Employee was using a sledge hammer to try to free the wear plate on a D10 dozer blade. A metal fragment broke off the plate and struck the employee in the leg.

Employee suffered a neck strain when a sledge hammer head came off of a hammer, striking them on the hardhat. The hammer was being held by another worker as the injured miner was assisting loosening a bolt.

Employee was using a bar to clear a sand hopper. Five days later, employee said elbow was hurting. Employee reported the injury on 12/20/2016.

Employee was installing rubber screens with a slide hammer in the middle deck of a screen. When he was hitting one screen in the bottom of the hammer slipped off the panel and causing the whole slide hammer to fall suddenly and smash his finger between the handle of the hammer and the bucker bar rail on the top deck.

Employee was on p-8 stacker to perform a belt splice. While securing the tail of the belt to the splicing board the employee struck his right thumb with a hammer.

While removing the use liner from the head of an Omni Cone Crusher, the employee was lifting with a crane and hitting the liner with a 3 lb hammer to break it loose. The hammer struck the employee's right middle finger.

Employee was prying worn rubber that had been siliconed to a chute with a crowbar. When the rubber came loose the crowbar slipped from the employee's grasp causing the bar to drop to the ground which caused the employee to drop to his knees striking his left knee on the center hold down bolts of a screen.

While using a 12 lbs hammer to clear rocks form a vibrating grizzly screen, the employee struck the right index finger with the hammer.

The employee began to notice pain in his low back after using a 16 lb hammer to install a head bolt of a crusher. As the pain began to intensify the employee requested medical intervention for the low back pain.

The employee was clearing out a head-box with a long bar when his left hand became caught between the bar and a piece of angle stock inside the box.

While cutting flag cable for a water conveyor extension with a short handled four pound sledge hammer, the hammer missed the correct contact. It instead struck the employee on the webbing of the left hand between the forefinger and thumb.

The employee was using a utility knife to open a package of padlocks for his work locker. He bended his knee and used the upper thigh as a wooden block for cutting purposes. The utility knife slipped and sliced open the left leg, upper thigh.

The employee was hammering a keeper pin in a new ripper shank guard on D11 dozer. He steadyed the guard with his legs while hammering, striking his left knee with the hammer.

The employee was removing the outside bolts of a tailpulley take-up unit. The inside bolt was missing from the unit which allowed the top of the take-up unit to come loose, striking the employee on the left side of his face.

The employee was using a sledge hammer to know loose a used bucket tooth from it's chank. A small, sharp fragment flew off the edge of the tooth and impaled the employee in the right calf muscle.

While removing a rock from a conveyer belt with a shovel handle, the employee was struck on the left hand thumb with the other end of the shovel. The left thumb then struck the side of the conveyer.

The employee was cutting a piece of conveyor belting. While doing so, his right hand slipped causing the razor knife he was holding to strike his left thumb.

EMPLOYEE WAS CUTTING RIBBER BELT WITH A RAZOR KNIFE WHEN THE KNIFE SLIPPED AND STRUCK THE EE'SRIGHT KNEE.

EE WAS REPAIRING A BROKEN HYDRAULIC HOSE ON THE LOWER PORTION OF DREDGE CLAMSHELL BUCKET RAM. HE WRAPPED A CHAIN COME-ALONG AROUND THE SHAFT OF THE RAM TO MOVE THE RAM BACK FOR ACCESS TO THE HOSE FITTINGS. AFTER TENSION WAS APPLIED TO THE COME-ALONG, THE EE REACHED BEHIND THE RAM TO REMOVE THE BROKEN HOSE & THE HOSE FITTING. THE CHAIN ATTACHED TO THE RAM SLIPPED & ARM WAS CAUGHT.

The employee was tightening the conveyor belt on F1 conveyor. Employee hit is left hand fingers between a ratchet handle and conveyor frame.

EMPLOYEE, ALONG WITH TWO OTHERS, WERE CHANGING BUCKET TEETH AND SEGMENT ON EQUIPMENT. THE TEETH AND SEGMENTS FIT OVER ADAPTERS SECURED WITH PINS THAT ARE DRIVEN THROUGH ALIGNMENT HOLES IN THE WEAR PART AND THE ADAPTOR. ONE OF THE SECTIONS WOULD NOT LINE UP WITH THE PINHOLE SO THE EMPLOYEE TAPPED ON IT WITH A HAMMER. A METAL SPLINTER FROM THE SEGMENT FLEW UP AND HIT THE EMPLOYEE IN

EE WAS USING A BOTTLE JACK AND A PIECE OF PIPE TO RAISE THE ENGINE ON A LOADER. HE RAISED THE FRONT OF THE ENGINE AND INSTALLED A 1" PLATE TO MAKE ROOM TO REMOVE THE OIL PAN. THE EMPLOYEE LOWE RED THE JACK AND REMOVED THE JACK AND PIPE. THE PIPE SLIPPED OUT OF HIS HAND AND FELL ON HIS ARM CAUSING A LACERATION TO THE LEFT ARM.

EMPLOYEE WAS DIGGING DIRT AND GRAVEL FROM AROUND THE BASE OF OMNI #2 CONE CRUSHER. MUSCLE SPASMS IN THE BACK OF THE RIGHT SHOULDER CREATED PAIN FOR THE EMPLOYEE.
