EE was using a pry bar to unclog rip rap that was hung in a chute. When the rock came loose it fell onto the EE's hand smashing their left index finger. The EE was treated and received 3 sutures in the finger making this reportable to MSHA.

Mostrando los 47EE was attempting to clear debris from the spray nozzle on a wash down hose with EE's pocketknife. The knife slipped and the EE stabbed self in the inside left forearm requiring several stitches making this accident reportable to MSHA.
EE was unstopping feeder in surge tunnel. Rock came out of feeder and pinched EE's thumb between a feeder support and the rock, causing a laceration just behind the cuticle on the thumb. The laceration required 3 stitches.
The EE's were pulling a shaft through a pipe for a gate at the plant with a come-along. They had a pad eye welded to the end of the shaft. The EE looked in the pipe to measure distant left. The pad eye pulled a chunk out of the shaft and hit the EE in the safety glasses. The safety glasses pressed hard enough to cut the right side of the EE's face and require six stitches.
EE was tightening bolts on pipe flange on pump in the pit. EE had wrench in left hand and ratchet in right. Left hand slipped off wrench and right hand pulling pinched middle left finger between pipe and wrench. Laceration to left middle finger requiring 6 stitches.
EE was helping change segment plates on 990 loader. While taking the segment plates off, the EE pushed the plow bolt up into the slot to secure into place. While holding it, they turned the bolt with a wrench. The bolt head had a sharp edge, and when it turned, it cut EE's right index finger, causing stitches.
The EE was cutting a section off of a conveyor belt using a razor knife. The knife slipped, cutting the EE on the Left Wrist. No Stitches were required, but on 9/16/16, the EE went back to the doctor for a follow up and they prescribed antibiotics for fear of infection, thus making the accident reportable.
The EE was attempting to change a leaking hose on a CAT 988 Loader. The line was under the cab, so the employee was in an awkward position getting to the bolts. While trying to loosen the bolts, the EE felt a pain in abdomen. EE did not go to the doctor until 6/13/16, where the doctor diagnosed a hernia, placed EE on restrictions, thereby making the accident reportable.
EE was attempting to dislodge a rock from a tail pulley with a pry bar. The pry bar slipped and the EE hit his right thumb on a brace of the conveyor frame. The frame cut his hand behind his right thumb, requiring six stitches.
EE was swinging a 10 lb. sledge hammer to tighten a lock nut on a crusher. After about 5-6 swings, he felt a sharp pain in his right lower back that began to radiate to his right leg. He went to the doctor and he had pinched his sciatic nerve. The doctor prescribed steroids and wrote him out of work for the next two days, thereby making the accident reportable.
The EE was replacing poly screens. He was attempting to remove some of the pins. He swung the hammer, hit a piece of angle iron, the hammer bounced over and hit his left index finger causing a fracture. The EE's finger was bandaged with a splint, but is still able to perform his normal job (control room operator) with no restrictions.
EE was using pry bar to wedge gate into a diverter box. When the gate fell into place, the employee lost balance and caught left hand between gate and pry bar, fracturing his 3rd and 4th fingers on left hand.
The contract employee was notching a board that he was replacing on the office wall. As he was cutting the board, the knife slipped off of the board and hit his right thumb. The employee went to urgent care and the laceration required 5 stitches, thereby making the accident reportable. Name of contractor is Osada Construction
EE was changing a hydraulic hose on a rock breaker. He was pulling with one wrench and pushing with another. When he broke the connection loose, the wrench came back and hit him in the mouth. He bruised his lip and chipped his front tooth
While cutting a plug out of cylinder for an excavator, the knife slipped off the plug, cutting the left hand of the employee.
EE was attempting to cut hole in skirt rubber with knife. Knife closed on employee and cut his right index finger.

WHILE REMOVING A BOLT FROM P-9 CONVEYOR THE EMPLOYEES FINGER BECAME CAUGHT BETWEEN THE WRENCH IN HIS LEFT HAND WHILE PULLING THE RATCHET IN HIS RIGHT HAND.

EMPLOYEE WAS STANDING IN COLLECTION BOX SHOVELING ROCK OUT OF IT, LEFT THUMB GOT CAUGHT BETWEEN SHOVEL HANDLE AND SAND PIPE CUTTING LEFT THUMB.
EE WAS HOLDING PIN IN PLACE, WHILE OTHER WAS DRIVING PIN INTO BUCKET BOSS WITH A SLEDGE HAMMER. OTHER STRUCK EE'S HAND.

EMPLOYEE HAD A PIPE UNDER BELLY PAN OF 988 LOADER. A FLOOR JACK AND BLOCK WAS HOLDING THE PAN. A HELPER REPOSITIONED THE JACK THE INJRUED EKMPLOYEE WAS UNABLE TO HOLD THE PAIN AND IT CAME DO WN AND MASHED HIS FINGER BETWEEN PIPE AND FLOOR.

EMPLOYEE WAS SEATING A BUSHING IN A LOADER ARM BY HITTING A METAL PLATE WITH A SLEDGE HAMMER. THE PLATE WAS ON TOP OF A PIN THAT WAS ON TOP OF THE BUSHING. HE HIT THE PLATE OFF CENTER CAUSING IT TO FLIP END OVER END INTO THE AIR. THE PLATE CAME DOWN ON HIS HEAD CAUSING A CUT.
EE WAS CLEANING OUT STOPPED UP BOX AT TOP OF #1 SCREEN. HIS HAND WAS CUT BETWEEN SHOVEL HANDLE & METAL PLATE INSIDE OF BOX
CUT HAND WHEN KNIFE SLIPPED WHILE CUTTING TAPE OFF CONNECTIONS ON A FEEDER MOTOR AT THE JAW CRUSHER.
EE AND ANOTHER EE WERE ATTEMPTING TO REMOVE THE REDUCER FROM SAND CONVEYOR HEAD PULLEY. THEY WERE EXCERTING DOWNWARD PRESSURE WITH A 5 FT BAR, WHEN THE BAR SLIPPED. BOTH MEN LOST THEIR BALANCE AND FELL AGAINST THE HAND RAIL. EE BEING ON THEBOTTOM, BROKE SEVERAL RIBS.
EMPLOYEE HAD BEEN SHOVELING ROCK FROM THE FLOOR ON TO THE CONVEYOR BELT IN THE ROCK TUNNEL. WHEN HE FINISHED HE REPORTED A PAIN IN HIS BACK.
EMPLOYEE WAS ATTEMPTING TO BEND A FLANGE BY STRIKING IT WITH A HAND SLEDGE HAMMER THE METAL BOUNCED UP & STRUCK THE EMPLOYEE ON THE RT HAND
INJURED EMPLOYEE WAS HOLDING A TRACK LINK FOR A 245 HOE WHILE ANOTHER EMPLOYEE WAS DRIVING THE CONNECTION PIN. A SLIVER OF STEEL BROKE FROM THE PIN, STRIKING INJURED IN THE THROAT, SEVERING TH E JUGULAR VEIN AND CAUSING SEVERE LACERATION. WILL BE LOST TIME.
EMPLOYEE WAS HEATING TRACK RAILS WITH A TORCH ANOTHER EMPLOYEE WAS DRIVING TRACK PIN INTO PLACE WITH SLEDGE HAMMER A PIECE OF METAL BROKE OFF THE PIN & HIT EMPLOYEE IN LEFT UPPER ARM IT WAS A NEW PIN

EMPLOYEE WAS MOUNTING A SPLIT PULLEY ON AN AIR COMPRESSOR SHAFT. HE HAD DRIVEN A CHISEL INTO THE SPLIT TO INLARGE THE OPENING AND AS HE WAS TAPPING THE PULLEY ONTO THE SHAFT, THE CHISEL FELW OUT STRIKING HIM IN THE CHIN.
EMPLOYEE WAS HELPING A MECHANIC KNOCK A BEARING OUT OF THE FRONT WHEEL OF B70 EUCLID EMPLOYEE WAS HOLDING A BAR AS THE MECHANIC STRUCK IT WITH A LARGE HAMMER THE BAR SLIPPED AS IT WAS STRUCK & CAUGFHT THE EMPLOYEES FINGER BETWEEN BAR & WHEEL

TWO EMPLOYEES WERE PULLING ON A BAR TRYING TO SHUT TRAIN DOOR. THE BAR SLIPPED AND STRUCK INJURED EMPLOYEE ON THE HEAD BESIDE THE RIGHT EYE.
EMPLOYEE WAS HAMMERING OLD TEETH OF A BUCKET.THE HAMMER GLANCED OFF CAUSING EMPLOYEE TO TWIST AROUND.

I NAME WAS REPA3RING PART FOR PUMP THE PART HAD A RUBBER STEEL ON IT AND IT WAS NECESSARY TO SLIT TO REMOVE.KNIFE SLIPPED AND CUT LEFT THUMB WHICH REQUIRED 3 STITCHES.
