Changing conveyor pot on cont. miner, using slate bars to remove CLA disc from pot. Laid a slate bar on the ground after removal of disc. Disc slid off conveyor pan catching end of bar causing it to flip up and strike employee in cheek causing cut requiring 8 stitches.
Employee was tightening bolts on fan motor. Base moved and motor fell on left hand. Fractured left middle finger, cut ring finger, middle finger and pinky finger. Employee had to have stitches in all three fingers.
A GRIP HOIST WAS BEING USED TO PULL BROKEN BELT TOGETHER WHEN THE ACCIDENT OCCURED. EE WAS STRUCK BY THE HANDLE EXTENSION OF THE HOIST WHEN THE HANDLE WAS RELEASED. INJURIES OCCURRED TO THE LE FT CHEEK, CHEEK BONE, AND NOSE OF EE.
