Employee was beating on the outside of a raw stone bin trying to dislodge the frozen material. He was using a sledge hammer and scaling bar which caused pain in his right hand. The initial report of this injury was made on 12-19-08, the injury did not become reportable until 2-5-09 when a prescription was filled.

Mostrando los 5Employee was using a hand wood chisel to create a space to install hinges on a wooden door. The chisel slipped striking the employee's right thumb.
Employee was cutting electrical tape from electric motor wires and knife slipped cutting his two fingers on the left hand. One stitch to middle finger.
Employee was using a pry bar to re-postition a mill cradle. The pry bar slipped out of cradle, thus causing the opposite end to slide through the employee's right hand. This punctured the skin between the base of the thumb and the base of the finger.
Installing a pipe on the elevator to the dust collector. The pipe was tack welded in place. EE was pulling on the pipe with a comealong to line it up. When the weld broke and the pipe fell hitting his finger between the pipe and the elevator. 12 stitches.
