Mostrando los 23Two maintenance mechanics were disassembling a bucket elevator. One mechanic was holding a punch in place with a tool holder while the other mechanic struck it with a sledgehammer. A piece of metal that had mushroomed around the end of the punch sheared off and cut the left thumb of the mechanic holding the tool holder. The EE received three stitches to close the wound.
Employee was in the breakroom and used a utility knife to remove a flash holder from safety vest that had been secured with a zip tie when employee accidentally cut left thumb. Employee received 3 stitches to close the laceration.
Employee was injured while installing a sprocket on a drive shaft in the maintenance shop. EE was striking the sprocket with a hammer when a piece of metal approximately 1/4" in size sheared from the sprocket and stuck into EE's right arm. EE received one stitch to close the wound. EE was wearing gloves, safety glasses and a hard hat.
EE was chipping hardened cement off a conduit and a large chunk broke free and hit EE in the mouth. Medical treatment from dentist resulted in extraction of two cracked upper left teeth.

Tightening a nut and bolt with a spud wrench and the wrench slipped and hit EE in the nose. Doctors are saying that it is suspected to be broken. The practitioner report says that this person is on 'modified duty'. Modified to include limited lifting, minimize bending and no heavy pulling or pushing. This person has been off of work since the injury happened.
Employee was using a bar to dislodge another bar at a slag feeder. When attempting to dislodge the bar, he pinched his finger against the bar that was stuck suffering a laceration to his right index finger.
Employee was shoveling up a spill with 3 other workers when another worker's shovel struck him on the left wrist. He suffered a contusion to the left wrist.
Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated "ee was seen today in the ER. He should not work tomorrow".
EE was attemting to free up some material in the Gypsum material by using a pipe to stick into the access hole. While ramming the pipe in the hole, EE's hand struck the frame of the weigh feeder which resulted in a laceration to the right thumb.
Employee was operating a winch when his back become sore.
EE was using a prybar to close a door in the preheater tower, when the bar slipped and he fell spraining his knee.
Employee was cleaning out the clinker feeder when the bar slipped and his wrist hit the steel feeder. He has a contusion to his right wrist.
Employee was attempting to remove a bearing with a bearing puller. While applying pressure to the puller, the bearing housing broke and he fell down straining his lower back.
Employee was attempting to pry a piece of steel from a loader bucket. The steel suddenly came loose and the steel struck him on his lower right leg. The injury required 6 stitches and he is on regular duty.
While employee was poking at a plugged hopper, a piece of material fell aand hit the end of the bar. When it hit the end of the bar, the end he was holding went up and jammed his finger against the side of the hopper. He sustained a crushing injury to his ring finger on his left hand.
EE WAS WORKING ON A CHUNK OF CLINKER IN THE RECLAIM TUNNEL WITH A PRY BAR. BAR SLIPPED AND EE SMASHED HIS HAND BETWEEN PRY BAR AND SIDE OF FEEDER.
EE APPLYING FORCE TO MACHINERY WITH PRY BAR. BAR SLIPPED WEDGING HAND BETWEEN SUPPORT COLUMN & PRY BAR.
EE WAS ATTEMPTING TO FREE MATERIAL FROM A SILO. AS HE WAS STRIKING THE CHUTE WITH A HAMMER HE HIT HIS FINGER.
METAL BARS WERE BEING USED TO CLEAR CHUTE BAR SLIPPED OUT OF EE'S HAND FALLING ON HIS FINGER
