While using a pry bar to perform repairs on a bed of a pickup truck, the pry bar slipped and struck the employees lip resulting in an injury.

A maintenance technician was working on the latch bar mechanism on the dipper of an electric rope shovel. A small chunk of dirt fell from the dipper door striking the individual which caused the individual to lose their balance and striking EE's mouth on the end of the crowbar. The impact caused a cut lip which required stitches.

Employee was using a sledgehammer to connect poly pipe camlocks when EE felt a pain in EE's back.

While removing a dozer tooth tip with a sledge hammer, a piece of metal came off the tip and hit employee in the leg.

While trying to install a bushing into a right track frame on a Komatsu D475 track dozer a mechanic hit his thumb on his left hand with a soft faced hammer. This caused his thumb to smash against the bushing making a cut on the inside of his thumb.

A mechanic was installing a pin on a scraper hitch. The employee was using a sledge hammer to pound the pin into place behind him. When the hammer contacted the pin a piece of metal from the pin broke off and embedded into the mechanic's right leg. Employee was taken to ER but had to have surgery to remove the piece of metal from his leg.

One employee was holding a B&O tool in order to remove a wedge from a socket. Another employee hit the impact area on the B&O tool with a sledge hammer when a piece of metal chipped off of the B&O hit the employee, holding the tool, in the forehead. The employee was transported to ER where doctor removed the piece of metal.

An employee was cutting zip ties with a utility knife. He cut through the last zip tie and the knife blade contacted the palm of his left hand causing a laceration. The employee was transported to the ER where he received 2 stitches.

An employee was pounding an 8 inch pin with a sledge hammer on an 8750 dragline when a piece of metal from the hammer broke off and hit another employee standing 10 feet away. The piece of metal lacerated the left chest/shoulder area on the employee.

Employee was struck by a 4 lb hammer while helping to position the top section of a portal, resulting in a fracture to the third distal phalange of the right middle finger.

A mechanic EE experienced back pain while working on a track dozer

Emp was moving a loading shovel from one ramp to another. The shovel spun out on the slope causing the cable to tighten. He cut the rope that held the cable that was tied off to the shovel. When he cut the rope the cable whipped, knocking him to the ground. When he fell to the ground he landed on his right shoulder.

Torquing bolts on hoist drum, right hand got crushed between the hoist and the torque.

WHILE AN EMPLOYEE WAS REMOVING A MUFFLER ON A PIECE OF EQUIPMENT AND USING A PRY BAR TO SEPARATE THE INLET PIPE FROM THE MUFFLER, THE PRY BAR SLIPPED AND THE BLUNT END STRUCK THE EMPLOYEE ON T HE LEFT SIDE TEMPLE AREA OF HIS HEAD. RESULTED IN A SMALL LACERATION REQUIRING SUTURING.

EE WAS STRIPPING INSULATION FROM A PIECE OF CABLE WITH A UTILITY KNIFE WHICH SLIPPED AND STRUCK HIS LEFT THUMB RESULTING IN A LACERATION THAT REQUIRED STITCHES.

WHILE AN EMPLOYEE WAS USING A THREE HOOK SPREADER CHAIN TO LIFT A MOTOR IN PLACE, ONE OF THE HOOKS CAME LOOSE AND STRUCK THE EE IN THE LEFT EYE, RESULTING IN A BROKEN NOSE AND A SMALL LACERATI ON TO THE SCLERA PART OF THE EYE THAT REQUIRED SURGERY TO REPAIR.

WHILE CHANGING HOIST ROPES ON AN 8750 DRAGLINE BUCKET AND REMOVING A WEDGE FROM A HOIST ROPE SOCKET WITH A BATTERING RAM, EMPLOYEE'S LEFT HAND THUMB WAS PINCHED BETWEEN THE BATTERING RAM HANDL E AND AN OUTRIGGER ON A BOOM TRUCK. THIS RESULTED IN EMPLOYEE SEVERING THE TIP OF HIS THUMB ON HIS LEFT HAND, WHICH REQUIRED SURGERY.

WHILE TORQUEING ROLLER CAP BOLTS ON THE INSIDE L-H RAIL FOR A DOZER, THE REACTION ARM ON THE TORQUE ON THE WRENCH WAS AGAINST THE SIDE OF A ROLLER AND AS THE TORQUE ON THE WRENCH WAS INCREASIN G THE ROLLER TURNED CASUING THE WRENCH HANDLE TOGO AGAINST THE TRACK RAIL PINCHING THE EMPLOYEE'S L-H MIDDLE FINGER BETWEEN THE REACTION ARM AND THE TRACK RAIL.

WHILE AN EE WAS WELDING ON A DRAGLINE BUCKET RIGGING DRAG HITCH PIN AND WAS LIFTING THE PIN WITH A BAR TO REPOSITION IT, THE PIN ROLLED OFF THE TABLE AND STRUCK THE EE ON THE SHIN OF HIS RIGHT LEG. RESULTED IN FRACTURING HIS LEG.

WHILE AN EMPLOYEE WAS USING A 4 LB. HAMMER TO BEND APIECE OF METAL ON TOP OF A VISE. HE STRUCK HIS THUMB ON HIS LEFT HAND, FRACTURING THE TIP OF THIS THUMB.

AN EMPLOYEE WAS SEPARATING HAMBURGER PATTIES WITH A KITCHEN KNIFE WHEN THE KNIFE SLIPPED & STRUCK HIS LEFT THUMB CAUSING A RELATIVELY DEEP LACERATION REQUIRING SUTURING

WHILE CHANGING TOOTH ADAPTERS ON A DRAGLINE BUCKET, AN EE STRUCK THE TOOTHE ADAPTOR WITH A SLEDGE HAMMER AND A SMALL PIECE OF THE TOOTH ADAPTER BROKE OFF AND STRUCK THE EE IN HIS UPPER LEFT CH EST AREA. THE METAL CHIP EMBEDDED IN THE TISSUE IN THE CHEST AREA. AFTER A DR.'S EXAM, IT WAS DECIDED NOT TO REMOVE THE METAL CHIP.

WHILE CUTTING A PIECE OF 2 INCH RUBBER HOSE WITH A UTILITY KNIFE.THE KNIFE SLIPPED AND STRUCK HIS LEFT FOREARM.REQUIRED THREE STAPLES TO CLOSE THE LACERATION.

WHILE CHANGING CUTTING EDGES ON A CATEPILLAR BALDE,A BOLT HUNG UP CAUSING THE CUTTING EDGE TO SWING OUT AND FALL ONTO THE LITTLE TOE ON HIS LEFT FOOT,BREAKING THE TOE.

WHILE EE POUNDING A 1/4 INCH SHIM, THE HAMMER HEAD GLANCED OFF THE TOP OF THE SHIM AND STRUCK HIS INDEX FINGER ON HIS RIGHT HAND. HIS INDEX FINGER REQUIRED THREE STITCHES.

THE INJURED PERSON WAS SWINGING A 12 POUND SLEDGE HAMMER IN AN ATTEMPT TO REMOVE A PIN FROM A DRAGLINE BUCKET TOOTH ADAPTOR.

A FELLOW MINER WAS USING A CHAIN COMEALONG TO PULL A GRATE INTO PLACE THE CHAIN BROKE AND THE GRATE SWUNG STRIKING THE INJURED SUTURES WERE REQUIRED TO CLOSE THE INJURY

AFTER VULCANIZING A 23 KV CABLE I.NAME WAS ATTEMPTIGN TO DRESS UP THE EXCESS RUBBER WITH A UTILITY KNIFE WHEN HE CUT HIS LEFT INDEX FINGER SUTURING WAS REQUIRED

A PIECE OF META5 CAME OFF THE PIN OF A DRAGLINE TOOTH WHICH HE WAS ATTEMPTING TO DRIVE WITHA SLEDGE HAMMER. THE METAL CHIP WHICH WAS LODGED IN HIS STOMACH WAS REMOVED BY A PHYSICIAN. HE RETURN ED TO WORK ON H3S NEXT SCHEDULED SHIFT.

I,NAME WAS ATTE4PTING TO REMOVE A U-JOINT FROM THE DRIVE SHAFT OF A PICKUP TRUCK BY HITTING THE U-JOINT WITH A HAMMER. A CHIP OF METAL CAME OFF THE U-JOINT AND LODGED IN HIS THUMB. A PHYSICIAN WAS REQUIRED T2 REMOVE THE CHIP.

WHILE REWIRING 5N ELECTRICAL PANEL ON AN EASI-MINER, AN ELECTRICIAN CUT HIS LEFT INDEX FINGER. THE CUT WHICH REQUIRED SIX STITCHES WAS INFLICTED BY A POCKET KNIFE WHILE ATTEMTING TO STRIP ELEC TRICAL WIRE. 3

A MECHANIC WAS 3NSTALLING A BEARING IN A RUBBER TIRE DOZER USING A HAMMER AND DRIFT PUNCH WHILE TAPPING ON THE BEARING WITH THE P UNCH AND HAMMER A PIECE OF THE BEARING CHIPPED O OFF AND LODGED 3N THE INDEX FINGER OF RIGHT HAND OF MECHANIC THE STEEL WAS REMOVED AND 5 SUTURES WERE ADMINISTERED
