Material handler used a shackle to open a slide gate where EE smashed EE's left thumb between shackle and slide gate. Xray showed a slight fracture.

The injured miner was swinging a 10 pound sledge hammer to attempt to unplug a lime kiln dust (LKD) bin and chute. While swinging the hammer they felt a pop and pain in right shoulder/arm. They visited an orthopedist who released them to restricted duty. An MRI is scheduled for 12/15/2023 to determine the extent of the injury.

Worker was repairing a belly dump trailer. EE used a jack to jack up the trailer. When the EE reached through the gates to move a railroad tie. EE either hit the jack or the jack moved in the dirt causing the trailer to drop, closing the gates on the EE's arm. Barney trucking reported verbally to MSHA on 10/20/2020.

Emp was swinging a sledgehammer and felt a pain in their back. Emp was taken in for treatment and was given restrictions. Emp worked 1 day with restrictions.

EE was removing bolts from a final drive on haul truck. Later that afternoon, back started bothering EE (strain).

Employee was cutting holes in the bottom of a plastic tool bucket with EE's Leatherman knife to drain water out. The knife slipped and EE cut EE's hand.

EE used a come along to reposition a take-up pulley, this process was inadequate. So they got a crane. The come along was hooked onto an I-beam which worked with the previous direction of pull. As the crane passed the come along, direction of pull changed. Adjusting, the employee pulled the come along to the side, allowing it to fall off the I-beam causing an ulnar fracture.

Employee was using hammer to work on trailer gates (sprained shoulder).

employee was cutting skirt board with a razor knife. The knife slipped and caused a small laceration on his upper left thigh, requiring 3 sutures.

Employee was trying to remove build up of material from a product gate, when pry bar struck the employee below the eye.

Employee was raking ash. He lost his grip on the rake handle. He made a grab for it and pinched his thumb between the handle and a handrail.

Employee was raking ash. Felt pain in arm.

Miner was cleaning coal mill grizzly --- fractured tip of left pinky with steel bar.

Hand was pinched between chain come-along and pipe elbow while moving a pipe elbow into place. Chain come-along slipped on metal, finger got in the way.

EE was removing fittings from old section of waster pipe when pocket knife he was using slipped and came down across back of left hand and first knuckle of thumb.

Employee was repairing a ram. Lifting ram with bar. The bar shifted allowing the ram to pinch finger causing a laceration on finger.

Employee was rodding #3 Kiln preheater & felt pain in lower back & leg.

Employee was tightening coal roll bolts with a wrench and felt pain in left shoulder. This was not immediately reportable but became reportable when it did not heal. He is scheduled for surgery 1/16/09. Employee has not missed any work as of today with this injury.

Employee putting new cable on chute and forgot to remove vice grip from motor shaft they were using to line hole for new cable. They were tightening cable against limit switch and vice grip came around and struck employee in the face.

Rodding out chute with metal rod, chunk of material fell and hit rod causing cut on chin.

Splicing belt and cut finger

Employee was cleaning out chute. He "hit the chute with a bar and smashed his right index finger between the bar and a beam". Avulsion to right index finger.

Prying on wear liners and strained shoulder

EE was cutting rubber sheet with razor knife and cut left hand.

Injured left shoulder whle beating on a coke silo.

Working on level ground, he cut his finger with a razor knife fixing lights on a trailer.

EE WAS SITTING HAUL TRUCK USING LEATHERMAN. WENT TO CLOSE BLADE & SLIPPED SLICING RIGHT THUMB.

EMPLOYEE WAS LOADING RAILCARS & HURT LOWER BACK WHEN PRYBAR SLIPPED FROM HOPPER DOOR.

WORKER WAS USING A PICK AXE TO CHIP AWAY A BUILDUP OF FINES AGAINST A HOPPER WALL. HE SAID HE FELT A POP IN HIS RIGHT INDEX FINGER BUT CONTINUED WORKING. AT THE TIME, CLAIM WAS NOT REPORTABLE.

STANDING ON HANDRAIL WITH 1" RATCHET; PULLING DOWN TRYING TO BREAK 2" NUT LOOSE; SOCKET SLIPPED OFF NUT; RING FINGER OF LEFT HAND GOT CAUGHT BETWEEN RATCHET AND I-BEAM.

ON A JAMMED ELEVATOR BUCKET WHILE HAMMERING A LARGE SPROCKET WITH A SLEDGE HAMMER A PIECE OF METAL CAME OFF. SMALL PUNCTURE WOUND TO LEFT LEG.

PUTTING BELTS ON MICROPULVERIZER, HE USED PIPE WRENCH TO TURN PULVERIZER. PIPE WRENCH CAUGHT LEFT INDEX FINGER BETWEEN WRENCH AND PLATE CAUSING LACERATION.

PRYING WITH BAR HEARD POP IN SHOULDER. DIDN'T FEEL THIS WAS ANYTHING. COMPLETED SHIFT. AWOKE NEXT MORNING ON DAY OFF. SHOULDER WAS RED AND SORE. NOTIFIED SUPERVISOR.

WHILE BREAKING LOSE A BOLT, THE WRENCH SLIPPED, CAUSING HADN TO SWING INTO A BROKEN WELD. RESULT WAS EIGHT STITCHES.

WORKING ON #2 DOUBLE BELLS TRYING TO PUT RAM BACK IN PLACE WITH HAMMER, MISS RAM AND HIT RING FINGER.

OPENING A RAILCAR TO CLEAN IT, WITH A METAL BAR. BAR SLIPPED AND EE HURT HIS BACK, THEN NEXT NIGHT HE HURT IT AGAIN OPENING A LID ON TOP OF A RAILCAR.

PINCHED LEFT THUMB BETWEEN WRENCH & PIPE WHILE TIGHTENING PIPE FLANGE BOLTS.

EE WAS DIGGING OUT A SCREW CONVEYOR WHICH CAUSED A THICK DUST CLOUD. THE EE INHALED THE LIME DUST WHILE BREATHING WHICH CAUSED DAMAGE TO THE LUNGS.

WORKING ON STUCK GRATE CASTING WITH CROW BAR. CROW BAR SLIPPED AND HIT LEFT HAND ON CASTING.

REPLACING MISSING CASTING AND GOT FINGER PINCHED BETWEEN PRY BAR AND CASTING.

EMPLOYEE WAS TRYING TO DISLODGE PRODUCT FROM CORE BIN USING SLEDGE HAMMER HE FELT PAIN IN UPPER BACK BETWEEN HIS SHOULDER BLADES EMPLOYEE WENT TO CHIROPRACTOR THE NEXT AFTERNOON

EMPLOYEE USED SLEDGE HAMMER SEVERAL TIMES DURING HIS SHIFT TO DISLODGE PRODUCT FROM DUST & HYDRATE SILOS BY THE END OF DAY HAD SHARP PAINS IN HIS LOWER BACK

HE WAS USING A CUT OFF TYPE GRINDNG WHEEL ON A 7 INCH GRINDER TO CUT SOME GUTTER FLASHING TO SIZE WHEN THE WHEEL EXPLODED. HALF OF THE WHEEL REMAINED IN THE GRINDER & JUMPER OUT OF THE STEEL & STRUCK HIS HAND [LEFT HAND].

EMPLOYEE WAS CUTTING CARDBOARD WITH A RAZOR KNIFE. KNIFE SLIPPED AND CUT HIS FOREARM. (REQUIRED SEVERAL STITCHES AND A TETANUS SHOT- NO LOST TIME)

EE WAS CLOSING A BOTTOM DOOR ON A RAILCAR AFTER CLEANING IT OUT. THE BAR WHICH HE WAS USING TO CLOSE THE DOOR SLIPED AND HIT HIM ABOVE THE RIGHT EYE. BRAKING HIS GLASSES AND CUTTING THE SKIN O PEN, HE RECIEVED FIVE STICHES. THERE WAS NO LOSTTIME.

TRYING TO OPEN TAIL GATE ON DUMP TRUCK, AIR CYLINDER FAILED, HE THEN PRYED ON LATCH WITH BAR WHEN LATCH OPENED BAR AND SMASHED RIGHT HAND PUNCTURING WOUND ON THE HAND HAD TO HAVE ONE STITCH IN HIS RIGHT HAND.

WORKING WITH CO-WORKER TO REMOVE BEARING RACE, CO-WORKER HIT RACE WITH HAMMER, A SMALL FRAGMENT OF BEARING RACE CHIPPED OFF RACE AND HIT EMPLOYEE IN THE CHEST. DR. TOOK 3 X-RAYS AND TRIED TO R EMOVE CHIP. CHIP EMBEDDED TP DEEP AND REMAIN. 3 STICHES TO CLOSE WOUND.

EMPLOYEE WAS USING A BAR TO LOSSEN MATERIAL IN A VIEW PORT HE HIT WRIST ON THE EDGE OF VIEWPORT.
