Employee was working on continuous miner head when the tool he was using broke striking him in the mouth resulting in a cut requiring 7 stitches.

EE was driving a pin in the 16G grader, when he hit the pin with the sledge hammer a piece of the pin chipped off and went into his right upper arm.We took him to the doctor and the doctor told him to leave it in the arm no treatment needed.Then he went to a family Doctor he took it out 5-22-12 had 3 stitches He hasn't missed any work.

Employee was taking cover off ram car in #6 entry when ratchet slipped and then caught, employee then felt sharp pain in left shoulder, he work till 5/1/12 then his doctor told him he was going to need shoulder surgical repair outpatient.

He was changing out drive shaft on vessel and was going back in with new shaft while still attached to chain hoist he stopped hoist to rest, placed his hand on shaft, weight shifted causing shaft to move and pinch his finger in between chain, shaft and vessel.

Employee was struck in neck with a chip from pin as the track pin was being driven in with a hammer. Bleeding was stopped in less than 1 minute. He was transported to hospital for further exam. He was airlifted to U.K. for precautionary examination.

Employee, was using a small pry bar to work on a piece of equipment. The bar slipped and struck him on his face near his mouth resulting in a laceration that required stitches.

Employee was holding a pipe wrench attempting to loosen a pipe fitting when a fellow employee struck the handle of the wrench with a hammer. The hammer struck the employee on his small finger on his left hand resulting in a fracture.

Employee, was removing the insulation on an electrical cable using a utility knife in preparation to make a cable splice. During the process he accidentally cut his left hand. The laceration required 10 stitches to close.

EE was working in the warehouse. He stated he was placing a large torque wrench (61 lbs) on a bottom storage shelf. He stated that as he was lifting the wrench, the device twisted and he felt a sharp pain in his right forearm.

A CREW MEMBER WAS USING A RAILROAD JACK TO ALIGN A DRIVE UNIT. THE JACK HANDLE BECAME STUCK. WHEN THE HANDLE WAS FREED UP IT WAS EJECTED FROM THE JACK HANDLE SLEEVE. THE HANDLE TRAVELED ABOUT 5 FEET AND STRUCK THE INJURED EMPLOYEE ABOVE THE RIGHT EYE CAUSING A LACERATION.

Employee hit a drill chuck with a wrench to insert it into the drill pot. A piece of metal came off the chuck and entered his lower right leg. He was treated in the E.R. and returned for the next two shifts of work. On Aug 3rd he returned to hospital where a Dr located and removed the piece of metal but in process damaged an artery.
