EE was in the process of taking the wheel unit bolts out of a shuttle car he was using a impact wrench and 3/4" Allen plug to loosen the bolts someone had placed a nail were the dow pin went to hold the plug in the socket and he started the impact wrench and it caught his glove on his right hand and caused a laceration requiring 4 sutures to close the wound.

Mostrando los 26EE was placing a wrecked flatcar back on track and had hooked a chain ratchet into the eye of a roof bolt plate and the eye broke out allowing the chain ratchet to fall and hit EE in the head causing a mild concussion. He had asked off prior to the accident for a funeral on 1-28-11 and he wasn't scheduled on 1-29-11 He returned on 1-31-21.
EE wrecked the mantrip at bk-50 on Rh 1 track he was using a 10 ton railroad jack when the bar struck him on left side of chin at his jawline he lost consciousness for a few minutes. He was taken to hospital and x-rays were negative and he was released back to work on his next shift he did return to his normal job.
Hitting insert in the continuous miner with a hammer piece of metal went into index finger. Had to be removed by a doctor embedded object. The piece of metal was removed and he returned back on his next shift without restrictions.
EE was cutting the plastic wrapping off of a bearing when he cut his right hand with a utility knife requiring 4 stitches to close the wound.
EE was using slate bar to hold rail over, he had placed his hand under the rail tie to hold it up against the track. The slate bar slipped allowing the rail to move which caused a laceration to his finger requiring stitches.
Employee was cutting the bands on a pallet of 48" top belt structure. When he cut the last metal band six pieces of structure fell over hitting his right foot behind the metatarsal guard on the leather boot. This caused fractures to the bones in the top of his foot.
Member was cutting the metal strap on a bundle of roof bolt plates. When he cut the strap, it contacted his right forearm causing a laceration requiring stitches.

HE WAS USING A 2 LB. HAMMER AND CHISEL TRYING TO CUT A 1/2" BOLT OFF OF A BELT RAIL ON THE NEW INSTALLATION OF THE LONGWALL BELT WHEN A PIECE OF METAL PENETRATED HIS RIGHT EYE. HE WAS TAKEN BY AMBULANCE TO THE HOSPITAL WERE THE METAL WAS REMOVED.
EE WAS USING A CHAIN RATCHET TO REMOVE A CABLE TRAY OFF OF OLD STAGE LOADER IN OLD PULLOUT AREA WHEN CHAIN RATCHET CAME LOOSE STRIKING HIM IN THE MOUTH, CAUSING A LACERATION REQUIRING 4 SUTURES. THIS WAS EE'S LAST SHIFT BEFORE STARTING HIS ALREADY-SCHEDULED VACATION. HE RETURNED TO WORK ON NEXT SCHEDULED SHIFT.
EE WAS REMOVING THE TOP OF A HYD. OIL CAN THAT HAD BEEN PARTIALLY CUT WITH A UTILITY KNIFE. HE WAS USING CHANNELLOCKS TO PULL ON LOOSE LID WHEN IT CAME OFF. IT HIT HIS LEFT HAND ON TOP OF THUMB, CAUSING A LACERATION.
Employee was using a railroad jack & a jack bar to help put a flat car back on the track that had been wrecked. When he went to release the jack the bar slipped out of his hand & hit him over the left eye resulting in a laceration for which he received stitches.
EE WAS CHANGING INSERTS ON THE RCM WHEN A PIECE OF METAL WENT INTO INDEX FINGER RT HAND. HAD TO BE REMOVED & STITCHES PUT IN.
EE WAS USING A RAILROAD JACK TO RAISE THE OUTBY END OF TAILPIECE TO RESET A CRIB BLOCK UNDER THE LEG WHEN THE JACK SLIPPED OUT CAUSING HIM TO FALL ON RT HAND & ARM. EE HAS CONTINUED TO WORK U NTIL 12/11/03 WHEN HE HAD TO HAVE A TENDON IN HIS RT ELBOW REPAIRED.
EMPLOYEE HAD A LACERATION THAT REQUIRED STITCHESAT HOSPITAL. EE WAS USING A CHISEL AND 4 LB HAMMER WHEN THE CHISEL OR A PIECE OF THE HAMMER SPLINTERED OFF AND WENT INTO LEFT LEG.
MEMBER WAS USING A SLATE BAR TO GET ROCK OFF TOPTHE NO.2 CM MACHINE. WHEN THE SLATE BAR SLIPPED HE WAS THROWN ONTO THE PIECE OF DRAWROCK. HE HAS CONTINUED TO WORK UNTIL 9/24/03 WHEN THE DOCTOR HE WAS SEEING REMOVED HIM FROM WORK UNTIL SURGERY COULD BE PERFORMED ON HIS RIGHT SHOULDER.
EE WAS STRIKING CHAIN GUIDE IN THE PAN LINE WITHA 10LB SLEDGE HAMMER WHEN A PIECE OF THE CHAIN GUIDE BROKE OFF GOING INTO HIS LEFT KNEE. HE WASTAKEN TO HOSPITAL WHERE THE METAL HAD TO SURGICA LLY REMOVED FROM HIS KNEE. NOTE EE WAS SCHEDULEDOFF UNTIL THE 1-5-03 HE WAS WORKING ONE OF HIS DAYS OFF AT THE TIME OF ACCIDENT.
EMPLOYEE WAS TRYING TO PUT A WRECKED FLAT CAR BACK ON THE TRACK WHEN THE RAILROAD JACK HE WAS USING CAME OUT CAUSING THE BAR THAT WAS IN THE JACK TO STRIKE HIM IN THE UPPER LIP CAUSING A LACERATION REQUIRING STITCHES.
EE WAS HELPING MAKE A CABLE SPLICE. HE WAS USING A UTILITY KNIFE TO CUT THE CABLE AND THE KNIFE SLIPPED LACERATING HIS LT. THIGH. STITCHES WERE REQUIRED TO CLOSE THE WOUND.
EE WAS USING A UTILITY KNIFE TO CUT BELT THE KNIFE SLIPPED AND LACERATED HIS LEFT WRIST. TENDONS WERE SEVERED RESULTING IN SURGERY TO REPAIR THE INJURY.
EE WAS LAYING TRACK. HIS FOOT WAS FRACTURED WHEN CHAIN RATCHET BROKE. RAIL FELL ON TOP OF HIS RIGHT FOOT.
EMPLOYEE WAS USING A BOW SAW TO CUT TIMBERS WHEN HIS HAND SLIPPED AND STRUCK THE SAW TEETH CAUSING A LACERATION THAT REQUIRED STITCHES.
