Carrying out maintenance on a Sandvik 431 Cable bolter. removing built up grout from bolts was a hammer and bar. Hammer handle has broken mid swing personnel has hit hand on the bar and machine causing an acute fracture to middle finger on right hand.

An UG maintenance mechanic was using a soft steel hammer to break free a bucket tooth (G.E.T.) from a 9 yard loader bucket. When the soft steel hammer made contact with the tooth, a small metal shard sheered off and imbedded in the employees right leg calf muscle.

Employee was using a small crowbar to pry off bearings to prepare to line bore. Crowbar popped off the bearing and the bearing fell off striking the employee on the inside of the left knee.

Injured worker was attempting to cut through a seam weld on existing geomembrane liner with a utility knife when the blade slipped loose from the liner and struck the top of the thumb on the opposite hand that was holding the knife. The hooked blade penetrated the worker's leather glove. Several sutures/stitches were required to close the wound.

While attempting to pry bent ladder on PC4000 over to lower it to the ground. Technician 1 was using a 36" pry bar, it slipped and fell approximately 6 feet striking Technician 2's (IP) hard hat and mouth, splitting both IP's lips resulting in 6 stitches total, 4 in the upper lip, 2 in the lower lip. Doctor cleared IP to return to work on full duties.

A driller sustained a strain/sprain injury to the right upper arm while assisting another driller in breaking a bit free from the drill hammer. Die blocks of the breakout jaws were covered in buildup, preventing the jaws from opening fully. A soft faced sledgehammer was used to break the bit free. While swinging the sledgehammer, the assisting driller felt discomfort in elbow.

Maintenance Technician sustained a laceration to the left hand fingers from sharp metal.

The right pinky finger was caught between pipe wrench and drill rod guard on exploration drill causing a fracture and laceration requiring stitches.

Employee, in a kneeling position (right leg down) was using a spring loaded, self-retracting utility knife to cut a zip tie. As the cut was made, the knife slipped, causing a laceration to the left shin. Employee received 6 sutures to close the wound, and returned to work immediately.

Operator was clearing the crusher discharge, rock released from above contacting the 5' steel mill bar operator was working with and it hit them in the face/mouth area.

The employee was working on the slope detailing the geocomposite, the weather was overcast with a temperature of 70°. As EE went to make a cut with the straight blade EE stabbed EE's leg with the tip of the blade. EE's co-workers helped administer first aid.

Process Operator was clearing a chute blockage underneath the Jaw Crusher using a metal bar when a rock became dislodged and struck the operators finger causing injury.

Employee was unplugging a rock chute with a steel pry bar when the bar kicked back, striking EE in the right jaw, lacerating it. Two sutures were required to treat the laceration.

While driving a t-post with a driver, the driver slipped off the post, striking the employee in the head, knocking off employee's hardhat. Employee was diagnosed with a contusion and lost two workdays as a precaution.

Employee was attempting to remove teeth from a loader bucket. When they struck a stuck tooth with a soft-face hammer, a piece of the tooth broke off, lodging in their left leg. Employee required surgery to remove the piece of metal.

Improper use of knife to remove insulation from wire-out finger & required 4 stitches.

Laborer was using a t-post driver to drive a t-post. EE raised the driver up too high as ee was attempting to drive the post, missed the post as EE brought the driver down, and hit leg with the driver, which resulted in a small laceration that required two sutures.

EE was getting set up to use a threading machine for some conduit. EE was adjusting the machine & finishing up by tightening the adjustment nut using an adjustable wrench. The wrench came off the nut & the forward motion caused EE's hand to hit the reamer attachment, causing a cut to middle finger on right hand. EE received 9 stitches as a result.

On July 31, employee broke a rock core sample with a hammer. A rock sliver punctured EE's finger. On August 1, EE reported the incident to the site nurse, who referred EE to a doctor. The physician opened the wound to remove the sliver, and then closed the incision with one suture and covered with a band-aid. EE was prescribed antibiotics and returned to work the next day.

The employee was sawing a piece of core/rock and alleges he felt something fall into his left eye.
