Employee was trying to clear a rock that was stuck in the head pulley of the 2320 conveyor when the hammer EE was using came in contact with EE's right index finger causing a laceration to EE's right knuckle requiring 4 stitches.

Miner sustained a shoulder injury while using a sledgehammer to hit a pipe to clear a plugged line.

Employee was utilizing a prybar to assist with removal of motor for replacement when the prybar slipped and struck employee on the right side of the hardhat/head on the right ear causing a laceration to the upper portion of the right ear. The hardhat took the brunt of the force from the blow of the prybar.

Miner was raking ash at kiln hood when a piece of ash hit the end of the rake causing the rake to hit the miner on the chin. Miner sustained a laceration requiring stitches.

Miner was using a pipe wrench to remove an incline water strainer, and when the fitting broke, the wrench to slam against an I-beam pinching the right-hand pinky finger.

The employee was installing brackets for form scaffolding. While EE was trying to shift an alignment rail, EE had to loosen the alignment bolt. While hitting the bolt, the hammer ricocheted off the bolt and into EE's left hand EE had placed on a 4x4 form brace.

On 08/01/2024 miner was changing a drive belt using a wrench when injury occurred to shoulder joint. On 08/06/2024 miner was issued restricted duty by medical clinic.

EE was replacing skirt material on CL- 303, decision was made to change size of material from a 3" to 6" skirt material. EE was utilizing a pair of needle nose vise grip pliers along with channel lock pliers to pull the material into position between the hold down bracket and chute when the vise grip pliers slip free striking the EE in the mouth fracturing the #9 tooth.

Employee 1 was holding wrench while employee# 2 was pushing on wrench to loosen leveling nut for column. Employee #1 's finger became caught between the wrench and the concrete pad that the column sets on when the leveling nut broke loose and the column base shifted down

On 07/18/19 miner reported to supervisor that they were using a wrench to tighten packing on the pressure vessel discharge at the hydrator, and when they pulled the wrench upwards, they felt a pain on left side of back from neck to lower back. Miner refused treatment on 07/18/19 but on 07/31/19 the miner requested medical treatment.

Employee was attempting to remove the packaging from surveying tape. Employee went to cut zip ties with pocket knife. The employee's momentum from the upward force cutting the zip tie. The knife slipped and cut the employee on the left pointer finger. The cut was deep enough that medical treatment was required. The employee received 5 sutures to close the wound.

Miner was using a come-along wench to extract a probe from kiln transfer chute. The wench strap slipped and got caught on a T-pipe connector, causing the T-pipe to break-off, go airborne, and hit the miner on the left elbow.

EE was raking ash when a rock hit the opposite end of the rake causing the rake handle to jump out of the EE's hand and strike the EE on the right hand thus causing a contusion to right ring finger.

Worker was splicing a flexible PVC pipe with a utility knife. While making the cut, the pipe moved causing the knife to deflect onto the workers left hand. Worker was seen by medical personnel and received four sutures.

EE was using a sledge hammer to loosen plug chute and reported sore shoulder to Supervisor. EE did not want to seek professional medical treatment at the time of the incident. On 7/5/2016 EE notified Supervisor that EE wanted see MD.

On 03/06/2015, EE was trying to maneuver around belt skirting and had to twist to access 3/8" bolts underneath the skirting. After completing task, EE noted a 'twinge' in the lower right side of his back. EE did not request medical. On 3/23/2015 EE reported to work and requested medical treatment. Medical issued prescription for pain.

Employee was pushing on pry bar at dyer discharge screw, when the pry bar slipped and caught employee's right thumb between pry bar and metal. Employee received 14 stitches.

Miner cutting tubing with a utility knife, cut through tubing and into his own leg.

Employee was opening container lids when bar used in the process slipped striking employee in the lower lip causing laceration and fractured tooth.

He was closing a slide gate with a sledge hammer. The hammer slipped and he hit his left index finger.

Miner with muscle strain had surgery to repair shoulder.

Using pipe wrench while doing maintenance on equipment

Employee was laying brick and went to hit the brick with a brick hammer to lodge it into place. Instead of hitting the brick he hit the metal shim beside it and the hammer slipped off the shim and hit his finger.

EMPLOYEE WAS SPLICING CONVEYOR BELT USING A BOX CUTTER TO TRIM BELTING MATERIAL WHEN THE KNIFE BROKE THROUGH THE BELTING CUTTING LEFT INSIDE OF LOWER FOREARM.

EMPLOYEE WAS REMOVING MATERIAL BUILDUP FROM #2 CHUTE IN 1300'S TUNNEL WHEN THE CHUTE GUARD ACCIDENTLY FELL ONTO BAR BEING USED TO DISLODGE MATERIAL. THE GUARD FELL ONTO THE BAR CAUSING THE BAR TO JUMP UP PINNING EMPLOYEES FINGER BETWEEN THE BAR AND STRUCTURE. EMPLOYEE REFUSED MEDICAL ATTENTION AT TIME OF ACCIDENT. EMPLOYEE WENT TO THE DR ON 9/24/02. IT WAS DISCOVERED FINGER BROKEN.

EE WAS CLEANING SCRUBBER FAN USING CHIPPING HAMMER. PIECE OF SLAG FLEW OFF AND STRUCK EE IN LEFT EYE. SAFETY GLASSES WERE NOT WORN DUE TO HUMID CONDITIONS THEY KEPT FOGGING UP AND EE COULDN'T SEE TO DO THE WORK.

USING WRENCH TO LOOSEN PIPE AT #1 SCRUBBER. WRENCH SLIPPED, EE FELL BACKWARDS INTO GUARD RAIL.

EE WAS CLEANING SIDES OF TRAILER WHEN HE SCRAPED HIS HAND ON THE INSIDE OF THE TRAILER CAUSING A BRUISE AND ABRASION. EE DID NOT GO TO THE DR UNTIL 12-2-97 FORTREATMENT OF THE ABRASION. HIS HA ND STARTED SWELLING AND HE FELT HE HAD A INFECTION STARTING. THE EE REPORTED THIS ACCIDENT TO THIS DEPT ON 12-15-97. INFECTION IN HIS RIGHT HAND.

EMPLOYEE WAS WATCHING OTHER EMPLOYEE DRIVE PIN OUT OF STEERIING COLUMN USING ANOTHER BOLT, WHEN A SLIVER OF METAL FLEW OFF OF THE PIN STRICKING EE IN THE LEFT EYE.

USING A PORTA POWER TO PUSH TRUNION THE PORTA A POWER SLIPPED AND PRESSURE WAS RELEASED WHEN IT CAME LOOSE IT STRUCK THE EE S KNEE CUTTING HIS KNEE THAT REQUIRED 4 STITCHES TO CLOSE.

IN THE PROCESS OF CHANGING A CONVEYOR BELT AT THE PRIMARY CRUSHER.A ROPE USED TO HELP KEEP THE BELT TIGHT SLIPPED OFF AND INJURED FELL AGAINST STRUCTURE WITH HIS SHOULDER BEING INJURED.BEGAN S EEING THE DOCTOR EVERY 3 TO 4 WEEKS FOR THERAPY.

EMPLOYEE WAS INSIDE #2 KILN KNOCKING BRICK LOOSE WITH A SLEDGE HAMMER THE SHAPE OF THE KILN MAKES IT DIFFICULT TO STAND & SWING THE SLEDGE HAMMER

EMPLOYEE WAS REMOVING BOLTS USING A HAND WRENCH AND STRAINED WRIST BUT DID NOT REALIZE THAT THERE WAS ANY DAMAGE UNTIL IT BEGAN TO SWELL AFTER CONTINUED USE FOR SEVERAL DAYS.

HANGING 36 IN PIPE AND GOT RT HAND BETWEEN PIPE AND HANDRAIL WHEN CHOKER SLIPPED WHICH RESULTED IN AMPUTATION OF RT THUMB REPAIRED BY SURGERY
