EE was cleaning the feed chute. EE repetitively struck and scraped the bentonite off the bottom of the chute. After performing the task EE felt pain in the right hand and wrist. EE did not seek medical attention at the time. EE sought medical attention on 1/21/25. EE was put on restrictive duty on 1/21/25. The diagnosis is possible tendinitis in the right wrist.

Mostrando los 10Employee was trying to operate a rail switch. The rail switch was not operating correctly. Employee used a prybar to try and get more leverage to operate the switch. While pushing on the prybar the prybar slipped and the employee fell and hit their knee on the rail. The knee was swollen and bruised.
Employee was pounding in posts and the post pounder struck the employee on top of the head causing a laceration. Medical treatment for the laceration was one stitch and one staple.
The employee was using a pipe wrench to take a knuckle valve apart. The knuckle valve was secured in a vice. While rotating the pipe wrench, the pipe wrench slipped off the knuckle valve causing the employee to lacerate a finger on metal ducting that was on the work bench. The laceration required six stitches.
Employee was attempting to remove bearings off a shaft. The puller slipped off the bearing housing and hit the employee above the knee. The employee had a sore leg and swelling. The employee did not seek a medical evaluation at the time. On 12/20/2019 the employee received a medical evaluation. On 12/30/19 the employee had a follow up medical evaluation.
Helping maintenance with replacement of lid over the hammer mill, employee used a sledge hammer to align the lid. Metal fragment about 1/4"long and 1/8" thick broke off and penetrated his left arm just below the elbow. The metal fragment was not visible and had to be removed at the hospital requiring a couple of stitches to close the wound.
Employee was removing the bearing housing from the #1 mill fan and bumped the fan housing with pry bar causing dust to fall off of the fan housing and into his eye.
The wrench the employee was using to loosen a nut on the #3 hot bin auger bracket slipped off causing the employee to catch his finger on the edge of the bracket and cut it open. The cut on the employee's left index finger required 6 sutures.
Employee was pushing on pipewrench. Pipewrench popped off of bar. Employee lost his balance and struck the side of the car with the left side of his head.
EE WAS CUTTING SHRINK WRAP OFF OF A PALLET OF EMPTY BAGS WHEN HIS HAND SLIPPED AND WAS CUT BY THE KNIFE.
