Employee was removing a bolt from a filter housing using an 18 inch extension on a breaker-bar. As torque was applied to the breaker-bar the extension broke lacerating his palm against the broken extension. The laceration required two stitches to close.

EE, a section mechanic was making a splice on a miner cable and as he went to lower the cable his left hand came in contact with a razor knife which caused a laceration that required stitches.

EE was helping replace a section of ventilation tubing. As he was holding up the tube another miner was attempting to attach the tube hanger to the mines roof mesh using a bar. The bar slipped off the tube hanger and struck EE across the nose fracturing his nose. the fist day of work missed due to this accident was 2-27-2012.

Employee stated that on 6-23-11 he had struck the back of a mason hammer with a claw hammer and received a laceration to his thumb. In November, several months later he started to have problems with his thumb. On 11-04-11 he was examined and a piece of metal was embedded in his thumb. He believes this was from the incident on 6-23-11

Employee is a bathhouse attendant and was repairing a wash down hose by putting on a screw clamp. He was holding the hose and clamp with left hand and tightening the screw with his right hand. The screwdriver slipped of the screw and punctured his left hand which required stitches to close wound.

Employee was swinging an axe to knock a hole in a stopping. As the axe went past his belt his hand hit a razor knife attached to his belt which had the blade out. This caused a laceration to his hand that required stitches. Note: Mine was idle due to vacation shut down from 7-26-2010 through 8-9-2010 to reflect reporting time frame difference.

Employee was using a porta power jack to straighten out a section of bent metal when the jack became dislodged and struck him in the face causing a laceration and fractures to his face.

Employee was attempting to remove a damaged wheel bearing with a hammer and a chisel when a piece of metal from the bearing race chipped off and struck him in the face and become imbedded in his left cheek.

Employee was replacing a damaged track on a ASV loader. As he was attempting to pry the track into place with a bar, the bar slipped and caused the employee to strain his abdomen which herniated.

EE was working with another employee installing a belt take-up. The second employee was setting a wedge in place using a hammer and as he swung the hammer to strike the wedge, he missed the wedge and the hammer slipped from his hand and struck EE in the lip, causing a laceration to his lip.

Employee was cutting a piece of rubber to make a seal when the knife he was using slipped and the the back of his left middle finger causing a laceration which required stitches.

Employee was attempting to loosen a bolt with a ratchet when the socket slipped off and struck the employee in the chin knocking a tooth out.

Miner was using a chuck wrench to knock a hole in a stopping. As he swung the wrench, it hit a cable and deflected the wrench and it hit his hand, causing a fracture to08 his finger.

Employee was attempting to loosen a bolt on a belt pre-cleaner with a socket and ratchet. The socket slipped off the bolt and the ratchet struck him in the mouth causing a laceration to his lip.

Employee was in the process of setting rock props and as he was handed the setting hammer for setting the props he felt something pop in his shoulder. the time differaece between the accident date and the date submitted is because the employee did not loose any time until May 12 2006 from this incident

Employee was removing bolts from a pre-cleaner on a belt when the wrench he was using slipped of the bolt and struck him in the mouth.

THE EE WAS POUNDING ON A CHAIN TRYING TO SEPARATE A LINK WHEN A CHUNK METAL FLEW OFF THE EDGE OFA LINK AND STRUCK THE EE.

EE WAS HELPING CUT DOWN WIRE MESH FROM THE MINE ROOF WHEN A SECTION OF MESH STRUCK HIM IN THE FACE.

EE WAS CLEANING A BELT CUTTER & RAN THE EDGE OF HIS FINGERS ACROSS THE BLADE

EMPLOYEE WAS ATTEMPTING TO REMOVE AN OLD BOLT FROM A SECTION OF BELT STRUCTURE, WHEN HE STRUCK THE BOLT THE AXE GLANCED OFF THE BOLT AND HIT HIS ANKLE.

EE WAS CUTTING A PIECE OF MINE BELT WITH A RAZOR KNIFE, THE KNIFE SLIPPED OVER A STRAIGHT EDGE STRIKING HIS THUMB.

EE WAS CUTTING A SECTION OF TAPE OFF OF A MOTOR LEAD, SLIPPED WITH THE KNIFE AND CUT HIMSELF.

A PIECE OF ROCK WAS CAUGHT IN THE CONVEYOR OF A SHUTTLE CAR AND EE GOT A BAR AND WAS TRYING TO PRY IT LOOSE WHEN THE BAR HE WAS USING SLIPPED AND HE SMASHED HIS FINGERS BETWEEN THE PIECE OF RO CK AND THE BAR HE WAS USING.

EMPLOYEE WAS ATTEMPTING TO CUT A PIECE OF CABLE AND STRUCK HIMSELF IN THE HAND WITH A HAMMER.

EE WAS HOLDING A METAL STRAIGHT EDGE ON A SECTION OF BELT WHILE ANOTHER EE WAS CUTTING THE BELT. THE RAZOR KNIFE USED TO CUT THE BELT SLIPPED AND STRUCK THE INJURED EE.

EE WAS LIFTING THE BELT HARDWARE WITH SOME COMALONGS, HE LOOKED UP AND GOT SOMETHING IN HIS EYE.

EE WAS CUTTING A NYLON TIE TO PUT ON VENTILATION TUBING WHEN HE SLIPPED AND CUT HIS LEFT THUMB WITH A KNIFE. THE CUT REQUIRED 4 STITCHES.

EE WAS SPLICING A FEEDER BREAKER CABLE WHEN HE WAS STRIPPING INSULATION OFF OF A LEAD AND HIS RAZOR KNIFE SLIPPED CUTTING HIS LEFT HAND BETWEEN THE THUMB AND INDEX FINGER.

WHILE USING A SMALL HAND WINCH, THE WINCH HUNG UP. HIS PARTNER PULLED ON THE CABLE AND THE WINCH CAME OUT OF ITS ANCHORED POSITION AND CAUGHT THE EE'S LITTLE FINGER BETWEEN THE WINCH AND PANLI NE.

EE WAS CUTTING A SPLICE IN A CABLE WHEN THE KNIFE SLIPPED AND CUT HIS FINGER.

EE WAS INSTALLING A PUMP COUPLER USING A CHISEL.AS HE STRUCK THE CHISEL HE MISSED & HIT HS THUMBWITH A HAMMER

USING A SLEDGE HAMMER TO BREAK UP A PIECE OF COAL WHILE SWINGING THE HAMMER HE FELT A TWINGE IN HIS GROIN AREA. LATER DIAGNOSIS SHOULD A HERNIA REQUIRING SURGERY. FIRST DAY MISSED TO DO ACCIED ENT WAS 3/9/98

EE WAS CUTTING A 48" CONVEYOR BELT WITH A RAZOR KNIFE, WHEN IT PULLED THROUGH CUTTING HIS RIGHT LEG ABOVE THE KNEE.

MOVING A JACK IN THE RAM CAR AND SMASHED HIS 5 FINGER ON HIS LEFT HAND.

PULLING ON THE LONGWALL MONORAIL WITH A CHAIN. THE CHAIN CAME LOOSE & HE FELL BACKWARD LANDING ON HIS BACK. NOTE: CONTINUED TO WORK UNTIL 1/15/98 AT WHICH TIME HE WENT OFF WORK FOR OPERATION O N NECK. ACCIDENT BECAME MSHA REPORTABLE ON 1/15/98.

TRYING OT TAKE A CRESENT WRENCH APART. SLIPPED AND STRUCK AN ELECTRICAL TYPE SCREWDRIVER IN HIS HAND.

PULLING ON A TAIL ROLLER WITH A CHAIN BLOCK WHEN CHAIN BLOCK SLIPPED OFF AND HIT HIM ON SHOULDER AND NECK. THIS REPORT IS BEING COMPLETE TO TAKE A VIOLATION.

CUTTING OUTER CABLE INSULATION WITH UTILITY KNIFE ON 4/0 CABLE. KNIFE SLIPPED CUTTING LEFT WRIST

WAS JACKING UP BELT WITH A HANDY MAN JACK WHEN HANDLE FELW BACK & STRUCK HIS THUMB.

EMP WAS LEVELING BELT STRUCTURE USING A COME A LONG. COME A LONG SLIPPED PINCHING RIGHT INDEX FINGER BETWEEN COME A LONG AND BELT STRUCTURE CHAIN CAUSING LACERATION TO RIGHT INDEX FINGER.

TIGHTENING BOLT WITH TORQUE WRENCH WHEN THE WRENCH SLIPPED OFF OF BOLT AND HIS FINGER WAS PINCHED BETWEEN WRENCH AND METAL FRAME.

EE WAS CUTTING A LOOSE PIECE OF BELTING FROM THE LONGWALL FEET. THE KNIFE HE WAS USING SLIPPED AND HE CUT HIS FINGER WHICH REQUIRED STITCHES.

INJURED SUFFERED A LACERATION TO THE END OF HIS LEFT LITTLE FINGER WHEN A CAT PAD PIN PINCHED IT AGAINST THE MACHINE FRAME. HE WAS HOLDING HIS FINGERS BETWEEN THE CAT PAD AND FRAME TO CATCH TH E PIN AS ANOTHER WORKER STRUCK THE PIN, KNOCKING IT THROUGH THE PAD. REQUIRED STITCHES.

KNOCKING A HOLE IN A DROP INLET WITH SLUDGE HAMMER. HIT MIDDLE FINGER, RIGHT HAND.

HOLDING PIPE WITH WRENCH. WRENCH FLIPPED EMPLOYEE IN THE MOUTH CHIPPING HIS TEETH.

EMPLOYEE WAS MOVING BELT TAIL PIECE OVER USING A JACK. JACK SLIPPED AND FLIPPED UP STRIKING EMPLOYEE IN CHIN CAUSING A CUT THAT REQUIRED STITCHES.

REMOVING TUBING IN THE 2ND E MINER SECTION. EMP WAS USING A STEEL BAR TO HELP REMOVE THE TUBING.HE GOT HIS FINGER CAUGHT BETWEEN THE BAR & SHUTTLE CAR FRAME WHICH CAUSED A CUT TO FINGER.

BARRING DOWN SOME LOOSE ROOF AND PIECE OF METAL MESH STRUCK ARM CAUSING LACERATION REQUIRING STITICHES.

STRAINED HIS BACK PULLING ON A RATCHET. COMPANY NOTIFIED OF ACCIDENT ON 6-10-94.
