Employee was laying on EE's back in an awkward position using a pry bar trying to pry a pin out of the bore on revolving frame on the 4600. Doing so EE strained EE's back.

Employee's knife slipped while cutting a piece of rubber. Knife cut left pinky finger requiring 2 stitches.

Removing a guard on the push arm of a 375A bulldozer to make a minor repair using a wrench. Wrench slipped and struck employee on right shin causing a laceration.

EE was using a utility knife to cut bucking rubber. As EE was shaving the bucking rubber which was on a steel sheet, the sheet moved and began to fall over. As EE went to grab the steel EE unintentionally cut hand with the utility knife. EE received 5 stitches on the inside of palm near the thumb.

HELPING CHANGE THE HOIST ROPE ON THE 7450 DRAGLINE. WAS REMOVING THE WEDGE FROM THE HOIST CABLE SOCKET USING A RUBBER SLEDGE HAMMER WHEN EMPLOYEE INJURED ARM WHILE SWINGING.

Employee was cutting a piece of channel rubber to length with a utility knife. EE was kneeling on ground and cutting rubber on a pallet in front of them. They cut the rubber towards themselves, and with such force, that after making the cut, the knife continued on and sliced thru their coveralls and work pants. EE sustained a laceration on the right thigh - 7 stitches.

Cut finger while using a knife to cut a piece of belt.

Employee was using a sledge hammer to hit a tooth on the 992 bucket to remove the tooth. A small piece of metal broke off the tooth & hit employee in the thigh.

Tightening bolt on side seal area of bagging machine when hand slipped hitting the corner of a piece of metal on the machine.

Employee was removing bearing race with hammer and chisel. Piece of metal came off race, hitting him in the left eye. Employee was wearing safety glasses

Employee was stripping a wire for the turn signal on truck #83 at the truck maintenance garage. He was using a razor blade. The blade slipped and cut a finger on his left hand.

Beltrami was tightening a spring break on #85 tractor with a wrench and pulled his right shoulder.

Climbed on top of 777 truck to put tow cables on in order to pull truck. This resulted in pulling or pinching testicles.

Employee was installing a bearing on a 4600 dragline. We used a chisel to wack it on, shattering the end of the chisel and resulting in a 5/16 inch piece of steel lodging into his stomach (right lower abdomen)

Working under 773B water truck. Bent over working on a hydraulic hose when he felt a pop in his back.

Was under loader and got dirt/metal in eyes.

EE was using a pry bar to put pin in track of Demag excavator, when another person was hitting the pin with a sledgehammer and a piece of pin broke off and hit employee in the upper lip. Note: EE did not seek treatment until 1/17/2013.

Employee was removing cylinder pin on Demag bucket using a metal bar. When pin was pushed through, cylinder slid against bar, causing hand to be pinched between bar & bucket.

Hit in head with hammer

Heating and bending round stock with sledge hammer. Woke up next morning with what he thought was tendonitis. Went for x-ray on 5/31/11 and found out hand was fractured.

EE was using a ratchet and pipe to loosen bolt. Ratchet broke and hit his finger between the pipe and ground.

Holding wrench for mechanic, wrench slipped and smashed finger.

As another mechanic was using a hammer to drive a steel pin into a house roller of a dragline shovel, a piece of metal slag broke loose while hammering and struck EE in the corner of his right eye, while standing at least 10' away.

WRENCH SLIPPED AND LEFT HAND HIT OBJECT AND BRUISED. INITIALLY IT WAS THOUGHT THAT THERE WAS A FRACTURE OF THE FINGER. TEST AND X RAYS WERE DONE. SEVERELY BRUISED.

EE WAS HAMMERING ON A CHISEL. THE OBJECT SHIFTED& RELEASED THE CHISEL UNDER PRESSURE THE CHISEL CAME UP & HIT EE IN THE HEAD 1" ABOVE LEFT EYEBROW

CUTTING CHAIN LINK FENCE WITH BOLT CUTTERS EE HAD SHARP PAIN IN HIS BACK AND DROPPED TO KNEES.

EE REPORTED THAT RATCHET SLIPPED OFF A DRAIN PLUG HE WAS REMOVING FROM A D-10 DOZER WHILE CHANGING THE ENGINE OIL CAUSING HIM TO STRIKE HIS FINGER ON A STATIONERY OBJECT

EMPLOYEE WAS MOVING A METAL PLATE WITH A BAR. WHEN THE BAR SLIPPED THE EMPLOYEE FELL OFF THE SURFACE HE WAS WORKING ON AND ONTO THE GROUND APPROX 3' IN HEIGHT. WHEN EMPLOYEE FELL HE HIT HIS HE AD AND SHOULDER ON A DRAGLINE BUCKET THAT WAS POSITIONED NEXT TO THE BUCKET HE WAS WORKING ON.

EE WAS HAMMERING A PIN ON A DOZER AND A PIECE OF STEEL HIT HIM IN THE LEFT EYE.

EE WAS HOLDING A BAR WHILE ANOTHER EE WAS SWINGING A SLEDGEHAMMER. THE SLEDGEHAMMER SLIPPED AND HIT THE RIGHT SIDE OF THE EE'S HEAD. IT RESULTED IN A LUMP.

WHILE TRYING TO PULL A HUB FROM AN AXLE ON TRACTOR-TRAILER USING A WHEEL PULLER - WHILE PULLING THE HUB THE WHEEL PULLER JUMPED OFF THE HUB & HIT EE ON LEFT SIDE OF FOREHEAD.

SHOVELING MATERIAL, EE SLIPPED AND TWISTED BACK.

EMPLOYEE WAS REMOVING 6" VALVE FROM PIPELINE. WHEN REMOVING LAST BOLT VALVE FELL FREE, HIT A PRYBAR CAUSING IT TO BECOME LEVERED. PRYBAR LAUNCHED ITSELF AND HIT EMPLOYEE IN THE ABDOMEN NEAR TH E NAVAL AREA.

THE EMPLOYEE WAS PULLING ON A 3/4" BREAKER BAR WITH SOCKET WRENCH IN AN ATTEMPT TO LOOSEN A 1 1/2" NUT ON THE DRUM ASSEMBLY OF THE 7450 SHOVEL. AS HE WAS PERFORMING THIS ACTIVITY, HE FELT A SE VERE PAIN IN THE ENTIRE LOWER BACK AREA.

EMPLOYEE WAS STANDING, WATCHING ANOTHER EMPLOYEE TIGHTENS A NUT WITH A WRENCH. THE ACITIVITY WAS TAKING PLACE AT EYE LEVEL. THE WRENCH SLIPPED OFF THE NUT AND STRUCK THE SPECTATIVE EMPLOYEE I NT HE RIGHT LENSE OF HIS GLASSES. THE LENSE SHATTERED IN TEH CENTER, CAUSING A PIECE OF A GLASS TO ENTER HIS EYE.

EMPL TWISTED BACK WHILE STEMMING BLAST HOLES WITH DIRT. EMPL DID NOT SEEK MEDICAL ATTENTION UNTIL 2/7/91 AT WHICH TIME HE WAS TOLD NOT TO WORK.
