads to 001 section. The fall is approximately 9 feet thick, 60 feet long and 20 feet wide. This area was initially supported with 60 inch fully grouted rods and 8 foot cable bolts. Laminated shale gave way from deterioration.
The above mine has experienced a non-injury unintentional roof fall in the outby areas of the mine. The roof fall occurred on the first left panel on nine South Mains which is in non-producing status. The fall dimensions were approximately 38 feet long, 20 feet wide and 7 feet thick. The roof strata was shale and initially supported with fully grouted resin bolts.
The injured sprained his right knee when he slipped while pulling a piece of ventilation brattice cloth out of mud. The employee worked until 6/14/2012 when this injury become a reportable injury. Further medical examinations revealed a tear in his right knee that has to be surgically repaired. Lost time began on (6/14/2012).
Flint Ridge Mine #2 has experienced a non-injury unintentional roof fall approximately 2000 feet outby the 002 section in the return of 4 Main in #1 entry. The fall was approximately 8 feet thick, 20 feet long and 50 feet wide in an intersection. The area was initially supported with 60 inch fully grouted rods. Laminated shale gave way due to deterioration.
Employee was assisting roof bolter operators conduct a methane test by the use of an extendable probe when a piece of mine roof fell from between 2 steel channels,(that were supported with 60" fully grouted rods), striking his right foot, causing a crushing injury to the tip of his big and third toe. The piece of mine roof did not appear to be loose on initial examination.
The employee was lifting a de-watering pump into a supply truck when he felt a sharp pain in his right shoulder. The employee continued to work until further medical examinations revealed a tear in his right shoulder that must be surgically repaired. This injury become a lost time injury on 02/06/2012.
The injured was changing a bearing on a 2 person rubber tired man trip in the mine shop on the surface. The injured was beating the bearing into the housing with a header board and a hammer when the board bounced up and caught his right index finder between the hammer handle and the board. The laceration required 4 stitches.
The injured was riding a rubber tired mantrip on his way to his work site when the mantrip dropped into a hole in the roadway on 4 South. The mantrip bounced causing the injured to bounce up and down in his seat dislocating the tip of his tail bone.
A non-injury unintentional roof fall occurred on 2 North intake for MMU 002-0 section. The fall was in #8 entry at cross-cut 48 on 2 North Mains in an intersection. The fall was approximately 10 feet thick, 70 feet wide and 20 feet long. This area was initially supported with 60" fully grouted rods.
The employee was helping install 6 inch discharge pumping water line when he slipped and fell breaking his left arm.
Flint Ridge Mine #2 has experienced a non-injury un-intentional roof fall approximately 4,500 feet outby the 003 working section on the intake airway in #6 entry at cross-cut #6 on 2 South Mains. The fall dimensions are approximately 30 feet long, 40 feet wide and 4 to 6 feet thick with laminated shale.
A non-injury un-intentional roof fall occurred (outby) on 1 North Main (return) in #2 entry between cross-cut 16 and 17. The dimensions are approximately 40 feet long, 20 feet wide and 6 feet thick. Rod material is laminated shale. The area was initially supported with 60 inch fully grouted resin rods.
Flint Ridge Mine #2 has experienced a non-injury, unintentional roof fall outby the working section. The fall was discovered on #1 Main at cross-cut 52, #9 entry/intersection on return. The fall is app. 20' long by 20' wide and 5' thick. Laminated shale had deteriorated. The area was initially supported with 48" fully grouted rods.
Initially EE strained his upper back while lifting a box of resin on 02/28/2011. EE worked until March 09, 2011 when his strain become a lost time injury. This injury become reportable on 03/09/2011
The employee was attempting to throw a top conveyor belt roller underneath the belt when the roller struck him on the side of his left foot.

Flint Ridge Mine #2 has experienced a non-injury un-intentional roof fall outby the 003 working section at cross-cut #42 along 2 South Main in #3 entry, (neutral).The roof fall dimensions are approximately 7 feet thick, 35 feet long and 30 feet wide.
A non-injury unintentional roof fall occurred at the mentioned mine approximately 1000 feet outby the 002 active section. The fall occurred in a neutral entry (#4) in an old roadway intersection on #5 Main. The fall was app. 7 to 8 feet thick, 26 feet wide and 25 feet long. Laminated shale gave way which was supported with 60 inch fully grouted rods.
While operating a Continuous Mining Machine on MMU 001-0 section in #8 right x-cut (punch thru), EE was struck on top of his shoulders with a piece of mine roof that appeared to be secured to the mine roof, became loose while mining #8 x-cut into #9 entry. The piece of rock fell from between permanent roof support and the coal rib.
An employee was dropping a cable and water line that was hung with a cable hanger with a metal hook, the hanger slipped causing the hook to cut his right hand, which required 3 stitches.
This report was filled out on 10/18/2010 but somehow the on line filing system did not accept the report. The employee was injured when a man trip he was riding overturned on the surface at an underground mine.

The mentioned mine has experienced a non-injury un-intentional roof fall outby the 002 working section located between #7 and #8 entries approximately 15 cross-cuts outby the working section.The fall is 20' wide, 25' long and 6' high with laminated sandston and shale. The are was initially supported with 60" 3/4 fully grouted rods.
The employee cut his right little finger when it was caught between a hammer and the metal frame of a roof bolting machine while splicing a tram chain.
The employee picked up a 40 pound bag of rock dust and strained his lower back on 4/5/10. this injury became a lost time reportable on 4/8/10
While the employee was using a utility knife to cut mining conveyor belt, his knife slipped and cut his right leg, requiring 3 stitches.
Flint Ridge Mine #2 experienced a non-injury unintentional roof fall on the MMU 001-0 section while retreat mining. The caved area on the Continuous Mining Machine was approximately 5 to 7 feet thick, 11 feet wide and 15 to 20 feet long. There was no damage.
EE was tramming the continuous mining machine across the 003 section, when he stepped in a hole in the mine floor and twisted his right knee.
Injury initially sprained his right knee on 2/15/10 lifting mine belt for set up and has worked until today, date (3-2-10) he has become a reportable lost time injury after an MRI revealed some tissue damage in his right knee.
The employee was attempting to hang an electrical cable with a cable hanger with a metal hook when the cable slipped, puncturing the employee in the right middle finger with the metal hook which required 3 stitches.
a non-injury unintentional roof fall occurred while retreat mining on 001-0 working section in #1 entry on 2nd cut of pillar block #1. The roof fall wedged the continuous mining machine between rock and the coal rib. app. 3 ft. of rock on the machine, laminated shale and sandstone.

Employee punctured his right hand with a magnetic pocket screw driver when he was attempting to dislodge a bolt from his tool socket. The puncture wound was lanced and cleaned requiring 3 stitches.

A piece of mine roof app 2 feet thick, 20 feet wide and 12 feet long fell on the MMU 002-0 retreat mining section in the first lift of initial #8 pillar block. No injuries to this occurence.
The employee was operating a 2 person personell carrier, when he ran over some gob in the roadway. The steering wheel turned sharply, bending his right hand back, breaking his ring finger on right hand.
Due to exesive rain fall in a short period of time, a mud slide occured at no 1 portal, partially blocking Left return.
Due to excessive rain fall, the drains near the mine portals overflowed and allowed water to flow into the mine. No one was injured or trapped. We ditched the water away from the protals and pumped water from the mine. All the employees were withdrawn from the mine immediately.
THE EMPLOYEE WAS USING A SHROTER DRILL TO DRILL ANCHOR HOLES IN THE MINE FLOOR, WHEN AN OIL SEAL BLEW ON THE DRILL, SPRAYING OIL INTO HIS EYES.

The fall is approximately 30' long, 25' wide and 10' to 12' thick. Roof is shale which gave way. 4' fully grouted rods was initial support.
EE was going to the continuous miner to get a load of coal, when he struck the coal rib with the shuttle car he was operating. He struck the canopy with his head, springing his neck.

An unintentional roof fall occurred at Flint Ridge #2 mine outby the active sections on #2 main line belt line at cross-cut #12. No injuries, some damage to belt structure. The fall area strata is laminated shale and sandstone. Fall was app. 8 feet thick, 20 feet wide and 60 feet long.
CONTINUOUS MINER OPERATOR WAS ATTEMPTING TO REPLACE A METAL GUARD BACK ONTO THE CONTINUOUS MINER USING A SLATE BAR WHEN HE FELT PAIN IN THE MIDDLE PART OF HIS BACK.
A ROOF FALL OCCURRED ON 8-5-2008 ADJACENT TO THE #3 BELT IN THE #7 ENTRY AT THE 34 CROSS CUT. THE FALL WAS APPROXIMATELY 20' WIDE 50' LONG AND APPROXIMATELY 5' THICK.
ELECTRICIAN WAS TIGHTENING A WATER LINE BAND WITH A BANDING TOOL WHEN THE BAND BROKE CUTTING HIS LEFT HAND.
AT APPROXIMATELY 2:00 PM A ROCK FALL WAS DISCOVERED IN THE NUMBER FOUR (4) ENTRY APPROXIMATELY 70' LONG 20' WIDE AND ABOUT 4.5 FEET THICK. THE ROOF WAS SHALE AND APPEARED TO BE A HORSE BACK WITH SLICK SIDES.
EMPLOYEE WAS TRIMMING A BELT SPLICE AT THE #3 HEAD DRIVE WHEN HE SLIPPED AND THE KNIFE HE WAS USING WENT ACROSS HIS THUMB ON HIS LEFT HAND RESULTING IN A LACERATION REQUIRING STITCHES.
EMPLOYEE WAS PULLING A SET OF BATTERY JUMPER CABLES AT THE CHARGING STATION WHEN THEY HUNG CAUSING HIM TO FALL AND HE STATED WHEN HE GOT UP THAT HE EXPERIENCED PAIN IN HIS MIDDLE AND LOWER BACK. HE STARTED MISSING ON 6-18-2008.
Electrician alleged that he was throwing a leveling chain on the roof bolter when he lost his footing and fell straining his left hand.
WE HAD A ROOF FALL OUT BY THE FEEDER AND WHILE DOING THE INVESTIGATION, WE FOUND ANOTHER FALL 6 BREAKS OUT BY THE FIRST. BOTH FALLS ARE IN LOW COVER AREAS.
WE HAD MINED UNDER LOW COVER AND HAD ENCOUNTERED ADVERSE ROOF CONDITIONS. ROPE BOLTS HAD BEEN INSTALLED TO CONTROL THE ROOF WITH OUT SUCCESS.
Roof Bolter Operator was in the #7 entry and was in the process of installing a roof bolt when his drill steel hung up. When he reached over to pull the steel out of the hole, a piece of draw rock fell from the roof and struck him on the right hand.
EE bent over to move the miner cable, when he felt pain in his lower back. EE started missing work on 2/11/08.
The mine experienced an unintentional roof fall outby the active section on the parallel leatherwood mains at crosscut #45. The fall is approximately 160' long & 5' to 6' thick. The roof in the area is layered shale. This area had been permanently supported with 4' fully grouted rods. The fall extended thru the intersection from #3 to #6 entries.
Employee was in the process of removing a steering jack from a DBT Ram Car when the jack slipped catching his middle finger on his left hand between the jack and the frame of the car.
A rock fall was found in the #3 entry at the 36 break. This fall was approximately 5 feet high, 20 feet wide and 20 feet long. Area supported with 4 foot roof bolts and 8 ft. cable bolts was spotted in the area.
A rock fall was found in the #4 entry at the 42 break adjacent to the #2 belt. This fall was approximately 5 feet high, 20 feet wide and 80 feet long. This area was in a low cover area supported with 4 foot roof bolts. The top is shale and is layered.
EE was removing the panel board on a roof bolt machine and while pulling on his wrench it slipped which resulted in a fracture to his little finger on his right hand.
EE stated that he did not stop his ram car quick enough. He ran into the feeder and hurt his lower back. He did not start missing work until Monday February 5, 2007.
EE was pulling a bent roof bolt out of the top before the glue had time to set up when he felt pain in his lower back. He did not start missing work until 10-3-06.
