While using a hammer and chisel to knock out bolts after being torched, IE received a sliver of metal in the web of EE's hand between the thumb & index finger. Cause of this injury was awareness of this employee. Compliance with company policies & PPE may be a factor along with miner proficiency. Regulations/mining equip/systems, not a factor.

The employee was attempting to tighten bolts on the tailgate drive when ee claims to have felt pain in lower back. The following were not a factor: Rules, Regs, Skills, Proficiency, Training, Equipment, Protective Devices and Clothing. Attitude may have been a factor.

The employee was cutting an old drive belt with a utility knife when EE slipped and cut EE's thumb. The following were not a factor: Rules, Regs, Skills, Protective Devices, Training, Equipment and Clothing. Proficiency and Attitude may have been a factor.

The employee was releasing the cable on a come-a-long when his hand was stuck by the release latch causing a contusion. The following were not a factor: rules, regulations, mining equipment or systems, protective devises on equipment or clothing, skills, proficiency, or training. Attitude may have been a factor. Miner did not start to lose time until 8-24-2015.

The miner was using a jack to place a rail car back on track. The miner sprained his wrist during this process. The following were not a factor: rules, regulations, mining systems, job skills, proficiency, training attitude, clothing, or protective devices.

The miner was attempting to pry-up on a track rail with a stone bar. When the employee applied up-pressure he strained his back. The following were not a factor: rules, regulations, skills, equipment, clothing, or protective devices. Attitude may have been a factor.

The miner was performing a cable splice on a shuttle car cable. The employee was cutting away from himself when his finger slipped down toward the blade the tool. The following were not a factor: Rules, Regs, Skills, proficiency, Protective devises, equipment or clothing. Attitude may have been a factor. it was at the end of the shift and he may have been in a hurry.

A foreman was using a slate bar to remove a stone from a scalping screen when he struck his right ring finger on an I-beam, causing a laceration.

Employee was changing a filter on a water filter system. As he removed the holding bracket for the filter he felt pain in his shoulder.

Employee was using a bar to move a track rail with another employee. He felt pain lower back.

Employee was cutting open rockdust bags with a knife and cut through his gloves. 3 stitches on finger.

Employee was putting the rockdusting machine back on track. As he raised the rockduster with a track jack he felt pain in his groin.

Employee was cutting a section of hose with a razor knife and cut his thumb. He got 5 stitches on his thumb.

Employee was working on a continuous miner. He was using a ratchet in a confined space to loosen a bolt (load lock). The bolt let loose under full force causing him to smash his thumb.

Employee was using a belt bed rail to pry up on a roll of belt and felt pain in his back.

Employee was moving a pile of rock and was on his knees. The rock pile slide and struck him on the right wrist. The wrist is fractured.

EE was knocking a hole through a wall. The tool went through the hole smashing his finger. 7 stitches.

Employee was watching an employee tighten a bean with a pick hammer. Hammer slipped out of his hand and struck the foreman

Employee at the prep plant was tightening a bolt on a CMI when his hand slipped - puncturing his hand

EE cutting a rockdust hose wiatha a razor knife. Knife slipped cutting his thumb. 2 stitches.

EE was building 24" cuttable cribs on the LW recovery chute. He was breaking bundle of wedges with a hammer (pick hammer) and struck his hand.

Employee was installing a door in a wall. He hit his left middle finger between a wall and sledge hammer handle. 2 stitches

EE was cutting a plastic band and cut his palm of his left hand.

EE part of the track crew. They were laying track rail (#60) when the EE was prying rails apart the pry bar slipped.

Employee was prying on a sheave wheel on a longwall dolly. The bar slipped striking him above the rt eye brow.

The employee was constructing a stopping when his injury occured. He was striking a concrete block with a hammer when he swung and struck his left thumb. He fractured his left thumb. He did not begin loosing time until 11/23/09.

The EE was striking a wooden post with a track jack to remove it from holding a tailpiece in place. The jack slipped and the EE pinched his finger between the jack and the post.

Employee was using a knife at No. 1 Portal supply yard whereupon he lacerated his left thumb while using a razor knife while cutting mine belt.

The EE was hooking up a longwall shield, when the chain he was using, slipped out of his right hand and struck his left index finger. The EE has a fractured finger.

Employee was using hand held come along to raise belt structure, when come along slipped lacerating EE's left hand. EE received 17 stitches.

EE was cutting open a box in the supply house. In the process of cutting, the razor knife slipped lacerating the EE's left hand. (EE received 4 stitches).

Employee was working with the track crew installing 85 lb. rails on the main south track. Track rail was jacked up with a lifting jack so the crossties could be installed. Employee was pushing a crosstie under the rail when the rail fell off the track and pinched his finger between the track rail and the crosstie.

Employee was insulating a damaged place in the trailing cable of #12 shuttle car on the New South Mains continuous miner section (025-0). He was using a knife to cut off the tapes. He cut the top of his left ring finger for 1 inch. The knife was a razor knife. Five sutures were required to close the cut.

The employee was building a ventilation stopping when his incident occurred. The employee swung an axe to split a concrete block and struck his left thumb. He broke his thumb in two places. No lost days.

Employee was performing maintenance work on a continuous miner. He was changing out a cutter motor. He was using a stone bar to pry the motor into place. The bar slipped out and he fell backwards. He tried to catch himself with his left arm. He sprained his left arm, wrist and shoulder. Last day worked was 6/20/08.

Baring a track over to get lined up, underneath a belt/track winder chute tail track, as he was prying up, felt pain in right shoulder.

Raising track rail with a lift bar so that a track tie could be slid under the rail, he felt pain in his chest.

Throwing track switch at 6 West, bar slipped and pulled hip and upper leg.

While putting fish plates together on track rail, employee who was holding fish plate had hand slip and employee lifting fish plate struck his left middle finger with hammer.

Injured was in the process of removing a mechanical splice from the belt system using a utility knife to cut the belt. The knife blade slipped off of the conveyor belt and struck him in the upper right leg. He suffered laceration that required four stitches.

Using bar to move belt stand, felt pain in lower back. Out by long wall face.

Employee was sawing a post on tap drive of main north belt. Hit right elbow on belt structure.
