Mostrando los 40A maintenance technician was working on the latch bar mechanism on the dipper of an electric rope shovel. A small chunk of dirt fell from the dipper door striking the individual which caused the individual to lose their balance and striking EE's mouth on the end of the crowbar. The impact caused a cut lip which required stitches.
Employee was using a sledgehammer to connect poly pipe camlocks when EE felt a pain in EE's back.

Emp was moving a loading shovel from one ramp to another. The shovel spun out on the slope causing the cable to tighten. He cut the rope that held the cable that was tied off to the shovel. When he cut the rope the cable whipped, knocking him to the ground. When he fell to the ground he landed on his right shoulder.
Torquing bolts on hoist drum, right hand got crushed between the hoist and the torque.
WHILE AN EMPLOYEE WAS REMOVING A MUFFLER ON A PIECE OF EQUIPMENT AND USING A PRY BAR TO SEPARATE THE INLET PIPE FROM THE MUFFLER, THE PRY BAR SLIPPED AND THE BLUNT END STRUCK THE EMPLOYEE ON T HE LEFT SIDE TEMPLE AREA OF HIS HEAD. RESULTED IN A SMALL LACERATION REQUIRING SUTURING.

WHILE AN EMPLOYEE WAS USING A THREE HOOK SPREADER CHAIN TO LIFT A MOTOR IN PLACE, ONE OF THE HOOKS CAME LOOSE AND STRUCK THE EE IN THE LEFT EYE, RESULTING IN A BROKEN NOSE AND A SMALL LACERATI ON TO THE SCLERA PART OF THE EYE THAT REQUIRED SURGERY TO REPAIR.
WHILE CHANGING HOIST ROPES ON AN 8750 DRAGLINE BUCKET AND REMOVING A WEDGE FROM A HOIST ROPE SOCKET WITH A BATTERING RAM, EMPLOYEE'S LEFT HAND THUMB WAS PINCHED BETWEEN THE BATTERING RAM HANDL E AND AN OUTRIGGER ON A BOOM TRUCK. THIS RESULTED IN EMPLOYEE SEVERING THE TIP OF HIS THUMB ON HIS LEFT HAND, WHICH REQUIRED SURGERY.
WHILE TORQUEING ROLLER CAP BOLTS ON THE INSIDE L-H RAIL FOR A DOZER, THE REACTION ARM ON THE TORQUE ON THE WRENCH WAS AGAINST THE SIDE OF A ROLLER AND AS THE TORQUE ON THE WRENCH WAS INCREASIN G THE ROLLER TURNED CASUING THE WRENCH HANDLE TOGO AGAINST THE TRACK RAIL PINCHING THE EMPLOYEE'S L-H MIDDLE FINGER BETWEEN THE REACTION ARM AND THE TRACK RAIL.
WHILE AN EE WAS WELDING ON A DRAGLINE BUCKET RIGGING DRAG HITCH PIN AND WAS LIFTING THE PIN WITH A BAR TO REPOSITION IT, THE PIN ROLLED OFF THE TABLE AND STRUCK THE EE ON THE SHIN OF HIS RIGHT LEG. RESULTED IN FRACTURING HIS LEG.
WHILE AN EMPLOYEE WAS USING A 4 LB. HAMMER TO BEND APIECE OF METAL ON TOP OF A VISE. HE STRUCK HIS THUMB ON HIS LEFT HAND, FRACTURING THE TIP OF THIS THUMB.
AN EMPLOYEE WAS SEPARATING HAMBURGER PATTIES WITH A KITCHEN KNIFE WHEN THE KNIFE SLIPPED & STRUCK HIS LEFT THUMB CAUSING A RELATIVELY DEEP LACERATION REQUIRING SUTURING

WHILE CUTTING A PIECE OF 2 INCH RUBBER HOSE WITH A UTILITY KNIFE.THE KNIFE SLIPPED AND STRUCK HIS LEFT FOREARM.REQUIRED THREE STAPLES TO CLOSE THE LACERATION.
