Employee was using a crowbar to break rocks stuck in the vibrating screen. one rock broke easier than expected, causing to reach farther then intended and fingers were smashed between bar and screens.
Employee injured his left hand while doing repairs to a sledgehammer in the tool room. Employee was hammering a wedge into the top of sledgehammer to tighten the handle to the head, the sledgehammer was clamped in a vice, and he hit the base of his left thumb with a glancing blow while trying to drive the wedge in with a hammer. No broken bones, released for modified duty.
Employee was assisting in opening a rail car bottom dump door by attaching a come-along to the door to try to pull it open. The day after the event, he felt discomfort in the left side of his back. On Aug.20th the employee reported that his back was still hurting, the company arranged for a Dr's. appointment that day. The extent of the impairment is not yet known.
Employee was pushing upward with a long metal rod trying to dislodge a tire that was jammed up on the tire rollers (conveying system). In doing so, employee strained his lower back.
WHILE EE WAS ADJUSTING THE PULLER, HE PULLED HIS BACK MUSCLES CAUSING SPASM TO HIS MID BACK.
Rubber tires were moving along a roller conveyor. A tire became jammed against a steel beam, other tires continued to tighted up the row of tires. The injured employee attempted to 'unjam' the tires by pulling up on the first tire in the row. As he pulled on the tire with a long hooked bar, he strained a muscle in his lower back.
Using sledge hammer, swung and missed piece of angle welded to shaft on screw for 4SC29. Pinched right middle finger between handle of sledge and the angle iron that was sticking up.
Hitting side of chute, smashed ring finger between sledge handle and fin on ball hopper chute. Employee not watching clearances.
Cut finger while using a knife to cut lunchmeat. Finger was cut to the extent that it was necessary to seek medical assistance. Stitches were required..
TIGHTENING BOLTS IN OSEPA INLET LOUVER AND STRAINED SHOULDER. *EMPLOYEE DID NOT SEEK MEDICAL ATTENTION AT TIME OF INCIDENT NOT REPORTED UNTIL RECENTLY.

EE WAS POKING DOWN WHEN DUST CAME THROUGH THE SAFETY EQUIPMENT INTO LEFT EYE.
CLEANING OUT A PLUGGED CHUTE AT A CONVEYOR TRANSFER POINT. MATERIAL BROKE LOOSE AND STRUCK EE ON THE ARM
EMPLOYEE WAS POKING OUT THE HOPPER. WHEN HALF THE BALLS WERE REMOVED FROM HOPPER, THE HOPPER LIFTED UP AND LANDED ON EMPLYEES FOOT THEN BOUNCED BACK UP.
WHILE CLEANING 4CD4 OVERFLOW PIPE MATERIAL FROM TOP DISLODGED AND CAME DOWN PIPE AND EXITED THE CLEAN OUT HOLE STRIKING EE ON RT SIDE OF FACE, HOT MATERIAL CAUSED SOME BLISTERING.
SHOVELING DIRT. DIRT POSSIBLY GOT IN EAR.

HOLDING DRIVE CHAIN WITH A BAR WHILE FELLOW EMPLOYEES WERE BRINGING CHAIN TOGEETHER THE BAR SLIPPED CAUSING EMPOLOYEE TO FALL TO DECK 5FT BELOW
EE WAS SWINGING METAL BAR TO DISLODGE MATERIAL ON GATE 3GA1A. BAR BOUNCED OFF OF THE MATERIAL STIRKING AND SMAHSED FINGER AGAINST GRATING ABOVE THE GATE.
USING PIPE WRENCH FELT PAIN IN LOWER BACK IPE WRENCH
EMPL WAS REPLACING BUCKET TEETH ON LOADER USING HAMMER PIECE BROKE FREE AND FLEW INTO AIR STRUCK LEG AND BECAME EMBEDDED
EMPLOYEE WAS POKING DOWN THE HOPPER AT THE OLD CLAY CRUSHER WITH A ROD. AFTER COOLING OFF, EMPLOYEE FELT HIS BACK START HURTING. (LAST DAY WORKED: 5-31-88)
