The tunnel belt head box became jammed with rock which was impeding operation. Trying to clear the jam with a pry bar, the workers hand was impinged between the bar and I beam.

Driller smashed right ring finger while using a hammer to loosen the bit from a drill steel. The hammer had a glancing blow, allowing driller's hand that held the handle to strike against the bit.
Employee sprained his left shoulder when a wrench slipped as he was tightening a hose fitting bolt on a drill. When the wrench slipped, he extended his left arm and hand to catch himself from hitting the boom.

Worker received contusion and small laceration to his left ring-finger as he attempted to free-up a roller on a moving conveyor belt by using a metal bar to dislodge stones from the roller. The metal bar got caught and pulled in by the conveyor motion, causing his hand to strike the side of the moving belt.
Worker was cutting a section of rubber conveyor belting when razor knife unexpectedly cut through the belting resulting in a laceration to his left upper leg. Worker received medical care including 3 sutures and anti-biotic medicine. He was received a full medical release and returned to work without any lost time.
Employee was holding a piece of drill steel as a punch which was struck with a hammer to drive out a pin. A piece of the steel broke off and impacted the side of the employee's head, giving him a laceration.
EMPLOYEE WAS REMOVING THE DRIVE SHAFT ON A LOADER WITH PRY BAR. THE DRIVE SHAFT FELL AND HIT THE PRY BAR CAUSING IT TO HIT THE EMPLOYEE IN THE CHIN. STITCHES WERE NECESSARY.
EMPLOYEE WAS REMOVING BUCKET TEETH FROM BUCKET USING A SLEDGE HAMMER TO DRIVE TOOTH OFF ADAPTER. A PIECE OF THE TOOTH BROKE OFF AND STRUCK EMPLOYEE IN HIS LEFT UPPER CHEST. THE PIECE OF METAL HAD TO BE REMOVED IN AN OUTPATIENT SURGICAL PROCEDURE. NO PERMANENT DAMAGE IS EXPECTED.
Working on a site after a misfire; working with a knife. Knife slipped, cut finger.
Employee was trying to dislodge a rock from a feeder pan with a bar. The bar slipped from the rock, and the employee's finger was pinched between the bar & chute wall. This resulted in three stitches on right hand, pinky. Employee was back to work the next shift with no restrictions.
Employee's hand was caught between shovel handle and side of conveyor while digging out tail pulley resulting in a contusion.

EE WAS USING RAZOR KNIFE TO CUT A PIECE OF NOSING FROM SCREEN CLOTH WHEN KNIFE SLIPPED, LACERATING THUMB ON LEFT HAND.
EMPLOYEE WAS CUTTING PIECE OF CONVEYOR BELT. PULLED KNIFE TOWARDS HIM AND ACCIDENTALY CUT HIS RIGHT KNEE.

EMPLOYEE SHOVELING MATERIAL CLOGGED IN PIPE WHEN SAND SLUG BROKE LOOSE. HE WAS SPRAYED WITH HOT WATER AND SAND, BURNING ARM AND CHEST AND SLIGHT AREA ON FACE.
EMPLOYEE WAS CUTTING FLASHING WITH UTILITY KNIFE--SLIPPED AND CUT HIS RIGHT LEG - 3 STITCHES.
EE WAS TIGHTENING BOLTS ON A SCREEN DECK AND DIRT FELL IN HIS EYES. THE EE WAS WEARING SAFETY GOGGLES AT THE TIME.
