The employee was cleaning a cement coupling with a drill grinder. The drill bit became dislodged from the drill and lacerated the employee's left index finger.

The employee was removing a rock from the grizzly head pulley at the 551BC101 conveyor belt when another rock fell and smashed employee's left index finger.

The employee was walking up the stairs on the front (west end) of the building and missed the top/last step. Employee fell face first and opened a 2" gash above right eye.

The employee was working on belt 611FN360 when their finger reached a pinch between the belt and the sheave. The ring and pinky fingers of their right hand were lacerated.

EE was putting new drive belts on the tail pull. As EE rolled it over EE's finger became caught between the pully and the belt causing a pinch.

The employee was driving the quarry pick-up truck and stopped to speak with the luber. As the conversation continued, the employee began to walk backward to the pick-up and lost EE's footing. The employee stumbled backward, and EE's left leg hit the bottom corner of the driver's door, causing a laceration. The employee caught self on the door and did not fall.

The employee was working on pump #331PP102, rotating coupling. The employee pinched their left index finger between the locking collar and bearing housing.

Employee lifted an elevator bucket from the ground to the back of a truck.

Employee was cleaning the hydrated lime system. They pulled the rubber hose connected to the inductor, and the fitting broke off. The employee fell backward and hurt their lower back. The incident occurred on February 8, 2023; the employee refused medical treatment. Employee decided they wanted to see a physician on February 13, 2023, where modified duties were placed.

Two employees were working on the blending limestone way-feeder, removing pins. One of the employees was pulling pins when the other employee swung a four-pound sledgehammer and smashed left index finger, causing a fracture.

The employee stepped out of the excavator, to check the ditch and stepped into a hole, and pulled their back.

The employee was walking under the under-screen conveyor without a hard hat. When the conveyor started to move, approximately a three-inch rock struck the employee on the head, causing a laceration.

The employee used a pallet jack to push pallets to get them closer to the edge of a trailer, where a forklift would pick them up and transfer them to another location.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. The jib was not adequately stowed correctly; the crane operator telescoped the boom out, and the fly-tip head sheave was pushed towards and bent the front fly storage bracket. This caused the jib to swing out violently, striking two of the four employees.

Four employees were stowing the jib on a Link-Belt RTC 8090 crane and were on the platform of the crane. The jib was not adequately stowed correctly; the crane operator telescoped the boom out, and the fly-tip head sheave was pushed towards and bent the front fly storage bracket. This caused the jib to swing out violently, striking two of the four employees.

Driving on a Taylor Dunn cart to a freight elevator onsite. Starting to feel pain while walking the stairs.

The loader operator was loading a haul truck. The loader operator had loaded the last load and blasted the horn, haul truck operator started to drive forward when a loud and hard impact was felt on the driver side of the haul truck. The loader bucket hit the haul truck on top of the bed. This impact whiplashed the haul truck operator, causing a neck strain.

The employee was removing the filter housing, to change the fuel CNG filter on a truck while standing on the first step of the stepladder. As EE put force on the pry bar, it caused the socket to slip off the nut, causing EE to fall from the ladder. EE landed on EE's left leg, where EE thought EE pulled a muscle.

While going up the stairs to the coal mill, the employee was carrying a bag of gypsum with both hands, and lost EE's balance and fell. The employee stated that carrying the bag, prevented EE from catching the handrail. EE's left ankle was twisted from the fall.

The crew was installing new parts in the clinker cooler. When EE felt sharp pain in my lower back. The following night shift there was tightness/tingling in Right leg and hip. After no improvement on night shift on 2/12/2022, EE reported it to safety at 5:30am, 2/13/2022. The object the employee was carrying was approximately 18 lbs.

Employee was cleaning sprayers on a downcomer and when putting in the long sprayer it slipped and crushed employees left pinky.

Investigating how to open the coal bin to clean it out. The explosion door was open then closed. Hot gases came out and burned the employee.

Opening fine coal bin, closed the bin. The explosion door blew open and flames came out burning the employees.

EE was cleaning off the back of the welding pickup truck (flat bed). When finished, EE "jumped" off the back of the flatbed onto concrete ground. EE stated did not feel any pain or discomfort at this time to classify an injury. EE reported the event the following morning as EE began to feel pain and noticed swelling, but did not request medical treatment until 10-06-2021.

Employee hit a bearing race with a hammer. the bearing race shattered and a piece of metal struck employee in neck.

Employee torched to remove a bad bearing and slag build-up. Employee used a grinder to remove the slag when a small piece of steel flanged from the grinder into employee's right underarm.

The employee was walking down the tower stairway. In the process of turning to the next flight, EE sprained EE's left knee.

Employee was working on cleaning a mixer at the conclusion of a job. The equipment was secured and properly deenergized, the employee then started to clean the tub and mixing blades of the mixer. As the employee was manually turning the mixing blades and removing material, EE's finger was pinched between the mixing blade and the tub causing a laceration.

Customer driver fell from the top of truck while opening the lids to be loaded with cement. Driver did not utilize the adjustable chains to ensure the safety cage was all the way down to close gaps posing fall hazard. The driver also was not properly aligned leaving gaps in which worsen the potential fall hazard.

Employee was loosening base bolts on the #1 bearing assembly drive end of FM1 when the #2 cylinder valve broke off and struck the employee on the inner side of the left leg.

Employee conducting maintenance on broken screw. Applying tension w/pipe wrench & come-a-long, during prep for welding - the wrench slipped causing employee to fall backwards, hitting elbow on guardrail. Injury non severe so employee continued to work. Employee realized the next morning that the pain had not gotten any better and pursued further evaluation.

Employee was using a cardox tube to shoot and clear a plug in a port on the preheater tower. When employee removed the tube from the port minimal material was released and worked it's way under employees PPE (Aluminized Coat/Aluminized Gloves) causing minor burns to employees left wrist.

Mechanic was removing a rear bumper and spare tire from a pickup truck on a hydraulic lift. Employee utilized co-workers for help removing both parts. Employee did not notice any discomfort at time of removal. After completing the task and taking next scheduled break, employee realized that ee had discomfort to lower back.

Operations of heavy equipment over a "long period of time". Employee states that EE has lower back pain, not directly related to one single incident. Employee on restricted duty, not lifting over 10 lbs., no mobile equipment operations, and "sitting work only".

CAT 777D operator was moving waste to the waste dump at mack's peak. The operator dumped load and started to drive away with the bed in the up position. The operator traveled 340 feet before making contact with a 12kv overhead power line. The power line made contact with the raised bed causing the line to arc and pole to break. No injuries occurred during this incident.

EE was walking and stepped on a rock and heard a pop in right knee. Pain was minimal EE did not seek medical attention until 7/19/17. EE treated as FA and released back to work with no restrictions or pain meds. EE went back to Dr on 8/24/17 and placed on modified duty. EE stated knee still pops and pain is still lingering.

EE reported injury on 5/11/17 but did not want medical evaluation until 5/12/17. EE stated while walking during shift felt a sharp pain in lower back. EE cannot recall anything they did that caused the pain. EE taken for medical evaluation and placed on modified duty for 4 working days.

EE was taking the passenger elevator on the preheater tower when a gust of wind moved the power cable and got attached to 8" piece of conduit. The power cable wrapped itself around the conduit and pulled the power cable apart, electrically disabling the elevator car. Fire rescue and Executive Elevator had to be called to rescue the EE from the elevator car.

The EE was hammering in pins on feed chute, while hammering the pins the EE struck the pin and EE's hand at the same time. The pin caused a laceration that required stitching to close. The EE was taken off work for two days and placed on modified duty after that.

EE was clearing dust collector pipes, when EE was clearing the pipe the tipping valve started moving and hit EE above the left brow causing a laceration that required medical attention. The laceration was closed and EE placed on modified duty.

EE was performing pre-shift examination of EE's 992C front end loader and while walking around the backside of the loader EE stepped on a rock that caused EE to loose EE's balance. EE twisted EE's knee and heard EE's knee pop. EE was taken for medical evaluation and placed on modified duty.

EE was welding flange on top of station 4 silos, when EE finished welding on one side of the flange the EE lifted welding hood to inspect work and move to the other side of the flange. While the EE was stepping to the other side the welding hood dropped in front of face during mid-step causing EE to fall onto shoulder. EE placed on modified duty.

EE was attempting to cut a plastic vacuum hose for the vacuum truck and while doing so the EE cut towards left hand that was being used to hold the hose with male fitting on the ground. The knife skipped across the hose and cut the EE's left hand. The cut required 5 stitches to close and EE was placed on modified duty for 4 days.

EE was installing hydraulic ram at top of finish mill elevator. EE attempted to kneel down to work from his knees and hit his right knee on the bearing of the mill elevator. EE reported the injury on 11/14/15 and seen at Dr. office and placed off work until 11/17/15. Returned on modified duty and released to full duty on 11/20/15

Employee was at end of shift storing his tools in his cabinet, which is the normal location when he heard and pop and felt pain in his right shoulder.

Employee was using a truck rack, as he was pulling it back into stored position he felt a pain in his right knee. Employee refused medical treatment at time incident was reported. Requested medical treatment that following Monday. Employee had placed himself in an awkward position which contributed to this incident.

Employee was using an air lance, wearing all appropriate heat resistance clothing when material blew under heat resistant hood. Hot material was blown on employee's face (nose, cheeks, brow and upper lip)

Employee was using a sledgehammer to drive a wedge behind liner. A small piece of metal broke off and hit him in the stomach.

Employee was stepping out of storage shop, stepped on pebble twisting/spraining right ankle.

Employee was wearing appropriate heat resistant protective clothing working on shutting off the air to an air cannon, as he shut off the air flow, the air cannon "popped" causing hot material to become airborne. Material fell down the hot work gloves causing burns to both wrists.

Employee was pushing a broom, cleaning up coal from around the coal unloading area. She stated she stepped on a piece of coal which caused her left knee to twist. She denied medical treatment and working through the rest of her shift without any issues (incident happened at 9:00 a.m. employee's shift ended at 3:30 p.m.)

Employee stated that he strained his lower back while operating a loader maybe 2-3 months ago but is unsure but know he did it at work. Employee stated that the bouncyness in the loader aggregated his lower back.

Employee removing hammers, dropped section onto his right hand causing abrasion to his right pinky finger

Employee was using cutting torch to remove slag, piece fell onto a rock and bounced across right eye- mild abrasion to right eye.

Employee was operating a loader, as he pushed the loader bucket into the stockpile, he stopped. During that time a rock rolled from under the bucket to under the left front wheel. As the operator backed away from the stockpile the left front tire drove over the rock, causing the tire to slam down on the ground.

Employee slammed right index finger in door.

Employee did not report incident until the following day - 3/20/2013. Employee stated he pushed the door of the Loader open and the door swung back hitting the employee's left elbow.

Employee backed truck into ballard, causing tightness in his lower to mid-back

Employee was moving a portable welding unit when right hand was placed between trailer tongue and elevator platform-contusion to right hand, index finger

Employee jumped from frame of 992C loader to ground, estimated 3 feet instead of properly dismounting unit. Tail bone came in contact with bolt on frame of bucket.

Employee using table saw to cut wedges and shims when wood bound in safety guard and kicked out causing blade to contact nail portion of thumb.

EMPLOYEE WAS WEARING SAFETY GLASSES, NOT GOGGLES AS INSTRUCTED TO CLEAN OUT ELECTRICAL CABINET, DUST GO UNDER GLASSES AND INTO LEFT EYE. INCIDENT OCCURRED, ACCORDING TO EMPLOYEE ON 4/3/12 BUT DID NOT REPORT UNTIL 4/5/2012 WHEN MANAGER QUESTION EMPLOYEE ABOUT HIS Red, irritated LEFT EYE. EMPLOYEE WAS SEEN AND RELEASED FROM DOCTOR ON 4/5/2012 WITH NO WORK RESTRICTIONS.

Employee was bent over, swung around to left and hit his upper lip, left side, on standing guard. Cut required a couple stitches.

Employee reported incident on December 15, 2011, which he stated occurred on December 13, 2011. Employee stated that while performing housekeeping duties in the storeroom he sustained a pulled muscle in his left shoulder.

Employee was repositioning dumpster onto the forks of the forklift when the dumpster shifted pinching his left middle finger in-between the dumpster and the frame of the forklift. (Placing body parts in-between a stationary and moving piece of machinery.)

Employee working in area where clinker is stored, employee made the decision to travel off the designated path into an area where clinker is stored, slipping on loose clinker- strained left knee. NOTE: EMPLOYEE DID NOT SEEK ANY MEDICAL TREATMENT AT TIME OF INJURY. MEDICAL TREATMENT WAS SOUGHT ON 10/26/11- PLACED ON MODIFIED DUTY.

Employee cutting up super sucker hose with cutting knife, slipped puncturing the skin on his left knee.

While climbing up the rock transfer tower stairs employee felt a sharp pain in his right knee. Employee later went to the Dr. and was informed that he tore tendons.

CONTRACTOR EMPLOYEE WORKING ON DEMO SITE, WHILE PREPPING STRUCTURAL BEAM FOR TEAR DOWN THE BEAM UNEXPECTEDLY TWISTED HITTING THE EMPLOYEE'S LEFT FOOT. BEAM CRUSHED EMPLOYEE'S LEFT FOOT TOE AREA.

Employee using air lance felt a pain in lower back.

Employee was using two wrenches to break loose a pipe fitting. As he placed pressure/weight on the wrenches he heard and felt a "pop" in his right shoulder. Strain/sprain to right shoulder.

Employee cleaning out plugged pipe with air lance decided to check plug by inserting his left arm inside the pipe. Material fell down from inside the pipe hitting the employee's left arm. Left arm bruised and sore.

EMPLOYEE WAS NOT USING HAND DOLLY FOR MOVING CYLINDERS. EMPLOYEE WAS CARRYING CYLINDER TO TOP OF 9-PACK. HE TRIPPED ON EDGE OF LAST STEP CAUSING EMPLOYEE TO FALL ON LEFT KNEE - SUSTAINED CUT TO LEFT KNEE.

Employee stated injury occurred on Saturday, 6/5/10 but reported it on Tuesday, 6/8/2010 as Report only, no medical treatment requested. Company sent employee to clinic where employee stated he was in a lot of pain. Employee was taken off work for a couple days. Injury sustained was lower back pain while climbing vertical ladders.

DEMO ACTIVITY: NON-MINING OPERATION: Employee was cutting off end cap on beam, which was on the ground. After the cut the employee moved forward to move his welding lead and the beam dropped the 7" onto the top of his steel-toed boots. Employee sustained three broken toes on left foot. Employee is off work until toes heal.

EE was welding at the top of polycom roll. Area was being heated with propane and diesel heaters to maintain the proper temp. of roll being welded per specifications. Goggles and welding hood #12 lenses were being worn. EE flushed eyes when irritation began and during shift, however burning sensation increased by end of shift.

Unattentiveness, not watching where walking, no keeping three-points of contact while descending stairs. While descending stairs employee was carrying items, looked away, missed the bottom step and fell forward onto ground. Abrasion to right knee and contusion to left hand. (Not keeping three points of contact and taking eyes off direction of travel.)

Employee was cutting tape off piece of rope, the rope slipped causing employee to cut left thumb. Employee was not wearing appropriate issued PPE (gloves).

Employee strained his lower back in the process of pulling up supersucker hose, for material clean-up on top of Finish Mill Separator.

LATE REPORT: Employee stated his ankle rolled over while walking towards the time clock on 7/16/09. The injury was not reported to his supervisor until 7/18/09.

Employee was using a hammer drill. As it rotated, the drill handle caught 2 of his fingers and the drill rotated out of his hand. The following day he went for x-ray which revealed hairline fracture to small finger and ring finger on right hand.

Employee was in manlift when the bottom portion of the cage came in contact with the bolts on the outside of the Finish Mill. This caused the manbasket to jerk, causing the employee to hit his left side on the hand rails of the manbasket. Employee was wearing harness with secured lanyard while in manbasket.

LATE REPORTING BY EMPLOYEE. NOTIFIED COMPANY ON 06/11/2009. Employee was picking up trash stood up and hit his head on cross beam.

No Injuries occured when the hydraulic line broke below Clinker Cooler Bedgrate causing oil to be picked up by the cooling fan air flow and taken into the chamber with hot clinker. The oil mixed with the high temperatures caused an ignition.

Injury was reported on 8/21/07 with no work restrictions. Aggravation of the injury, according to the doctor, caused the need for left hip surgery. Employee underwent left hip surgery on 4/27/09. (No accident or injury was reported after 8/21/07).

Late Report of Injury to Employer: Injury occurred on 4/2/09 but was not reported to management until 4/7/09. Employee states he was walking around the Coal Mill when his left ankle rolled over due to walking on uneven ground. Sprain to left ankle.

LATE REPORT OF INJURY BY EMPLOYEE- REPORTED ON 2/16/09. EMPLOYEE STATED THAT AS HE WAS PULLING ON THE BASE OF A STOP SIGN, WHICH WAS BENT, THE BASE BROKE CAUSING HIM TO FALL TO THE GROUND LANDING ON HIS LEFT ELBOW. (STRAIGHTENING OF THE STOP SIGN POST WAS NOT A JOB ASSIGNMENT BY THE SUPERVISOR. THE EMPLOYEE ELECTED TO PERFORM THIS TASK ON HIS OWN.)

Employee late reported accident on 3/16/09. Employee stated that while she was pulling up dust collector bags she felt a pain in her left shoulder. She felt that it wasn't much so didn't report it. During the lay-off she stated the pain was bothering her so when she returned on 3/9/09 she felt she needed to report it on 3/16/09. (Improper Lifting Procedures)

Injury was a contusion below left knee, which occurred while employee closing sectional elevator door. Employee was returned to full duty date of injury. However area started to swell and discolor, so employee when back to doctor on 2/3/09. Doctor lanced the bump and inserted a drainage tube. Employee then placed off work as of 2/3/09.

While employee was hooking up a cable to D9 Dozer, the cable slipped hitting his left ring finger. He was not wearing proper PPE- gloves.

While trying to remove a section of metal that was inside a reject bin, the middle shifted coming in contact with the employee's left ring finger.

While employee was exiting the cab of the water truck he missed the hand grab on the cab, not keeping 3-points of contact, and fell to the ground. Employee sustained abrasion and hairline fracture to right elbow. Employee was placed on modifed duty on 9/29/08 until he was seen by ortho dr. and then placed off duty.

Employee was assisting in removal of a faulty diverter valve using bull rigging when the valve got in a bind. Other employees helped to free it from the bind. When the valve freed from the bind it swung around and caught the employees arm in between the valve and a pipe support stand.

Lifting cumbersome choker cable to put onto storage wheel.

Employee was cleaning #13 rock belt when he slipped and sprained his R/ ankle.

Employee was servicing a filter while standing on an 8 ft. ladder. The ladder slipped and he fell approximately 5 ft. landing on his right shoulder and dislocating it.

Employee was cleaning an inlet shelf with an air lance when hot dust came out of the door and down his glove, burning the top of his right hand.

Employee was cleaning up dust when he stepped into a hole and strained his right knee.

No injuires occurred. Unexplained coal explosion in #5 Coal Mill.

Employee was transporting an empty oxygen bottle on a hand truck when it twisted and sprained his right wrist.

Employee was closing the tailgate on the dump truck when he caught his right ring finger in the latch.

EMPLOYEE WAS WALKING BY # 2 FINISH BALLMILL AND STEPPED ON A CLINKER BALL SPRAINING HIS RIGHT ANKLE.

Removing truck clutch and pressure plate when he felt a pain in his back.

The employee was knelt down performing work activity, when he attempted to stand; the fall protection lanyard got caught under his foot causing him to fall forward. In the attempt to catch himself, the employee fractured the forth Metacarpal bone in the right hand confirmed by physicians x-ray.

Employee was walking along 311 belt conveyor and miss stepped and twisted her right knee.

Employee was getting into his truck when he slipped on wet clinker and sprained his R/ankle and L/knee.

Team member was driving a ( gator ) down a 15 degree slope, he lost control and the equipment flip on its side. The injury to operator was left hand little, ring and middle fingers crushed by the (ROPS) rollover protection.

Employee states strain to lower neck from operating dump truck. Late reporting

Shermco employee sustained a lower back strain on 11-15-07 while performing testing/calabration of RTD's on a fan motor 304 on the second level of the clinker cooler, due to over extending his reach.

Employee was accessing an upper level of the penthouse floor located at the Baghouse (331). Employee grabbed a cable tray that was not fully connected. When the cable tray moved, the employee lost his grip, falling appox. 4 feet. In the attempt to catch himself, employee fractured his right forearm.

While working in the dust dump the employee stepped on the crust covering the dust and it gave way allowing his right foot to enter the hot dust. This caused dust burns to his right foot and ankle.

Employee was using cut-off blade on grinder. The grinder kicked back and struck emplyee on Rt. hand. The contact caused an avulsion requiring closure.

Employee was working inside of # 4 waste heat boiler when a piece or refractory fell striking him on the head. He suffered a laceration on his forehead that required stitches.

Employee was working under clinker cooler installing instrument tubing, felt discomfort to Lt. eye. Employee went to first aid and was transported to physician. Employee had FOB in Lt. eye.

Employee reported discomfort in Rt. eye, and upon examination employee had embedded metal in eye. Employee transported to physician for removal.

Employee was placing a 1 1/4" shackle under a load and caught 4th finger of right hand between the shackle and the load. The employee lacerated and fractured the tip of his finger.

Welding lead fell from preheater, and struck employee on silo cat walk. Employee experienced fractured right ulnar.

Either happened while grinding or chipping or blowing of the kiln shell.

Employee was using the vacumn hose and when he started putting it away he made contact with the hose clamp and burned his left forearm. He requested medical attention on 07/10/07.

Employee's fingers were caught between two pieces of steel.

Employee was rocking material back and forth and experienced a shoulder strain.

Employee's finger was caught between fork truck fork and a piece of material. Employee experienced an avulsion to finger.

Working over wide grating loss footing and stepping into opening

Employee was cleaning out # 11 kiln screw and during this action experienced dust burns to his wrists and ankles.

Employee slipped and fell, landing on a short piece of rebar, causing a leg contusion.

Employee was working on the spring assembly of the service truck when he experienced back pain.

Employee claims that she became overheated while working on #3 Kiln. She was transported to the hospital for treatment and returned to modified duty for 3 days.

EMPLOYEE WAS WORKING ON #3 COOLER (INSTALLING CHAIN ON BEDGRATE DR), WHEN HIS LEFT HAND WAS CAUGHT IN THE SPROCKET, CAUSING A COMPOUND FRACTURE OF THE MIDDLE FINGER ON HIS LEFT HAND.

Employee straddling a beam and was pushed across it by an incoming brace that he was guiding in, causing lacerations on the inside of his thigh.

While working in the feed end employee misstepped on the grating of #13 screw and twisted his left ankle.

Employee was working in compressor room when he dropped a valve and pinched his right hand between the valve and floor plate. This caused a contusion to his right hand.

A guide bar left from the previous shift vibated loose and fell striking the employee on the left instep, causing an contusion. the employee was working on the Hammermil screens.

Employee claims that he slipped while working on the Rock Stacker and twisted his knee. This was originally reported as an incident only on 01/10/07.

Employee was moving a steel beam manually with his hands. The beam shifted and slid onto another beam pinching his left pinky. The resulting injury was a 1/2" amputation.

Employee claims he injured his ribcage (strain) while using a sledge hammer on 3/23. this was reported on 3/29/07

Employee slipped on rock while moving crane matt

Employee was walking across the deck of a rough terrain crane, lost his balance and fell approximately 5 feet.

While jacking equipment, jack kicked out hitting employee in left cheek area which resulted in large gash from mid-cheek to under jaw.

Employee was driving out a bolt using a welders chipping hammer instead of using a punch. A sliver of metal became imbedded in his left hand.

Employee was walking by an area where two employees were working on a loader removing teeth and pins with a sledge hammer. A sliver of steel hit him in the left thigh.

On 12/21/06, an employee was using pliers to tie a piece of tie wire. The tie wire broke and the pliers hit employee in the mouth.

Changing hammers when one of them rolled onto the employees L/ring finger.

Lifting a screw section, he felt a pain in his back.

On 12/11/06 employee was hitting a drift pin with a 8 pound sledge hammer into a bolt hole on a beam. EE swung and missed drift pin, striking his own left thumb.

On 11/24/06 employee was working on a crane boom. A crane hook was coming down in the vicinity and the employee perceived the hook might strike him. The employee jumped down, landing on his feet and rolled forward.

At approximately 9:30 employee was requested by forklift operator to assist in unloading a steel beam. The tips of the forklift forks had placed the beam on its side so that it stands instead of lying. The beam became unbalanced and began to fall. Employee stated that he was in a position at the end of the beam. This resulted in the beam rolling onto his left foot.

On 11/14/06 employee reported an event that occured on 11/8/06. Employee describes stepping on a rock and feeling pain in his left knee. Employee did not report falling to the ground. Doctor diagnosis was left knee strain.

Employee reports while stepping down of off 50 ton RT crane using the crane steps & hand rail, his right foot was caught under the step's rung, causing him to lose balance. He held onto the handrail, however the shift in balance caused him to overexert the right knee.

On 10/20/06 Employee was positioned below another employee, employee overhead fell and landed on employee below. Employee below reported back pain.

Employee was walking along # 15 rock belt when he bumped his head on a piece of conduit, straining his neck.

Incorrect length of sock and safety booths.

Employee claims cause of injury unknown, feels it was repetetive.

Twisting improperly-exiting off forklift(from activity report).

This employee was working in the Finish Mill when a bag of oily rags was thrown from a cleaning area above her and struck her on the head. This was originally reported as a first aid injury. On 09/20/06 a request was made for treatment.

Walking under low clearance areas and raised head striking head causing lower neck pain.

Stepping down from chute and missing landing causing knee to twist.

When crossing over steel beam material, employee's foot caught against the material causing him to fall. Employee stuck out his right arm to stop the fall, striking forearm on material, then when landing on ground, overextending right elbow.

Dissambeling metal bracket; cutting small bolt with hand held power tool. Power tool slipped, small laceration to forearm.

employee states that he was stepping off a curb and felt a pain in his right knee.

Employee stated that while winding up an electrical cable in storage he felt a pain in his lower back.

Employee stated he was lifting a steel cone at the storeroom and felt a pain in his left shoulder.

Employee claims that he bumped his head while getting into pick-up. Reported on 08/02/06

On 7/15/06 employee was stripping concrete forms. Employee was handling forms in an awkward position. When handing down a section of form, the employee felt pain in his lower back.

Injury was originally reported as (report only) on 7/12/06. Request for medical treatment on 10/3/06. Employee dropped a sprocket that he was installing on # 2 cooler, felt right knee pain.

The employee was opening a valve for an airline at our Power Plant when it ruptured and blew the surrounding debris into his face. This caused facial abrasions.

Employee was cleaning out a clogged chute at Hammermill and when he was finished he felt a pain in his right shoulder area.

Wrist impairment (Carpal Tunnel Syndrome)

Employee using sledge hammer to pound out holding pin in bucket of 988 loader. A piece of metal hit the employee's right eye causing an abrasion.

Employee is unsure of when alleged injury occurred. Company was informed on 5/5/06 of the alleged injury. Employee stated that he believes he pulled his back while walking out in the plant back on 5/2/06. He didn't experience any discomfort or pain until 5/5/06.

Employee was working on # 1 Finish Ball Mill using a sledge hammer to clear dust collector pipe when he felt a pain in his lower back, right side.

Employee claims that he stuck something in his R/ring finger while working on #1 cooler on Friday the 17th. He reported this on Monday the 20th and was sent to the medical clinic for treatment.

The employee was in the basket of a manlift using a sledge hammer to clear a clinker pipe when he felt a pain in his right side.

Employee was working in the Layout shop cutting plate when he noticed an irratation in his right eye.

Employee was grinding a belt clip on the Rock Stacker when a part of the splice clip struck him in the chest. This caused a contusion to the right side of his chest. This was not reported until 12/06/05.

Employee was removing hard material from #10 Spitzer belt pulley when he caught his right little finger between the hammer and pulley. This resulted in a contusion. This was originally reported as a (report only).

Changing a Gudgeon bolt in # 4 dust chamber when there was a dust purge and it burned his neck and left ear.

Employee slipped while using a bar to knock down loose material and fell against a ladder causing a bruise to his back and hip area.

While working in the skip basement, employee got a piece of hot clinker on the top of left foot, causing a burn.

Employee slipped on some loose clinker in the storage area and strained his lower back.

A blocking timber fell on his foot.

(REPORTED AS A RECORD ONLY INJURY 8/25/05, REQUEST FOR MEDICAL ATTENTION 8/26/05) Employee was securing vacuum hose for labores when it flushed and struck him on the outside of right knee. This caused a strain.

The employee stopped his truck in order for another person to get out. When the employee got out he shut the door and that activated the drivers side air bag. The air bag then made contact with his right arm burning and lacerating it.

Employee was checking the operation of the stacker and when he bent over he felt a pain in his back.

While attempting to open cement cooler door, with a sledge hammer, employee caught his R/ring finger between the door and sledge hammer.

While working in #3 cooler installing striker plates overhead, something fell into my left eye. I had this removed at the Dr.'s office.

While lifting a 5 gallon can from the bed of a truck the employee strained his lower back.

The employee was cleaning out the cooler on #4 Kiln and the following morning experienced burns (rash) on his neck. This was no doubt caused by hot dust.

Employee was getting out of the Bobcat when she felt a pain in her left knee. This was later found to be a strain.
