Employee was using a sledgehammer to drive a wedge behind liner. A small piece of metal broke off and hit him in the stomach.

5/31/13 at approx. 4:30am, while hammering on the mill liner, a shard of metal came off caused by the concussion of the hammer and impacted the leg of one of the contractors on site. The injury was reportedly minimal and did not require the immediate need of a medical professional.

Employee cutting up super sucker hose with cutting knife, slipped puncturing the skin on his left knee.

Employee was using two wrenches to break loose a pipe fitting. As he placed pressure/weight on the wrenches he heard and felt a "pop" in his right shoulder. Strain/sprain to right shoulder.

Employee was cutting tape off piece of rope, the rope slipped causing employee to cut left thumb. Employee was not wearing appropriate issued PPE (gloves).

Employee claims he injured his ribcage (strain) while using a sledge hammer on 3/23. this was reported on 3/29/07

Employee was walking by an area where two employees were working on a loader removing teeth and pins with a sledge hammer. A sliver of steel hit him in the left thigh.

On 12/21/06, an employee was using pliers to tie a piece of tie wire. The tie wire broke and the pliers hit employee in the mouth.

On 12/11/06 employee was hitting a drift pin with a 8 pound sledge hammer into a bolt hole on a beam. EE swung and missed drift pin, striking his own left thumb.

Employee using sledge hammer to pound out holding pin in bucket of 988 loader. A piece of metal hit the employee's right eye causing an abrasion.

Using hammer to unchoke plugged chute. Hammer was being swung across the employees body and overhead in attempt to loosen material stuck in chute. Strained right hand.

Employee was working on # 1 Finish Ball Mill using a sledge hammer to clear dust collector pipe when he felt a pain in his lower back, right side.

The employee was in the basket of a manlift using a sledge hammer to clear a clinker pipe when he felt a pain in his right side.

Employee was removing hard material from #10 Spitzer belt pulley when he caught his right little finger between the hammer and pulley. This resulted in a contusion. This was originally reported as a (report only).

While attempting to open cement cooler door, with a sledge hammer, employee caught his R/ring finger between the door and sledge hammer.

Employee struck the feeder slide gate with a hammer causing hot dust to contact his face below the eyes, this caused some minor dust irratation.

Employee fell while swinging a sledge hammer at Gyp bin, straining his back.

EE was trying to open the knock over valve for #7 silo, when he pinched his right hand-ring finger between the bar he was using to pry the valve open and the recirculative fkline, the valve was sticking and would not open all the way.

EMPLOYEE WAS USING A COME-A-LONG WHEN THE CLUTCHFAILED AND HE CLAIMS IT CAUSED AN ELBOW STRAIN.

EE WAS WORKING WITH ONE OF FOUR COME-A-LONGS MOVING A GEAR BOX WHEN HE FELT A PULL IN HIS STOMACH.

EMPLOYEE WAS HAMMERING A KEY INTO PLACE IN A TIGHT SPOT AND TOOK HIS GLOVES OFF TO GET INTO THE SPACE EASIER.

EE WAS IN THE PROCESS OF INSTALLING A THRUST ROLLER ON #2 KILN WHEN THE COME-ALONG FAILED ALLOWING THE THRUST UNIT TO SWING INTO THE EE PINNING HIM BETWEEN THE THRUST UNIT & THE KILN PIER.THIS RESULTED IN A CRUSHING INJURY TO THE EE'S LEFT LEG.

WHILE CLEARING A FEEDER (#4) EMPLOYEE PINCHED HIS RIGHT LITTLE FINGER. THIS RESULTED IN A 1/2" CONTUSION.

EE WAS REACHING INTO A CHUTE TO CLEAR IT WHEN HE FELT A PAIN IN HIS BACK.

WHILE MAKING REPAIRS TO #1 SCREW IN THE FINISH END AN EYE BOLT THAT WAS BEING USED AS A LIFTINGATTACHMENT BROKE, DROPPING THE SCREW ON THE EE'SL. FOOT BREAKING 3 TOES.

EMPLOYEE WAS SPREADING GATE APART ON TRASH BIN WITH A PORT-A-POWER JACK AND EXTENSION. HE MOVED AND STRUCK HOSE ATTACHED TO JACK, CAUSING THE JACK AND EXTENSION TO JACKKNIFE AND HIT HIS FACE.

THE INJURED EMPLOYEE WAS LOOSENING BOLTS ON THE U JOINT FOR A CLINKER HOPPER WHEN THE WRENCH SLIPPED AND STRUCK HIM IN THE LOWER LIP. LACERATED 4 STITCHES.

EE WAS DRILLING THRU STARTER BUCKET DRILL CAME IN CONTACT WITH BUSS BAR, CAUSING A FLASH BURN TO EE'S HAND
