Tool slipped and employee fell to a lower surface injuring the right elbow.
IE was attempting to remove a retainer that had been welded to the shovel bucket door. EE struck the retainer and the recoil resulted in the hammer coming back toward the miner and striking the IE in the lower lip. 4 sutures were required to correct the wound
Installed engine and removed turnbuckles from lifting jig. Lifting jig got hung up on horse collar. Wiggled jig with both hands to try to get jig free. Jig started to move but got hung up again. Wiggled it and jig moved sideways, as I was moving out of the way of the jig it popped loose moving very fast and caught my right hand pinky finger.
Employee was priming bore holes using cord and boosters. Conditions were muddy and as employee cut through the cord his foot slipped causing a body position change and the knife cutting his left thumb. Employee required 4 stitches. Employee was wearing leather gloves and using a self retracting safety knife.
Employee was loosening a tight bolt with a wrench. While pulling down on the tight bolt, it loosened. He then felt pain in his lower back.
While working on equipment in shop, EE was removing a relief from hydrolic cylinder with a wrench and his back popped. It only felt like a sore muscle until 3/27/2006.
EE WAS WORKING ON AN ELECTRICAL FAULT ON UNIT HU714/174. THERE WERE SOME ELECT. WIRES THAT NEEDED TO BE COVERED TO PROTECT THEM FROM RUBBING. HE WAS CUTTING ON A SHEET OF RUBBER TO WRAP AROUND THE WIRES USING A UTILITY KNIFE TO CUT WITH, THE KNIFE SLIPPED AND CUT HIS LEFT HAND BELOW HIS THUMB. STITCHES WERE REQUIRED.
FRACTURE OF THE MEDIAL MALLEOUS AT THE DISTAL END OF THE TIBIA.
EMPLOYEE REPORTED THAT WHILE HE WAS STRIKING RUBBER TIRED DOZER BLADE WEAR PLATES, WITH SLEDGE HAMMER, THE BLADE FELL ON HIS LEFT FOOT, PINCHINGOUTER LEFT SIDE OF FOOT BETWEEN THE GROUND AND L OWER PART OF BLADE. EMPLOYEE STATES THAT ENGINE AND TRANSMISSION WERE REMOVED FROM UNIT. BLADE WAS TIED WITH CHAIN TO HOLD IN PLACE. EMPLOYEE IN TURN STRUCK BLADE WITH HAMMER AND CHAIN BROKE.
EE WAS CUTTING RUBBER MATERIAL WITH UTILITY KNIFE. BLADE SLIPPED THROUGH BLADE HOLDER CUTTING RIGHT HAND BETWEEN THUMB & INDEX FINGER. STITCHES REQUIRED BY DR. TO CLOSE CUT. THIS BRAND OF UTIL ITY KNIFE HAS BEEN REMOVED FROM STOCK & REPLACEDWITH A BETTER QUALITY KNIFE.
EE WAS HELPING REPLACE THE PINON GEAR TO THE FRONT SWING MOTOR OF THE P&H 4100 SHOVEL. HE WAS UNABLE TO PLACE A 2 1/4" IMPACT SOCKET WRENCH ON ONE OF THE RETAINING BOLTS DUE TO BEARING FAILURE SO A 24" CRESCENT WRENCH WAS USED. HE WAS SWINGING A SLEDGE HAMMER AGAINST THE WRENCH IN AN ATTEMPT TO FREE THE BOLT WHEN HE MISSED THE WRENCH, SMASHING THE TIP OF HIS RIGHT INDEX FINGER BETW
