He was putting a steel insert behind the shaft of the head pulley at the C.R.S. Two employees working on opposite side of solid guarded pulley had hoisted up the pulley when the hoist shifted and broke. The head pulley shot back (due to the conveyor belt) and the plate hit his hands.

EMPL WAS BREAKING UP CLAY BOULDERS IN CLAY RECEIVING PIT AND STRUCK HAND WITH CLEANING/CHOPPING POLE AGAINST SAFETY RAILING CAUSING LACERATION AT INDEX FINGER ON RIGHT HAND.
