Attaching a cylinder to the blade of a dozer ---when the pry bar slipped and the cylinder smashed the tip of the Emp left middle finger --- requiring stitches.

While wrenching on a cylinder on a piece of equipment the wrenches pressed together and cut into the nail and tip of the mechanic's rt ring finger requiring stitches.

Emp was hitting shoe on a blow bar and hit a hard surface, a piece of steel struck and cut EE's finger. EE got 1st aid and continued to work. Emp worked until 9-28-2020 when they found a piece of steel in EE's finger and had it removed. EE then went on RD as of 9-29-2020.

Employee was using a knife to open up a package of soap stone and cut left hand pinky finger... requiring 4 stitches.

Employee was cutting skirt rubber when the blade slipped and cut his finger

Employee was cutting a belt and the knife slipped and cut his little finger.

Employee was changing impact bars on a crusher. He attempted to block the rotor with a pry bar and the bar pinched his finger against the frame.

EMPLOYEE WAS STANDING ON 4TH ROW STEP TAKING OFF #3 BELT ROLLER WHEN THE WRENCH SLIPPED OFF CAUSING EMPLOYEE TO FALL.

EE WAS ATTEMPTING TO DISLODGE BUILT UP MATERIAL IN THE SCREEN CHUTE WHEN MATERIAL CAME LOOSE AND STRUCK HIS LEG.

EE WAS INSTALLING POLYDECK PANELS USING A HAMMER. WHEN EE STRUCK THE PANEL A PIECE OF METAL ENTERED HIS EYE.

EMPLOYEE WAS TRIMMING A URETHANE SCREEN WITH A UTILITY KNIFE PULLING IT TOWARD HIMSELF WHEN IT SLIPPED OFF THE SCREEN AND CUT HIS WRIST.

EE WAS OPENING DOOR ON CRUSHER HE PUT A BAR BETWEEN DOOR AND HOUSING AND BAR SLIPPED OUT CAUSING HIM TO FALL & TWIST KNEE.

EMP WAS TRYING TO GET FROZEN MATERIAL OUT OF THEHOPPER USING A DIGGING BAR. IT SLIPPED OFF THE ANGLE IRON HE WAS PRYING AGAINST AND CAUGHT HIS HAND BETWEEN IT AND THE SENSOR SWITCH.
