Employee was using a 3-lb hammer to hit a wrench in order to loosen a nut on a threaded rod on the motor mount of the slurry pump. The hammer deflected off a guard onto the back of left hand, resulting in injury. X-ray diagnostics showed a fractured bone.

Employee was assisting in maintenance activities within the shop to loosen a housing on a water pump. The employees finger was struck by the hammer being used causing laceration and fracture to the left ring finger.
Cutting scrap steel in yard, employee stepped on a piece of protruding steel from a cable and suffered minor puncture wound to foot. Tetnaus shot given and prescription to avoid infection.

EE WAS USING A HAMMER TO UNPLUG A MATERIAL CHUTE. EE MISSED HIS MARK. THE HAMMER HANDLE STRUCK THE EDGE OF THE CHUTE, HIS RT. INDEX FINGER WAS STRUCK BETWEEN THE EDGE OF THE CHUTE AND THE HAMM ER HANDLE.

EMPLOYEE WAWS USING A PRY BAR TO LINE UP BOLT HOLES ON A CUTTING EDGE. EMPLOYEE REMOVED ONE HANDFROM THE BAR TO INSERT A BOLT, THE BAR SLIPPED AND HIT HIS UPPER LT SIDE LIP.
