The EE was working inside a dust collector with a few co-workers, attempting to remove a bag/cage (with the help of a cage puller) that was partially full of material. In the process of doing this, the EE had right hand below the cage housing, allowing for their right index finger to be crushed/cut between the cage puller and the cage housing.

The employee was simply walking through the Maintenance Shop holding a box cutter they had just got out of the vending machine. Upon inspection of the box cutter, they noticed a foreign object (possibly glue) on the blade. They proceeded to wipe the blade off on their left sleeve, when they cut/punctured their wrist area.
