Employee was enlarging an existing hole in a section of rubber (conveyor belting) with a utility knife. In the process of making the hole the knife slipped and cut their hand resulting in 6 stitches.

Employee was working to remove the well at the re-crush plant. The well got hung up on a collar. Employee used a hammer and wedge to remove collar. Once collar got knocked free the well casing slid down and pinched employee's left hand ring finger tip.

Employee was using a hammer to attempt to unclog a chute from outside the chute. After doing so, EE felt pain in EE's right shoulder.

North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.

Employee was cleaning the feeder chute and smashed right thumb between the tool and the chute. Employee was taken to urgent care and received 2 stitches.

While swinging a sledge hammer to clean process chute, employee's finger got caught between the sledgehammer handle and the angle-iron of the chute. Employee reported the incident to his supervisor at lunchtime. Employee went to the hospital on 6/5/15, where it was determined that the employee has dislocated and fractured his finger.

Employee was using a bar with a hook on the end to move a steel plate on the screen deck in an area with very low clearance. While lifting the plate with the tool, it slipped off and struck the employee in the mouth chipping his front tooth.

While tapping a gear off a shaft with a hammer, a piece chipped off the gear hitting him in the stomach. Treated at ER for stomach puncture.

Opening a ballast car door. Door jammed while employee trying to lift it open with bar causing a muscle strain in upper shoulder and neck.

While using a pick axe to loosen dirt, the pick struck a cross member causing the pick to change direction and hit the inside of the employee's left foot. Treated for a foot contusion and placed on restricted duty.

Employee was trimming a rubber gate guard with a razor knife. The knife pulled free of the rubber, striking his left leg just above the knee.

Employee has his personal pocket knife in his hand and was leaning back in a chair when he lost balance and jolted suddenly and stabbed himself in the thigh.

while using a wrench with force caused an unknown collapse of vein in Right hand middle finger.

Pulled on the pipe extractor & it let loose, fell backward & hyper extended left knee.

WAS TIGHTENING VISE FELT PULL IN BACK.

CUTTING A PIECE OF ALUMINUM WITH A KNIFE, AND SLICED HIS RIGHT INDEX FINGER.

EMPLOYEE WAS PLACING A MACHINERY PART IN A VISE. THE PART DISLODGED & STRUCK HIS FINGER CAUSING A FRACTURE.

A CO-WORKER WAS STRIKING THE WRENCH THAT THE EMPLOYEE WAS HOLDING (THEY WERE TIGHTENING BOLTS). THE WRENCH STRUCK THE EMPLOYEE'S THUMB, FRACTURING IT.

EE CLAIMS HE FELT A "RIPPING" SENSATION IN HIS BACK WHILE PULLING RAILROAD SPIKES. HE IS CURRENTLY LOSING TIME FROM WORK. QUESTION IF WORK RELSATED DUE TO OTHER CIRCUMSTANCES.

EE WAS WIRE BRUSHING A BOLT - STANDING BELOW. AIRBORNE DEBRIS GOT BENEATH HIS SAFETY GLASSES.

EE WAS REMOVING/REPAIRING AN M-3 SCREEN DECK. A PRYBAR WAS BEING USED BY SOMEONE ELSE ON AN UPPER LEVEL. PRYBAR WAS DROPPED, SLID DOWN SCREEN THROUGH THE CRUSHER AND EVENTUALLY STRUCK EE IN TH E NECK.

WHILE JACKING THE WEDGE ONT HE JAW CRUSHER, THE JAW SLIPPED OFF THE COLUM. ONE OF TWO SPACERS STRUCK EE IN THE L SHOULDER. NOT INFORMED OF INCIDENT UNTIL 1-4-99.

EE WAS INSTALLING RADIATOR HOSE ON EUCLID USING A HAMMER TO TAP THE HOSE INTO PLACE. EE STRUCK HIS L PINKY WITH HAMMER SUSTAINING A FRACTURE AND LACERATION WHICH REQUIRED STITCHES. EE WAS NOT WEARING GLOVES.

EE WAS IN THE PROCESS OF TIGHTENING A COME-A-ALONG. WHILE TIGHTENING IT BROKE FREE AND STRUCK EE S RIGHT LOWER LEG.

TOCKS BECAME STRUCK IN THE CRUSHER ' S THROAT. IN LIELL OF USING THE ROCK BREAKER MACHINE, EE ATTEMPTED TO DISLODGE THE ROCK WITH AN IRON BAR. THE ROCK TWISTED AND EE THUMB BECAME PINCHED BETW EEN THE BAR AND SIDE OF THE CRUSHER.

WHILE EE WAS PAINTING TRUCK PARTS, HE BEGAN TO FEEL PAIN IN HIS LOW BACK & RIGHT LEG.

EE WAS HOLDING A WRENCH WHILE ANOTHER WAS USING A HAMMER. HAMMER STRUCK EE'S FINGER.

EMP LOST CONTROL OF UTILITY KNIFE CAUSING IT TO COME INTO CONTACT WITH RIGHT LEG.

WHILE WORKING ON A PIECE OF EQUIPMENT EE'S HAND BECAME CAUGHT IN A JACKSTAND. SUSTAINED A TENNON INJURY & LACERATION WHICH REQUIRED STITCHES.
