Employee was using a pry bar to unplug a clogged bit on a blast hole drill. Once the clogged bit was free of material, the employee dropped the pry bar onto EE's foot which contacted EE's foot in between the safety toe and the met guard of the boot causing a superficial fracture to EE's big toe.

Employee felt a pop in the left arm while using a pry bar to straighten a metal step. An MRI was performed with a reading on February 8th revealing a torn bicep tendon that will require surgery.

Employee was hurt leaning into man-lift basket.

IE was using a safety self retractable knife when the blade broke causing a thumb laceration requiring stitches.

While repairing a loader in the field, our service technician was struck in the face with a tool resulting in a fractured cheek bone.

Laceration to palm; left hand.

IE was removing a hydraulic hose. The IE was using an end wrench to remove the clamps due to the size of the area and when the last bolt broke loose IE's hand came in contact with the support bracket of the valve. IE was then taken to the hospital where an x-ray revealed a fracture to a 5th metacarpal bone on right hand. IE was wearing mechanics gloves.

While installing a pull down chain on a blast hole drill, the pry bar slipped pinching the IE's finger on the structure. The IE received a laceration requiring stiches.

Employee got a flat tire on pickup and called the mechanic to help change it. While pulling the jack off of the mechanics truck the jack slipped out of hand and fell to the ground. Then the jack struck the ground the handle on the jack bounced up and hit the employee in the cheek which caused a laceration which required stitches to close.

IE was working on a belt scraper when the wrench IE was using kicked back pinching IE's finger between the wrench and a piece of metal resulting in a laceration and fracture to the tip of IE's right ring finger. IE was seen at the emergency room where the decision was made to remove the tip of the finger.

Employee was stripping the insulation from a 480V power cable when the safety knife slipped cutting employee on the left palm of the hand. Employee was wearing gloves at the time of the injury. Employee received three sutures to close the wound and issued a full return to work release.

The injured employee received a laceration to the left arm after the box knife slipped when opening a mylar bag. The laceration required stitches.

While changing a return idler along a belt line, the employee made contact with the tool he was using causing a laceration above his right eyebrow requiring sutures making this a reportable injury.

IE was using a safety knife to strip a tech cable in preparation to make a splice. The blade of the knife came in contact with his right index finger resulting in a laceration that required three stitches to close. The employee was wearing leather work gloves.

Employee was changing a bit in a grinder tool when his wrench slipped causing a laceration to his left palm by his thumb.

Employee was tightening up bolts on the line boring equipment in preparation to align the bores on an equalizer of a P&H 85 yard Shovel Bucket. The employee was pulling a 1 1/8 box end wrench when the wrench slipped of the bolt striking him in the nose.

Employee was assembling two lengths of poly dewatering pipe. While installing bolts in the flanges, the wrench he was using slipped causing his right hand pinky finger to be pinched between the wrench and another bolt resulting in three stiches. Employee was wearing leather work gloves.

Employee was using a wrench to adjust a bolt, the wrench slipped causing employee to lunge forward and contact framework with lower lip. The injured employee received 5 stitches making this incident reportable.

Employee was cleaning pilot valve with putty knife and hand slipped, blade of putty knife came in contact with left thumb.

The injured employee was modifying his plastic tool box with a safety knife. The knife he was using slipped out of the plastic, did not retract and cut his left thumb and hand. He was wearing cut resistant gloves. The injured employee received eight stitches making this incident reportable.

While installing a hoist motor coupler the employee's finger was pinched requiring 5 stitches.

The employee was mopping the floor of the gate house. During which time he pivoted on his right leg twisting his knee.

EE was cutting the last piece of lagging to be installed on a 48" drive pully when his knife came into contact with the ceramic lagging causing the blade to break and slip onto his wrist resulting in two stitches.

EE was sharpening the blade on a electric powered skiver and the right index finger

The electrician was using a utility knife to strip a cable and expose the conductors. The knife slipped causing him to puncture his left hand with the knife blade. The puncture required 5 stitches.

The employee was using a router on plywood. He was wearing safety glasses with side shields and somehow got a small particle of wood in his eye that was embedded.

Changing idlers with another employee, when he hammered the idler out of the frame it bounced off another piece of iron and hit front tooth.

While installing tire deflating tool, plunge flew off from pressure and struck the technician in the face, cutting his upper lip.

Catering server was washing a kitchen knife and hand slipped causing a laceration to the right middle finger.

As the service technician was working beneath a pick-up truck, dirt was dislodged from the underside of the truck and entered the technician's right eye.

At the time of the incident, the technician was tightening a bolt on an intake tube. As he was applying pressure to the wrench, the socket slipped off the bolt, causing his hand to swing forward and strike the cylinder head, cutting his right "pinky" finger

Electrician was removing electrical tape from a motor lead with a utility knife. The knife slipped off the tape and cut his left thumb between the first and second knuckles. The cut was about 1" long and required four stiches to close.

Small laceration to right hand from a pocket knife.

The injured employee was using a knife to cut a nylon tie wrap off of a parts tote in the warehouse. When he cut through the tie wrap the knife continued in the direction of his left hand, puncturing it in the palm resulting in 6 stitches.

Employee was cutting an 8 kV cable using a utility knife to strip the insulation from the cable. The knife cut through his leather glove across the top of his left thumb. The injury required 15 stitches to close. Restricted duty.

The rubber that is used as filler in a cable belt splice is supplied on a roll which is suspended by means of an axle through the center. Rubber is pulled off roll by one employee and cut to length by another. During this task the injured employee got his hand in the way of the way of the knife, resulting in a laceration which required stitches.

Employee was clearing a chute that was plugging up and caught his right thumb, resulting in an avulsion.

Employee was installing concrete forms and had to pull a nail with claw hammer. When the nail came free the employee's hand came back and struck another form with a protruding screw, resulting in a laceration to the knuckle on the right hand, requiring five stitches.

Employee was working in a JLG basket w/co-worker bolting up iron. Co-worker struck ball pin with his hammer, it bounced off and struck employee in the mouth causing a laceration requiring stitches. Employee returned to work at full duty.

At the time of the incident the technician was disconnecting water lines from an expansion tank on a CAT 793 truck. As the technician was applying pressure to the wrench, the wrench slipped off the bolt causing the technician to strike his right hand against a wire clip. The resulting laceration required 8 sutures.

Installing wear bars. Pry bar slipped and struck employee's left eyebrow, resulting in 3 cm laceration.

Knife slipped cutting thumb, no other contributing factors

At the time of the incident EE was making repairs to an air compressor on a CAT 793 off-highway truck. As he was pulling on a wrench, to loosen an air line going to the compressor, he experienced pain in his upper left arm.

At the time of the incident, employee was removing a drain plug to take an oil sample. When he initially tried to remove the plug using a wrench, he found that he could not get it to turn. He then began to strike the end of the wrench handle with a hammer. While doing this he missed the handle and struck his finger.

At the time of the incident, employee was loosening bolts that held a transmission cooler in place. As he was applying pressure to a ratchet wrench, in order to loosen a very tight and rusted bolt, he felt the muscle in his left shoulder pull.

At the time of the incident ee was preparing to remove a roller frame from a CAT D11 dozer. As he was driving the pin, that attaches the equalizer bar to the roller frame, from its bore, a chisel that had been driven into the side of the bore to hold it open suddenly became dislodged and struck him in his face.

At the time of the incident employee was removing the engine from a CAT 797 off-highway truck. As the engine was being removed EE was using a pry bar to reposition the engine. As he applied force to the pry bar he experienced pain in his lower back.

Loosening ring bolts on revolving frame of shovel, back started aching.

AT THE TIME OF THE INCIDENT, EE WAS REMOVING A DRAIN PLUG FROM THE DIFFERENTIAL OF A CAT 793 OFF-HIGHWAY TRUCK. WHILE EE WAS APPLYING FORCE TO THE WRENCH/BREAKER BAR, THE DRAIN PLUG BROKE LOOSE, CAUSING EE TO FALL INTO THE CORNER OF A DRAIN CART. ON 5/8/04 EE CALLED IN AND REPORTED DIFFICULTY BREATHING AND PAIN ALONG HIS RIBS; DIAGNOSIS: CRACKED RIBS.

AT THE TIME OF THE INCIDENT, EE WAS LOOSENING A FITTING ON THE ENGINE OF A CAT 793 OFF-HIGHWAY TRUCK. AS HE WAS APPLYING PRESSURE TO A WRENCH THE BOLT SUDDENLY BROKE LOOSE, CAUSING THE HANDLE OF THE WRENCH TO DROP, PINCHING THE LITTLE FINGER ON HIS RIGHT HAND BETWEEN THE HANDLE AND A HARD LINE.
