Employee was standing next to another worker. The other worker struck a sledge hammer against a machine sprocket. A sliver of metal dislodged and struck the injured employee on the left cheek. There was a small laceration.

Employee was using a hooking tool to move a rock lodged inside of a machine. When he pulled on the tool, his right hand struck a metal guard rail smashing his thumb, breaking the tip joint of his thumb.

He jerked to pry a rock loose and strained his right shoulder. after monitoring his condition for a few days, he decided to receive treatment at a clinic. he was placed on modified duty and returned to work without loosing time from work.
EE WAS USING A PIPE WRENCH TO RELEASE A FITTING FROM A MACHINE. THE FITTING RELEASED AND THE WRENCH HANDLE STRUCK EE OTHER HAND AND MASHED IT AGAINST THE EDGE OF A FLANGE. EE RIGHT RING FINGER TIP WAS SEVERELY MASHED CAUSING FRACTURE WITH A CONTUSION, SPLINT AND THREE SOUCHERS NEEDED EE WAS RETURNED TO FULL DUTY ON THE SAME SHIFT
A "SPIDER" BUSHING WAS BEING REMOVED FROM THE SECONDARY CRUSHER. A COME-A-LONG WAS BEING USED & WAS UNDER TENSION. THE COME-A-LONG SNAPPED AND HIT THE INJURED AS HE WAS WALKING BY. THE SAFETY LATCHES ON THE COME-A-LONG WERE IN ORDER. TAKEN FOR MEDICAL TREATMENT, REMOVED FROM DUTY, CAST TO BE APPLIED ON 6/26/03 AND RETURN TO DUTY.
EMPLOYEE WAS TIGHTENING A BOLT BACK IN PLACE WHEN ANOTHER EMPLOYEE RELEASED A COME-A-LONG THAT WAS HOLDING THE FRAMEWORK WHERE THE BOLT WAS LOCATED. THE COME-A-LONG SNAPPED AND STRUCK THE TIP OF THE FINGER INFLICTING A LACERATION THAT TOOK 4 SUTURES TO CLOSE. THE EMPLOYEE WAS RELEASED TO WORK AFTER TREATMENT.
EMPLOYEE WAS USING A HAMMER WITH HIS RIGHT HAND TO UNLOG A PIPE BLOWLINE AND FAILED TO SEE WHEREHE HAD HIS LEFT HAND POSITIONED. HE STRUCK HIS LEFT HAND THUMB WITH HAMMER, SUSTAINING A CONTU- SION AND FRACTURE TO VERY TIP OF LEFT THUMB; THEWOUND WAS CLEANED; FINGER GUARD APPLIED; FOLLOW-ING DAY TAKEN TO CLINIC TO HAVE PRESSURE UNDER NAIL RELIEVED; X-RAYS SHOW FRACTURED THUMB TIP.
