A roof fall occurred on the Joe Day mains in the #6 entry between cross cut 33x34. The fall length was 50ft by 19 ft wide and 5ft thick.

A roof fall was found at CC 28 in the #2 entry on the north mains. The fall was 24 long by 20 wide by 8 feet high. The roof had a slip above bolt anchorage. The area had been bolted with 5 feet resin bolts and supplemented with 10 foot cable bolts.

He was installing a roof bolt when a piece of rock approximately 22'L X 14'W X 2' thick fell striking him on the top and side of the right foot.

A roof fall was found in the #5 entry at crosscut #40 which is the mines primary escapeway. The fall was estimated to be 18 feet by 40 feet by 10 feet in diameter.

A roof fall was discovered in the Northwest Mains near Crosscut # 84. The roof fall measured 4 feet by 10 feet by 60 feet. There were no injuries and the ventilation was not interrupted.

After the accident investigation was completed it was determined that the knee surgery was in fact due to the injury reported on June 10th.

IEE was at #2 drive with another EE when he tripped and fell hit his knee on a piece of metal we went to Norton Hospital and got two stitches.

EE was walking to #32 seal got his foot hung on a piece of rock pull his back Doctor took him off work 7/16/12 he work up till this time.

EE was driving a pin in the 16G grader, when he hit the pin with the sledge hammer a piece of the pin chipped off and went into his right upper arm.We took him to the doctor and the doctor told him to leave it in the arm no treatment needed.Then he went to a family Doctor he took it out 5-22-12 had 3 stitches He hasn't missed any work.

Employee was taking cover off ram car in #6 entry when ratchet slipped and then caught, employee then felt sharp pain in left shoulder, he work till 5/1/12 then his doctor told him he was going to need shoulder surgical repair outpatient.

The fall was approximately 7' high by 20' wide by 40' long

EE was lifting a 10HP cooling system pump motor in the BPM room when he felt something pull in his lower back we took him to the hospital Dr. said he was all right could go back to work. Started having more back Doctor took him off work on 2/7/12 did not meet the requirements to report till 2/7/12

He was changing out drive shaft on vessel and was going back in with new shaft while still attached to chain hoist he stopped hoist to rest, placed his hand on shaft, weight shifted causing shaft to move and pinch his finger in between chain, shaft and vessel.

Employee was removing a cyclone to change out. When he started lifting the cyclone, it suddenly turned and pinched his hand against a brace in cyclone tub and mashed his fingers/ hand.

Was riffling a coal sample and was turning his body while carrying a the sample, weighing approximately 70 pounds, and his left knee gave out on him. There was swelling and he could not put a lot of pressure on his leg.

Roof fall was found at 9:55am inby spad #750 in the #1 entry MSHA was called at 10:02am it has been cribbed off and dangered off.

Employee was adding steel to finish his drill hole when a rock fell from the top striking him on the right middle finger

Employee was moving a battery powered personnel carrier when the vehicle struck the coal rib causing the employee to strike his chin against the machine. This resulted in a laceration that required 6 stitches.

The mine suffered a gas or dust ignition in the # 4 heading, there were no injuries. The continuous miner was removed from the face and the area was re-ventilated.

Employee slipped twisting his knee, he continued to work until 05/02/2011 at which time he began loosing time.

Employee was lifting a miner cable when he strained his back. The employee continued to work until May 23, 2011 at which time he was removed from work by his physician. This accident has been under investigation and has now been determined (June 17, 2011)to be work related and reportable.
