The injured worker, was sharpening a chisel when a sliver of metal from the chisel became dislodged, sailed over the top of his safety glasses and embedded in his right eye

The mechanic was hammering a hard bar pin into a D10 Dozer and a piece of the pin broke off and became embedded in his left thigh

Employee was prying a bolt out of an A/C Compressor on a D10N employee back went out causing him to fall to the ground.

Employee was prying on a block of coal in the hopper, employees hand slipped off of the pry bar hitting the corner of the hopper resulting in lacerating his right finger, where he received four stitches.

Employee was prying on bar it slipped hitting the employee in the left eye lid resulting in him getting four (4) sticthes in his left eye lid.

Employee was hitting the side of powered bin with a sledge hammer. The hammer came back and struck the employee in the right rib cage causing him to black out hitting his head on the beam of the powered bin.

Employee was removing a hydrolic hose and the wrench slipped off the hose fitting causing the wrench to his the employee in the forehead, which caused a laceration to his forehead.

Employee was changing a tire on IT28 loader and the jack fell causing the tire to hit the employee in the left lower leg.

Equipment being used broke & came apart at the connecting joint throwing part of said equipment into hand of employee.

EMPLOYEE WAS HAMMERING PINS INTO TEETH THAT WENT ON LOADER BUCKET. A PIECE OF METAL PIN WENT INTO HIS LEFT THIGH.

WHILE REMOVING ENGINE FROM 992C #M96 EMPLOYEE'S WRENCH SLIPPED CAUSING HIM TO FALL TO GROUND, NO APPARENT INJURY, FEW DAYS LATER EMPLOYEE NOTICED NUMBNESS IN BOTH HANDS, BEGAN GETTING WORSE MISSED WORK 2-14 TO 2-19 RETURNED 2-20. DR TO SEND TO SPECIALIST.
