While attempting to move a contractors trailer, a Plant Maintenance EE was cranking up a tongue jack that was very stiff to turn. After EE was done, EE noticed that EE had some pain on the outside of the right elbow.
Plant Maintenance employee was trying to free up a preheater ram/block that was not moving with a come along to asist the hydraulic pressure of the ram cylinder. While doing so the come along broke off the hooked end and struck the employee in the right hand.
Employee was using a plasma cutter to make a hole in metal. When employee was striking the metal with a hammer to knock out the piece in the hole, employee felt debris fall on face. Employee wiped the debris off and accidentally rubbed it into eye.
Employee was swinging a sledgehammer to strike the side of a metal structure to dislodge choked material built up inside. Swinging the sledgehammer caused employee's arm to feel weak and sore.
ON 2/16/16, EMPLOYEE WAS SWINGING A SLEDGEHAMMER TO STRIKE THE SIDE OF METAL STRUCTURE AND DISLODGE MATERIAL BUILT UP INSIDE. SWINGING THE SLEDGEHAMMER CAUSED GREAT PAIN TO DEVELOP IN LEFT SHOULDER. Requested medical attention on 2/22/16.
Employee was hitting the outside of Hydrate Bin #3 with a hammer to dislodge choked up material. As he was striking the bin, a cap on an outside spout popped off. Lime shot out of the spout, went behind his glasses and into his eye causing an ocular burn.
Employee was attempting to remove some hard lines on the block inside Loader. The wrench slipped and his right thumb struck against the motor block and peeled his thumb nail backwards, separating it from the nail bed.
Employee was using a shovel to clean up fines material. When he forcefully drove the shovel into the pile, the shovel's head struck a rock that was hidden under the fines material - which he did not see. The shock/force caused trauma/pain in his elbow.
EMPLOYEE WAS USING A T-BAR TO A CLEAR MATERIAL FROM A CHUTE. WHILE PRODDING THE MATERIAL INSIDE THE CHUTE, IT BROKE AWAY CAUSING THE T-BAR THAT HE WAS GRIPPING IN HAND TO SLIDE FORWARD. HIS RIGHT PINKY FINGER GOT SANDWICHING BETWEEN THE T-BAR HANDLE AND STRUCTURE CAUSING A FINGER FRACTURE.
Employee put a fan blade in a vice grip and used a pry bar to try to straighten it out. He placed his other hand on the vice to get leverage. While pushing down on the blade with the bar, the fan blade spun and stuck his left index finger causing a laceration and fracture.
Employee was striking a piece of metal that had become stuck between the roller on the 10A belt conveyor and the chute with a sledgehammer to try to dislodge it. The sledgehammer head bounced off the metal and the sledgehammer head and shaft struck the employee in the mouth, chin and cheek.
Employee was changing out the old skirting on the north side of the 14A conveyor belt. He lay the old skirting across his legs and began cutting it with a utility knife to shorten it down. As he was making his first cut, the knife slipped and the blade stuck him in his right thigh causing a laceration requiring 4 stitches.
Employee was hitting the chute between 7A and 10A conveyors with a sledgehammer in order to loosen choked material. As he was swinging the sledgehammer, he felt pain in his back.
Employee was shoveling a pile of material. He stuck the shovel into the pile and material sprayed behind his glasses and into his left eye. Employee developed superficial eye injury to the cornea.
Employee was using a pry bar to clean around the pulley on the 12C conveyor. As he was pushing down on the pry bar, it slipped from his left hand. The pry bar fell on top of his left hand. His hand was crushed between the pry bar and the pulley structure.
Employee was using a sledgehammer to hit the damper to unchock material. The sledgehammer got hung up on the rails as they swung it back causing his elbow to bend backwards. Employee reported accident on 01/18/12. Employee requested medical treatment on 02/16/12 and given restrictions.
Employee reported feeling pain in her forearms when she had to manually open the hydrator gate, which is done by turning the gate wheel with a 10 foot long metal rod.
The employee was pulling ash ring off the wall of the kiln using a pry bar tool. He lost his balance, stumbled backward and landed on his back.
Employee was using a small hammer to loosen the flint from the limestone on the quarry floor. Employee struck a piece of flint to loosen the material, and some material struck his right hand between the thumb and 1st finger.
EE WAS WASHING A PICKUP TRUCK. AS HE BENT OVER TO USE A LONG-HANDLED BRUSH ON THE SIDE OF THE TRUCK, HE FELT A PULL IN HIS LOWER BACK.
EE WAS REPLACING THE BOTTOM SPROCKET & SHAFT ON A KILN BUCKET ELEVATOR, USING THE HAMMERS TO LOOSEN THE SHAFT FROM THE SPROCKET. A METAL SLIVER CAME LOOSE FROM THE HAMMER, PUNCTURED HIS GLOVE AND BECAME EMBEDDED IN HIS LEFT PALM.
EMPLOYEE CLAIMS THAT HE INJURED HIS LEFT MIDDLE FINGER WHILE ATTEMPTING TO OPEN A MANUAL GATE ON A SCREW CONVEYOR BY STRIKING IT WITH A HAMMER. HE FURTHER CLAIMS THAT WHEN HE SWUNG THE HAMMER, THE HAMMER HIT THE TIP OF HIS LEFT MIDDLE FINGER. HE WAS RELEASED TO RETURN TO LIGHT DUTY AFTER THE LOSS OF ONE DAY OF WORK.
EE CLAIMS THAT HE GOT HIS LEFT RING FINGER CAUGHT BETWEEN A SHOVEL HANDLE AND A GUARD RAIL WHILE HE WAS SHOVELING SPILLED CRUSHED STONE IN ROTARY KILN #3 TUNNEL. HE DECLINED TO SEE A DR UNTIL 8-9-02 AND WAS RELEASED TO FULL DUTY. ON 9-3-02 THE EE UNDERWENT SURGERY FOR REPAIR OF A TENDON. HE LOST ONE DAY OF WORK AND WAS RELEASED TO RETRUEN TO WORK ON LIGHT DUTY.
EE GOT A PARTICLE OF DUST LODGE IN HIS LEFT EYE WHILE USING A POKING ROD TO BREAK UP ASH RING FROM THE DISCHARGE END OF ROTARY KILN #3. HE CLAIMS HE WAS WEARING SAFETY GLASSES AT THE TIME. HE WAS SEEN AT THE MEDICAL CENTER AND EXCUSED FROM DUTY UNTIL HE CAN BE EVALUATED BY AN EYE SPECIALIST.
EE CLAIMS THAT WHILE HE WAS STRIKING THE SIDE OF A STONE BIN WITH A SLEDGE HAMMER, HIS FINGER GOT CAUGHT BETWEEN THE HAMMER HANDLE AND THE EDGE OF A BIN VIBRATOR. HE RECEIVED A CONTUSION TO HI S LEFT RING FINGER. HE WAS SEEN AT CLINIC AND RELEASED TO LIGHT DUTY. NO LOST TIME INVOLVED.
WHILE TRYING TO FORCE OPEN THE TAILGATE OF A DUMP TRUCK WITH A PRY BAR, HE SPRAINED HIS RIGHT SHOULEDER. HE WAS EXCUSED FROM DUTY BY HIS DOCTOR UNTIL HE IS ADMINISTERED AN MRI.
EE SPRAINED HIS RIGHT ELBOW WHILE BEATING ON A BIN WITH A 8 LB SLEDGE HAMMER. HE WAS BEATING ONTHE BIN TO BREAK LOOSE CRUSHED STONE TO ALLOW ITTO FLOW INTO A HAUL UNIT. HE USED THE HAMMER FOR ABOUT 5 MINUTES BEFORE HE CLAIMS HE FELT PAIN INHIS ELBOW. HE WAS RELEASED TO RETURN TO LIGHT DUTY BY HIS PHYSICIAN.
EMPLOYEE CLAIMS THAT HE WAS ROTATING A SHAFT ON TEH PULVERIZED BAGGER WITHA PAIR OF CHANNEL LOCKS WHEN THE SHAFT RELEASED SUDDENLY CAUSING HIM TO STRIKE HIS LEFT ELBOW AGAINST THE FRAME. HE W AS SEEN AT PRO-MED CLINIC AND RESTRICTED FROM USING HIS LEFT ARM, HE IS EXPECTED TO RETURN TO WORK ON 5-9-00.
