Slabs of concrete were caught in the Jaw. The worker was using a pry bar in an attempt to dislodge the concrete slabs. The worker's left hand was caught between the pry bar and the guardrail around the jaw.
EE was pounding in a wedge with a sledge hammer and the hammer struck the tip of his left index finger causing a fracture and laceration

EMPLOYEE WAS TIGHTENING A SHANK BELT ON A 988G FRONT END LOADER BUCKET USING A 3/4" DRIVE RACH-ET AND SOCKET. THE RACHET BROKE CAUSING EMPLOYEETO FALL DOWN. EMPLOYEE LANDED ON HIS RIGHT HIP & HAS NECK PAIN.
EE WAS POUNDING IN WEDGE AND HIT HAND LT W/HAMMER.
EMPLOYEE WAS PUNDING WEDGES INTO CONE ON THE CRUSHER AND WAS HIT IN THE HIP/ THIGH WITH A HAMMER.
ROCK WAS JAMMED ON CRUSHER FEEDER. EE WAS TRYING TO PRY ROCK LOOSE WITH LARGE PRY BAR WHEN PRY BAR SLIPPED AND EE FELL. HE FELL ON ROCK AND FINGER. SMASHED BETWEEN PRYBAR AND A ROCK.
