Employee was attempting to clear a bagger spout with a metal rod. While checking to see if spout was open, hydrated lime shot out of spout. The lime deflected off the cross member of the seat causing some lime to get into employee's left eye. Employee flushed eye out but after 45 minutes it still was bothering him. E'ee went to emergency room.
Employee was using a prybar to pull off grates. Prybar slipped and hit employee on his chest and left side. Caused bruising and cuts to his chest. Em'ee started work the night of 10/18 at 6:30 p.m.
EE was raking chunks of coal, while raking chujmks he twisted his back to toss chunks into a barrel. Ee felt pain in his lower back.
EE WAS USING A UTILITY KNIFE TO CUT THROUGH AN AIR HOSE ON A RAILCAR. HE WAS PULLING THE KNIFE TOWARDS HIM; THE KNIFE SLIPPED OFF THE HOSE & STRUCK HIS LEFT INDEX FINGER. EE WAS WEARING GLOVES BUT THE KNIFE CUT THROUGH THE GLOVE & CUT HIS FINGER. EE DID SEEK MEDICAL ATTENTION; "SUPER GLUE" WAS USED TO SEAL THE WOUND.
Ee was attempting to dislodge a chunk of lime from the bottom of a baghouse. The hot dust that was hung up poured out onto his left boot. The extreme temperature of the dust caused the boot leather to become burning hot. The heated leather caused burns to his left foot. Ee did seek medical attention on 10-16-04.
EE WAS REMOVING ASH RING FROM #2 KILN WITH A CHISEL. A PIECE OF ASH WENT INTO HIS LEFT EYE. EE DID RISNSE HIS EYE. AFTER A FEW DAYS, HE NOTICED THE EYE WAS STILL RED AND SORE. ON 5-14-04 HE DECIDED TO GET MEDICAL ATTENTION. HE RECEIVED THREE PRESCRIPTIONS.
EMPLOYEE WAS ATTEMPTING TO OPEN THE SLIDE PLATE ON THE GROUND LIME SCREW CONVEYOR. HE WAS POUNDING ON THE PLATE WITH A HAMMER WHEN IT CAME LOOSE CAUSING A BURST OF LIME DUST. LIME DUST GOT INT O BOTH EYES. EMPLOYEE IMMEDIATELY FLUSHED HIS EYES. HE DID SEEK MEDICAL ATTENTION.
EE WAS ATTEMPTING TO BREAK THE MERRICK AIRLOCK WITH THE CHAIN STILL ON. HE WAS USING A PIPE WRENCH, HIS RIGHT HAND SLIPPED OFF THE PIPE WRENCH. THIS ACTION CAUSED HIM TO SMASH HIS RIGHT MIDDLE FINGER. HE RECEIVED A SMALL CUT BELOW THE NAIL AND MASSIVE BRUISING OF THE NAIL.
EE WAS IN THE PROCESS OF "KNOCKING DOWN" KILN ASH. EE TRIPPED OVER AN ASH PILE: HIS LEFT KNEE WAS OVEREXTENDED. MEDICAL ATTENTION WAS RECEIVED.
EE WAS SWEEPING THE PREHEATER FLOOR WITH A BROOM. DUST AND DEBRIS ENTERED HIS RIGHT EYE.
EE WAS USING A UTILITY KNIFE WHILE UNTYING BULK BAGS. WHILE CUTTING THE INSTIDE STRAP, HE CUT HIS LEFT INDEX FINGER. THE CUT REQUIRED STITCHES.
EE WAS USING A SLEDGE HAMMER TO BREAK UP A PIECE OF KILN ASH. WHEN THE HAMMER FACE CONTACTED WITH THE PIECE OF ASH, THE EE FELT SOMETHING POP IN HIS LEFT WRIST AND FELT PAIN.
EMPLOYEE WAS SHOVELING LIME FROM THE HYDRATE ELEVATOR PIT. WHILE SHOVELING, EMPLOYEE FELT A PAIN IN THE LOWER MIDDLE BACK.
EMPLOYEE WAS TIGHTENING A CHAIN COME-ALONG TO REMOVE A COOLER PIPE FROM KILN #2 COOLER. THE COOLER PIPE CLAMP JAW BROKE. THE CLAMP FLEW BACK AND STRUCK EMPLOYEE IN THE LEFT SIDE OF HIS ABDOMEN . EMPLOYEE WENT TO THE EMERGENCY ROOM. CAT SCAN PERFORMED AND MEDICATION PRESCRIBED.
EE WAS PULLING ASH CHUNKS WITH A RAKE. HE NOTICED BACK PAIN. EE WENT TO THE DOCTOR ON 5-18-01.
EMPLOYEE WAS WORKING IN KILN #2 CLEANING OUT STONE AND ASH. HE WA SPULLING A RAKE THROUGHOUT THE DAY. HE FELT OKAY AT 2:30 PM WHEN HIS SHIFT ENDED. FRIDAY MORNING WHEN HE AWOKE, HIS RIGHT SIDE , LOWER BACK MID SECTION AND RIGHT SHOULDER WEREVERY SORE AND STIFF.
EES WERE RAKING ASH FROM KILN #2 GRATES; ASH DUST WENT INTO EE'S EYES.

WHILE AN EMPLOYEE AND THE INJURED WERE LOOSENING UP THE BOTTOM PLATE ON THE MIXER BOX WITH THE VISE GRIPS, INJURED GOT THE PLATE LOOSE AND THE PRESSURE RELEASED OUT AND THE VISE GRIPS FLEW BAC K AND HIT INJURED IN THE LEFT HAND PALM AREA BY THE THUMB, DEEPLY CUTTING HIS LEFT PALM AREA BELOW THE THUMB, STTITCHES WERE GIVEN.
WHILE EMPLOYEE WAS USING A TIN SNIPS TO CUT THE METAL BANDING ON THE BOXCAR, THE METAL BAND AFTER BEING CUT SNAPPED UP AND CAUGHT INJURED IN THE RIGHT HAND ON TOP OF THE THUMB AREA CAUSING A D DEP CUT. HE REQUIRED 6 STITCHES TO THIS AREA.
EMPLOYEE WA HELPING CLEAN OUT THE GRADING ON THEDOUBLE BELL DECK. WHILE POUNDING THE ROCK THROUGH THE TRADING, A STONE CAME UP. THIS STONE STRUCK THE EMPLOYEE'S MOUTH CAUSING IT TO BLEED. HIS FRONT TEETH WERE CHIPPED ALSO.
