HAMMER HEAD DEFLECTED OFF END OF DRIVING SHAFT AND THE HAMER CONTINUED TO SWING BRINING THE HANDLE OF THE HAMMEER TO IMPACT WITH CROSS SUPPORT I-BEAM. EMPLOYEES.
