Employee was trying to remove a taper lock from a tail pulley using a hammer and punch. A small piece, the size of a pen tip, broke off the punch and stuck in left cheek. It was removed with tweezers and antiseptic wipes.

Employee was pulling and lifting rocks with a hooked tool to remove the jam. Originally diagnosed as a left inguinal strain, returned to normal duty as a truck driver. 7/16/14 follow up doctor's visit indicated aggravation of the injury, employee placed on temporary modified duty with restrictions on driving, lifting and repetitive bending.

2 Employees were attempting to re-attach a bypass chute on a screen plant with the assistance of 2 - 5' steel pry bars. When pressure was released on one bar, it caused the other to fall 48" +/- onto one employee's foot, just missing the steel section of his boots resulting in a contusion/laceration of the right foot and broken second toe.

EE was stripping wire using his utility knife, he was cutting toward his body instead of away from it. The knife slipped causing a laceration to his left thumb.

Employee was replacing teeth on a drag line bucket, loosened retaining bolt and hammered tooth with a steel hammer. A metal sliver splintered off and penetrated skin imbedding in the joint area of his left knee.

Pump used to fill water truck lost its prime. Employee was trying to remove the cap from the top of the pump to reprime it: pipe wrench slipped off and employee fell backwards onto the ground. Suffered a contusion to his left hip and elbow; Xrays show no broken bones. Restricted duty until Monday 09/20/10.

Power outage caused shorhead crusher to plug with rock. After clearing rock the sheave needed to be pryed with a crowbar so the crusher would start. Employee was holding the crowbar when it slipped and jammed his left little finger tip. Fractured left little finger tip and laceration.

A crew was hammering a bearing race to remove it from a shaft and a piece of metal flew off it and penetrated employee's forehead just above the left eye.

WHEN CUTTING A PIECE OF HOSE, THE KNIFE SLIPPED AND CUT HIS LEG.

INJURED ALLEGES THAT WHILE ASSISTING LOADER OPERATOR IN A REPAIR TO HIS MACHINE. THE SCREW DRIVER HE WAS USING SLIPPED CAUSING A PUNCTURE WOULD TO HIS LEFT INDEX FINGER. THE SCREW DRIVER PENET RATED HIS WORK GLOVE BEFORE ENTERING HIS FINGER.

EE WAS SWINGING A 12 LB MALL TO HIT A POUND WRENCH WHEN HE FELT SOMETHING POP OUT OF PLACE IN HIS BACK, CAUSING LOWER BACK STRAIN.

CUTTING A STRIP OF SCREEN CLOTH RUBBER WITH UTILITY KNIFE. WASN'T WEARING LEATHER GLOVES & WAS CUTTING TOWARDS HIS HAND. KNIFE SLIPPED & CUT INDEX FINGER REQURING 3 STITCHES.

UWHILE CLEANING THE CRUSHER, A PIECE OF STEEL GOT INTO HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES.

EE WAS WORKING ON JOY DRILL AT DRILLING SITE IN QUARRY. WHILE TIGHTENING A BLET WRENCH SLIPPED CAUSING MECHANIC TO LOSE HIS BALANCE. HE TRIED TO CATCH HIMSELF WITH HIS RIGHT HAND AND LANDED ON A SHARP ROCK. LACERATED OUTSIDE BASE OF LITTLE FINGER HAND REQUIRING 4 STITCHES.

STRIKING METAL SHAFT WITH HAMMER. FRAGMENT OF METAL PIECED STOMACH AREA. PIECE OF METAL WAS REMOVED AT EMERGENCY ROOM REQUIRING 4 STITCHES.

EMPLOYEE WAS PRYING ROCK OUT OF FEEDER, PRY BAR SLIPPED AND HIT DRUM IN CRUSHER. THE PRY BAR CAME BACK AND HIT EMPLOYEE IN LEFT SHOULDER.

INDV HELPING PUT LARGE BOLTS IN CRUSHER AND ONE SLIPPED AND FRACTURED RIGHT RING FINGER UNDER THE NAIL.BOLTS 20-25 LB EA.

EE WAS TRYING TO DISLODGE A PIECE OF AGGREGATE FROM JAW CRUSHER USING LOADER TOOTH FOR LEVERAGE AND FELT PAIN IN LOWER BACK.

MEN WERE WORKING ON HAMMERMILL.MAN WAS TIGHTENING BOLT WHEN BOLT BROKE AND MAN FELL BACKWARD,LANDING ON HIS BACK ON A ROCK.

CLEANING THE GRIZZLEY ON CRUSHING PLANT HE APP TRAINED L LOWER SIDE OF BACK EMP IN PROCESS OF PULLING STONES OUT FROM BETWEEN THE IRON BARS WITH PRY BAR WHEN THIS TOOK PLACE.

EMPLOYEE WAS CHANGING THE SCREENS ON A SECO SCREEN WHEN THE INCIDENT HAPPENED.WHILE IN THE PROCESS OF INSTAALING THE SCREEN HE JAMMED HIS LEFT THUMG BETWEEN THE CLAMPING BAR AND THE FRAMEWORK.

ROCK BECAME LODGED IN FEEDER ATTEMPT TO FREE ROCK WITH PRY BAR PUSH AGAINST BAR TWISTED KNEE

INJURED WAS REPLACING HOPPER ON CONCRETE MIXER WHEN BAR HE WAS PRYING WITH SLIPPED & STRUCK RT.FOOT BROKEN & LACERATED BIG TOE ON RT.FOOT

EMPLOYEE WAS TI1HTENING SOME SCREEN BOLTS ON A TELESMITH SCREEN. APPRAENTLY WHILE USING A DRIVE RACHET TO TIGHTEN THESE WITH THE EMPLOYEE PULLED A MUSCLE IN HIS LOWER BACK. EMPLOYEE WAS ON G OOD WORKING PLA7FORM THE TIME OF THE ACCIDENT.
