During crusher maintenance that involved rotating crusher blow bars, one employee was using a sledgehammer to dislodge the blow bar wedges. While striking the wedge, the employee missed and inadvertently struck the rotor. As a result, a metal fragment broke off and struck a nearby employee in the chest.
