While descending down stairs at the finish end ee slipped and strained a muscle in right leg above knee. After the weekend it did not feel better so ee asked to go to Occ Health and was diagnosed with a torn tight quadriceps. Follow up with Dr. on 11/7/17. To have surgery 11/17/17

Mostrando 50 de 61Subcontractor Trucker felt ill pulled over on road leaving quarry Scale house and was found unresponsive by coworker. EMS was called, driver was pronounce dead.
After doing repairs at the crusher, the employee decided to place caution tape around the hole in the catwalk for the night. At that time, ee tripped on the step at the catwalk, trying to regain balance took a step back and fell into the hole approx. 5' to the top of a motor and then an additional 4' to the ground.
Employee was trying to remove a taper lock from a tail pulley using a hammer and punch. A small piece, the size of a pen tip, broke off the punch and stuck in left cheek. It was removed with tweezers and antiseptic wipes.
Crusher generator had shut down because it overheated. Employee went to the radiator and was going to check the antifreeze level. EE turned the cap to the first notch, while waiting for the pressure to equalize the cap came off spraying employee with hot fluid. This caused burns to left arm, chest, eye lid and around mouth.
Employee, Cruhser operator, was unplugging impactor which was plugged with rocks. He was pulling rocks out and to the side so he could wrap a cable around the largest rock to pull it out when a rock fell down against his right ring finger smashing it open.
Employee was pulling and lifting rocks with a hooked tool to remove the jam. Originally diagnosed as a left inguinal strain, returned to normal duty as a truck driver. 7/16/14 follow up doctor's visit indicated aggravation of the injury, employee placed on temporary modified duty with restrictions on driving, lifting and repetitive bending.
Changing screens on JCI, employee was required to climb up & down and bending and kneeling, employee felt popping in knee when bending, strained knee.
Employee was climbing up and down from a Cat haul truck.
Employee was grinding overhead to re-attach a support ear that had broken off the crusher when a spark from the grinder entered his left eye. The employee was treated in ER and given prescription eye drops and pain medication. He was assigned to regular duty, no lost time from work and no permanent impairment expected.
EE was working on the jaw crusher, he had to seperate the jaw to put the new togal in place , the seat fell out and hit his left hand and cut it.
2 Employees were attempting to re-attach a bypass chute on a screen plant with the assistance of 2 - 5' steel pry bars. When pressure was released on one bar, it caused the other to fall 48" +/- onto one employee's foot, just missing the steel section of his boots resulting in a contusion/laceration of the right foot and broken second toe.
EE was stripping wire using his utility knife, he was cutting toward his body instead of away from it. The knife slipped causing a laceration to his left thumb.
Employee was replacing teeth on a drag line bucket, loosened retaining bolt and hammered tooth with a steel hammer. A metal sliver splintered off and penetrated skin imbedding in the joint area of his left knee.
Employee was grinding on a suspended water pipe outdoors, wind kicked up causing sparks and slag to blow back under safety glasses, a piece of metal lodged in right eye.
Pump used to fill water truck lost its prime. Employee was trying to remove the cap from the top of the pump to reprime it: pipe wrench slipped off and employee fell backwards onto the ground. Suffered a contusion to his left hip and elbow; Xrays show no broken bones. Restricted duty until Monday 09/20/10.
After unplugging the air classifier on the Crusher, employee climbed down and slipped, dropping about 1 foot. He struck his left rib cage against the rail alongside the feed conveyor. He has 3 broken ribs and was prescribed Vicodin as needed. He was wearing proper PPE with a harness and retractable lanyard.
Employee working at finish end of crushing plant, on Eagle Wash Screw, when employee slipped and struck knee.
He was using a grinder with a locking trigger. The grinder slipped out of his hand and hit his forearm.
Employee was grading the haul road with a loader. Loaders bucket hit a rock stopping the loader causing employee to hit his head resulting in lacerations to his head.
EE was using torches to loosen a nut; while trying to turn nut with wrench, the torch flame made contact with the back of his right hand creating a possible 2nd degree burn. EE did not want to seek medical attention at the time; burn was cleaned with antiseptic and burn cream and bandaged. 04/24/09 EE felt the burn was becoming infected and went to the doctor to be treated.
Employee was repairing guard on gravel plant when he ran the torch across his right hand causing a third degree burn to the top of his right hand near the thumb.
Power outage caused shorhead crusher to plug with rock. After clearing rock the sheave needed to be pryed with a crowbar so the crusher would start. Employee was holding the crowbar when it slipped and jammed his left little finger tip. Fractured left little finger tip and laceration.
Employee alleges that he pulled a groin muscle while working on a generator in the Walpole Quarry.
While starting 95hp water pump employee pressed contact button on starter. Arc flash occurred. Second degree burns to right hand.
While cleaning out materials from truck body, employee slipped on mud and banged his knee causing a knee sprain.
Employee walking on dirt road with machete. No sheath on blade and no arm guards.
Driver was hauling aggregate from crusher to stockpiles. Driver stopped to get out of truck, and while exiting caught left foot between step and fuel tank cap that is just behing the step. Driver twisted left knee. Left knee and calf strain.
Employee was operating the John Deere 317 skid steer loader. He pulled back on the handle to back up and hit his right elbow (funny bone) on the arm rest. The foam covering the metal frame had come off over the years and bare metal was exposed. He said it hurt for a little while then was alright. He refused medical treatment until 6/13/07.
Employee was walking to another operator when he slipped on concrete cream and sprained his left ankle.
Blocking placed on railing before the blocking was secured and it fell striking EE on top of the hard hat. EE refused medical treatment until 10/03/2006 when he was sent to Healthworks--no action taken at that time. On 10/24/2006 Dr. placed EE on Physical Therapy.
While preparing to make crusher repair with rented crane and operator, employee on ground was struck by wood blocking being thrown from crane. Employee was reaching for gloves left on grownd when hit by blocking. Employee has hematoma-contusion to left lower front of leg. Employee on light duty work, no loss of time.
Thunderstorms were in the area, approx 3:45 pm quarry shut down & mains turned off. Employee had just finished & sat in a chair by door to wait out storm, lightning came in the power lines & destroyed the spare breaker box by door, employee jumped out the door and landed on right side & shoulder.
A crew was hammering a bearing race to remove it from a shaft and a piece of metal flew off it and penetrated employee's forehead just above the left eye.
Ee reached into a box to pull out some twin and ran his hand over razorblade that had been left there to cut the string. received laceration to fourth digit of left hand. required four sutures to close wound. no loss time. no restricted duty due to injury
Employee was stepping up on a ladder to a service truck to fuel equipment in the quarry when he lost his footing and slipped which resulted in his knee banging against the railing of the ladder.
EE WAS ON A LOADER WITH THE OPERATOR ABOUT MOVING SOME STEEL. ON DESCENT FROM THE LOADER, THE EE STUBBED HIS HEEL ON SOME STEEL THAT WAS ON THE GROUND NEXT TO THE LOADER, FALLING BACKWARD WITH HIS LEFT ARM EXTENDED BEGIND HIM. HE FELT PAIN IN HIS LEFT SHOULDER.
STEPPED ON 3" ROCK & TWISTED KNEE, STRAIN WITH PULLED LIGAMENTS
THE CRUSHER WAS STOPPED BECAUSE OF A ROCK STUCK IN THE TAIL PULLEY. EMPLOYEE HELPED PRY THE ROCK FROM THE PULLEY AND THEN RESUMED OPERATING THE LOADER UNTIL THE END OF HIS SHIFT. HE NOTICED SO ME STIFFNESS THAT NIGHT AND STATES HE HAD PAIN ON WAKING THE NEXT MORNING. AFTER TWO HOURS OF WORK THE PAIN WAS SO BAD HE COULDN'T CONTINUE & WAS TAKEN FOR MEDICAL EVALUATION.
AN OVERSIZE ROCK GOT CAUGHT IN THE JAWS OF THE CRUSHER. EE HAD TO STAND IN THE JAW TO GET A CABLE AROUND THE ROCK SO IT COULD BE REMOVED. WHILE HE WAS IN THE JAW THE ROCKS SHIFTED, AND ONE STR UCK HIM ON THE DORSAL INSTEP OF HIS LEFT FOOT.
EE WAS CLEANING FLOOR OF QUARRY AFTER BLAST WITH LOADER, IN THE PROCESS OF DOING THIS HE RAN OVER A LARGE ROCK WITH THE LEFT FRONT TIRE AND WHEN THE ROCK SLIPPED OUT FROM UNDER THE TIRE IT JAR RED THE OPERATOR'S LOWER BODY, TWISTING HIM & HE THOUGHT HE STRAINED HIS STOMACH AREA/RIB CAGE.
INDIVIDUAL WAS SHOVELING DIRT AWAY UNDER FEEDER. AS HE LIFTED AND TWISTED HE PULLED LOWER BACK AREA.
EE'S LEFT HAND CAUGHT IN CONVEYOR BELT AT HEAD PULLEY. EE CLIMBED OVER A TWO RAIL BARRIER TO REMOVE ROOT, WITHOUT FIRST SHUTTING OFF & LOCKING OUT CONVEYOR SYSTEM. LEFT HAND RECEIVED BRUISING & ABRASIONS. MEDICAL IMPAIRMENT IS UNDETERMINED AT THIS TIME.
EE ALLEGES HE MAY HAVE STRUCK HIS ELBOW WHILE TRYING TO UNPLUG CRUSHER. HE WENT TO THE HOSPITAL ON 9-8-02. ARM SWOLLEN AND INFECTED. HOSPITAL COULD NOT DETERMINE CAUSE, COULD HAVE BEEN AN INSE CT BITE. EE DID NOT ALLEGE WORK RELATED UNTIL 9-24-02. EE QUIT WHEN TOLD WE WOULD CONTEST THE INJURY AS WORK RELATED.
EE WAS OPERATING A FRONT END LOADER THAT OVERHEATED. HE REMOVED THE RADIATOR CAP BEFORE ALLOWING THE EQUIPMENT TO COOL.
EMPLOYEE WAS DRIVING A DUMP TRUCK WORKING UNDER CRUSHER HAULING 1" STONE. THE TRUCK INVOLVED HAS A DUMP BODY WITH A TAILGATE WHICH IS RELEASED BY A PULL HANDLE LOCATED ON THE FRONT OF THE BODY BY THE DRIVER'S DOOR. SHE OPENED THE DOOR, REACHED OUT WITH LEFT HAND TO PULL HANDLE TO RELEASE GATE TO DUMP LOAD, STRAINING HER LEFT SIDE AND BACK.
EE WAS ENTERING THE WELL HOUSE AT THE CRUSHER, AND STARTLED A STRAY DOG THAT WAS IN THE WELL HOUSE. THE DOG BIT EE ON THE RIGHT THIGH.
GRINDING ON AN OLD WELD UNDER A TRUCK IN A VERY RESTRICTED SPACE. A PIECE OF METAL BOUNCED OFF AN AREA OF THE VEHICLE AND INTO HIS EYE. HE WAS WEARING SAFETY GLASSES AND FULL FACE SHIELD. HE D ID NOT SEEK MEDICAL TREATMENT UNTIL THE FOLLOWING WEEK WHEN HIS EYE BECAME IRITATED.
DRIVER WAS GETTING OUT OF HIS SEAT AND TURNING AROUND SO HE COULD EXIT THE TRUCK, FELT SOMETHING POP IN HIS LEFT KNEE JUST AS HE WAS BEGINNING TO LOWER TO THE FIRST STEP.
LOADING EUCLIDS IN THE QUARRY, RIGHT EYE SUDDENLY BEGAN HURTING. FLUSHED EYE, BUT THAT DID NOT HELP. SENT TO THE EMERGENCY ROOM. EE WAS WEARING SAFETY GLASSES & IN ENCLOSED LOADER CAB.
