After unloading, EE drove to the cleanout to ensure the tubs were empty. Using a sledgehammer to tap the side and vibrate the remaining contents, it rotated in EE's hand, catching EE's glove, twisting EE's left thumb. EE was fitted with a splint to immobilize the thumb, was referred for a follow-up appt. w/ ortho and released to work w/o restrictions.

While prying bar slipped and chipped tooth

Employee was cutting sheathing from wires when tool slipped and cut EE's hand through EE's gloves.

Removing GET from lip of bucket. Was using a pry bar and soft face hammer to work the old tooth off with the crane attached. The tooth was moving and I was using the hammer to walk the adapter forward. When hitting the adapter a piece of the corner of the adapter splintered off and hit my left inner thigh.

While cutting rubber, the box cutter slipped and cut employee's forearm. Resulting in stitches

While using hand tool, tool contacted face shield causing laceration to mechanic face.

While replacing a chain link on the drill head crew was using a chain come-along to pull the slack out of the chain to install new link when the come-along slipped from its anchor point coming in contact with then assistants knee causing a cut.

Mechanic pinched finger between tool and car body of machine EE was working on while torqueing down bolts.

While attempting to remove a filter, the socket slipped causing employees hand to be pinched.

An employee was using an auto retracting knife with hook blade to cut through a liner weld to inspection it As EE cut the liner the blade met resistance at the weld seam. EE pulled harder to cut the weld seam. The blade cut through the weld seam and struck EE in the left leg. EE got a 2" cut which required 8 stitches and a tetanus shot.

Employee was cutting a length of electrical cable for installation in CIC-2. The employee was using a Klein cable splicer's knife to cut the zip tie holding the end of the cable secure to the reel. When the blade cut through the zip tie and the blade struck back of the employee's wrist cutting through the cut gloves that EE was wearing.

Employee was using a utility knife to cut a hose, the knife slipped and cut employee's arm. Resulting in stitches.

Employee was removing an exterior weld bead from a 10" HDPE pipe using a cold chisel. The employees hand and cold chisel slipped contacting EE's left forearm causing a laceration requiring 3 stitches. Employee was immediately released to work with no restrictions following medical treatment.

While driving a pin from a CAT 16M grader mechanic missed the pin driver and struck left hand with the hammer. Initial X-rays from the ER did not show a fracture. After the visit with the orthopedic specialist on Sept. 6th the mechanic was diagnosed with an oblique fracture to the 4th metacarpal.

Employee was swapping over engine and radiator and was removing old hose from lower cooling line on trk467 793D haul truck. He was using a pocket knife and cutting away from himself and had both hands pushing one on top of the other for better leverage and the knife slipped out from under hose and cut right little finger.

The dozer operator was assisting a mechanic change a ripper tooth on his cat and a piece of metal caused a laceration in his leg when the tooth was struck by a hammer.

While striking a pin on a dozer with a sledge hammer, pinched finger between hammer handle and dozer tool bar.

EE was working with a wrench to loosen a bolt off a 777 haul truck while doing so ee was putting his full body weight into it when the wrench slipped off the bolt. He fell forward striking his face on the 777 haul truck cutting his bottom lip. He was checked out in town, received 2 stitches and returned to work the same day.

According to IE , he was working on tightening a mirror on a truck in the lay down yard when the wrench he was using slipped off the nut striking him in the mouth chipping his tooth. EE reported the incident that afternoon and refused medical treatment. EE RTW with no lost time.

While using a screwdriver to tighten a screw the employee ruptured their right biceps muscle.

Employee was stripping the outer insulation off of #6 gauge submersable electric wire (outer diameter 1-1/16") utilizing a razor knife. The knife blade slipped/skipped off the poly-vinyl insulation. Upon slipping off of the insulation, the knife blade came in contact with the inner left leg, approx 4" above the knee, causing a laceration.

Cut right index finger with box cutter.

Employee struck by a small piece of metal that flew off from hammer strike causing a small laceration.

Attempting to dislodge pipe from frozen ground with a prybar. Prybar glanced off pipe and came into contact with right foot causing a minor contusion.

A small 1/4" piece of metal became lodged under the skin of employee's little finger of right hand while fellow employee was hammering bucket wear metal package off Hitachi 3600 shovel bucket they were rebuilding.

Employee was swinging a 20 lb. sledge hammer, driving a piece of all thread to remove pan liner bolts from the SAG mill. The hammer glanced off the all thread and pinched the employee's finger causing a laceration which required sutures to close.

As Mechanic was tightening a bolt the wrench slipped and cut across the employee's left thumb. Laceration required stitches.

A maintenance employee was repairing a water leak on a drill rotary head utilizing a 36 inch pipe wrench. While pulling on the pipe wrench the employee felt a sharp pain in his lower stomach. The employee was seen by a physician and diagnosed as having an abdominal strain.

Mechanic was replacing a lip protector on a bucket of an hydraulic shovel. A second employee struck the lip protector with a metal hammer which caused a piece of metal to fly off striking the mechanic in the mouth cutting his lip.

EE was stripping foam off of HDPE pipe when the knife he was using slipped and cut his finger resulting in 2 stitches. He was trying to remove too much at one time.

Employee was cutting the insulation off of well wire when it slipped and cut his left thigh. Employee received 9 stitches.

Employee was replacing tooth on a loader, as he was using a sledge hammer a sliver of metal flew and went into employees left elbow. Employee had the sliver removed and received 3 stitches.

An employee was struck with a 12lb sledge hammer, smashing the end of his left ring finger and glancing off his left middle finger. Received stitches and is on modified duty.

Employee was hit in the mouth while tightening rim liner bolts, received small cut and cracked both front teeth.

EE PINCHED FINGER BETWEEN PIPE WRENCH AND BLOCKING.

EE WAS SWINGING A 20 LB SLEDGE HAMMER IN AN AWKWARD POSITION AND FELT LOWER BACK PAIN.

EE WAS TAKING CHAIN BINDER OFF AC SCREEN WITH 2'LONG PIPE. IT CAME LOOSE AND PIPE HIT HIM IN THEJAW CHIPPING BACK TOOTH.

EE STATES HE WAS TIGHTENING A BOLT WHEN THE RATCHET HE WAS USING SLIPPED OFF HEAD OF NUT AND PULLED BACK INTO HIS TOOTH CAUSING THE TOOTH TO CHIP & BREAK IN HALF.

EMPLOYEE STATES HE WAS LAYING ON GROUND, LOOSENING BOLT ON HAUL TRUCK WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.

STRUCK HIS LEFT THIMB WITH A HAMMER ATTEMPTING TO CLOSE A VALVE. THIS RESULTED IN A FRACTURE.

EE WAS ASSISTING IN THE REMOVEAL OF A WING SHROUD. THE SHROUD WAS HIT WITH A HAMMER TO BREAK IT LOOSE AS THIS HAPPEND A METAL FRAGMENT BROKE FROM AND STRUCK THE EE IN THE UPPER CHEST. OBJECT W AS IMBEDDED AND HAD TO BE REMOVED BY A PHYSICIAN AND THE WOUND WAS CLOSED WITH STITCHES.

WHILE USING THE J WRENCH TO BREAK THE HAMMER LOOSE, THE DRILL STEEL DRIFTED DOWN SMASHING THE EE INDEX AND MIDDLE FINGERS BETWEEN THE J WRENCH AND THE WRENCH DECK. THIS RESULTED IN SURGERY.

WHILE REPAIRING AND USING TIRE HANDLER, THE EMPLOYEE BUMPED HIS HEAD ON A SPREADER BAR. THIS RESULTED IN A LACERATION THAT REQUIRED 4 STITCHES.

WHILE EATING LUNCH THE EE DROPPED A STEAK KNIFE AND ATTEMPTED TO CATCH IT WITH HIS LEGS. IN DOING SO THE KNIFE PENITRATED THE EE'S RIGHT LEG APPROX 1" IN DEPTH. THE PUNCTURE WOUND REQUIRED 3 S TITCHES TO CLOSE.

REMOVING ON ELECTRICAL CONNECTION WHEN THE WRENCH SLIPPED AND STRUCK THE EE INT HE LOWER JAW.

LOWERING A STEEL CAUSING ONTO A TABLE WITH A WRENCH. THE WRENCH CAUGHT THE WEDDING RING. THE STEEL CASING PINCHED THE FINGER CAUSING THE SKIN TO SPLIT. STITCHES WERE REQUIRED.

EE WORKING IN REFINERY BLDG MEZZANINE MECHANICAL ROOM TIGHTENING BOLTS AND CLEANING UP AREA. HE WAS IN A TIGHT SPACE AND TURNED HEAD TO LEFT. A VALVE STEM HIT CORNER OF LEFT EYE NEAR EDGE OF S AFETY GLASSES. THIS EYE WAS PREVIOUSLY AND HAD SOME INTERIOR REPAIR DONE AT THAT TIME. CURRENT INJURY WAS NOT APPARENT AT TIME OF IMPACT.

INJURED EE WAS PRY BARRING CHIME ON CORNERWELD FOR FITTER. FITTER TOOK 4 LB HAMMER TO KNOCK SHELL PLATE IN AND AS HE SWUNG HAMMER, CAUGHT EE'S LEFT INDEX FINGER BETWEEN BAR AND HAMMER.

FOREIGN BODY, RIGHT EYE. SOCKET WRENCH SLIPPED CAUSING SPLINTER TO HIT EYE.

EMPLOYEE WAS STRIPPING A CONCRETE FORM WHEN HE PULLED ON A SUPPORT STAKE WHILE STRIKING IT WITH A HAMMER. THE HAMMER MISSED THE STAKE AND STRUCK HIS HAND.
