Employee at 2 East Belt take-up found rope fouled in sheave. While using 3/4-ton chain ratchet on roof bolt plate, the plate eye broke, causing a fall and left knee injury. Later diagnosed with torn ACL. Employee continued normal duties until 8/26/2025, when they went off for surgery. First missed shift 8/26/2025. Claimed as NFDL.

Employee stated that EE struck EE's right forearm and right elbow on a slate bar while positioning a trickle duster. EE continued working EE's full shift. The employee continued to work, completing EE's normal work duties until 2/13/25, when EE began missing work.

Employee was cutting sprocket chain that was fouled on the offside drive axle to install a new intermediate shaft when the chain flew apart as it was being cut. The chain struck the employee on the left middle finger, causing a laceration. Employee received 6 sutures to close the wound and was given a full release to return to work on next scheduled shift. No Days Lost

Employee was using utility knife to skin back outer jacket of lifeline to connect to mine phone when the knife slipped, causing EE to lacerate their left thumb. Employee sought medical attention where EE received 3 sutures to close the wound. Employee was given a full release with no restrictions to return to work on the next shift, performing EE's normal duties. No Days Lost.

Wet, muddy conditions on slope. Employee was leveling a railroad track and was removing the jack used for leveling. When the employee released pressure on jack the bar slipped and contacted EE's left arm causing a laceration

Employee was building a stopping while using a ladder. When they struck the wedge to secure the stopping, the stopping fell into the ladder, causing them to fall onto a pallet of block. Employee sought medical attention and was diagnosed with a fractured L-1 and L-5 Vertebrae. This is a Lost Work Days Incident.

Employee was digging out gob from around the track with a pick hammer when a piece of rock broke off and hit the employee in their left front tooth, causing the tooth to partially break. No Days Lost - Medical Reportable Only.

Employee was prying on stub shaft with a slate bar while another employee was tapping on the stub shaft with a sledge hammer. The employee missed the stub shaft and struck the foot shaft which resulted in a piece of the foot shaft breaking off and entering the employee's left bicep. The employee later had the metal removed and received 3 sutures to close the wound. Medical Only.

Employee was installing a bit insert on a continuous miner and was striking the insert with a pick hammer when a piece of the hammer face broke off and embedded itself in EE's left pointer finger. Metal was removed from employee's hand by a surgeon on 12/29 and sutures were used after the procedure. No workdays were lost.

Employee was changing a bearing on a slinger duster with a chisel and hammer when a small piece of metal shot from the bearing and entered EE's right thumb. Metal was removed by a surgeon and sutures were then required at that time due to incision.

Employee was assisting crew with the installation of a section of 18" polyurethane pipe using two chain falls when the load shifted and hit the employee who was standing adjacent to the pipe on the hard hat, jamming the employee's low back.

Employee was making a belt splice and was cutting belt with a utility knife when the knife slipped and struck left thumb causing a laceration resulting in 4 sutures.

Employee was busting up rock in the walkway using a pick hammer when a smaller piece of rock broke off when EE hit the rock striking EE's left forearm which resulted in a laceration that required two sutures.

The employee stated EE's right hand was resting on the ground and was struck by a lifting jack that had been lifting a rover to place back on the track. The employee was diagnosed with a fractured thumb, index, and middle finger. This incident is being reported as a lost time incident.

Employee was cutting a band from a bundle of blocks with a utility knife and cut left forearm in the process. Employee received sutures to close the laceration.

The employee stated as EE was trying to get the 4-North belt back under the skirting when the bar EE was using slipped out and struck EE across the face. EE sought medical treatment and was diagnosed with a fractured left zygomatic bone and a chipped left front tooth and was taken off work. This incident is being reported as a Lost Time Accident.

Employee was lifting and using a chipping hammer throughout the day and began feeling discomfort in right elbow.

EE stated was taking a hydraulic fitting apart on the 206 miner when trying to loosen the fitting; EE stated wrench slipped off causing EE to strike right elbow on the frame of the miner. EE sought medical attention on 10/9/17 & was placed on restricted duty. *EE sought medical attention again on 10/31/17 & was diagnosed with a chipped bone in elbow.

Employee stated was making a belt splice. While attempting to skive the belt the knife slipped out of the belt and caught the employee on the left ring finger causing a laceration about 1 inch long. Employee later sought medical attention and received 3 sutures and returned to work the following day at normal job.

Employee stated was making a belt splice on 2 South Belt when foot slipped causing the knife to come out of the belt causing a small laceration to right leg. The laceration required 4 sutures. Employee returned to work next shift. This incident is a medical reportable incident.

The employee stated that while in process of skiving a piece of belt the knife slipped out causing a laceration to left hand. The laceration required 3 sutures.

The employee stated that they were cutting the insulation off of a motor tree for a new belt head setup when they accidently cut left index finger. The laceration required 3 sutures, so this is being reported as a medically reportable incident.
