Employee was working on breaking bolts free on the waste pump to inspect inner components. When the second bolt and nut broke free it jarred employee forward causing a slight pop and pain in right shoulder. Employee was using an adjustable wrench for the job because employee did not have a big enough wrench/socket for the job.

Mostrando los 10Mechanic was torqueing boom bolts on a crane to 2100ft lbs., the employee switched from torque wrench to a 24" swivel head ratchet to remove the pressure from the torque multiplier. As EE pushed down, the ratchet handle slipped out of EE's hand and struck EE in the face resulting in a fractured and lacerated nose.
Swinging a 4# sledge hammer with both hands to free up a cut piece of metal from beam. Metal broke free and upon completion of swing the left hand middle finger contacted the metal beam, resulting in a fracture of the middle finger on the left hand. Employee is right hand dominant.
Employee was removing chutes from the twin screw area preparing for its removal and replacement. EE was moving a section of a chute that was cut off; it was suspended by a 1-1/2 ton chain hoist. As EE was lowering the chute EE grabbed the bottom of it with right hand to guide it in place. The chute swung left and pinched EE's right middle finger.
Employee was on a catwalk next to the screen making a 4"x7" notch in a 10' strip of belting with a utility knife, when the knife slipped and struck his left wrist.
While installing side bits on a loader bucket, employee hit the bit with a hammer and a piece of steel came off hammer and hit left index finger. Employee went to clinic on 4/25/05. He was dispensed 800 mg of Motrin, 500 mg. of Ciprofloxacin and given a splint to wear on finger. Has follow up visit scheduled for 4/29/05.
