The injured employee was assisting with dragging a scrap truck frame when EE reached under the front bumper area to guide the Dyneema rope. A sharp metal shard protruding from the underside of the bumper contacted EE's right forearm.

Mostrando 50 de 106Employee was carrying a box of detonators while distributing them on blast pattern when they stepped into a low spot in the ground, employee felt discomfort in their abdomen. Employee was diagnosed with a hernia on 1-29-2026 making this a reportable incident.
IE slipped in mud as they tried to catch themselves with right leg they felt a pop in upper leg.
Tool slipped and employee fell to a lower surface injuring the right elbow.

On 06/12/2025 employee reported that a coworker kicked the back of the front seat from the back seat of a quad cab pickup on 06/11/2025. On August 4th the employee was treated at an urgent care facility and released with restrictions.
Left arm was caught between porta-potty door and frame due to excessive wind
Mechanic was attempting to install a hydraulic tank on a rubber tired dozer. The tank fell and struck the mechanic.
IE was removing #2 belly pan. IE planned to rattle hinge bolt with impact to get the pan to hinge downward. Bolt threaded out and allowed belly pan to move. Cable supporting the pan allowed the pan to move sideways due to weight. Side movement of pan pinched IE's arm between belly pan and side frame of tractor.
Contractor suffered fatal injuries while performing maintenance on "Dutchman" of electric shovel bucket. Fatal injuries resulted from being pinned between shovel bucket door and bucket.
IE was assisting another tech installing a bogie roller on a dozer. The IE was holding the bogie in place when it unexpectedly moved. The movement allowed the bogie to fall forward, raising the back of the bogie and pinching the IE's finger between the top of the bogie and bottom of the push arm.
IE was attempting to remove a retainer that had been welded to the shovel bucket door. EE struck the retainer and the recoil resulted in the hammer coming back toward the miner and striking the IE in the lower lip. 4 sutures were required to correct the wound
After lunch, the IE was dismounting dozer to do a walk around inspection. IE was dismounting the push arm of the dozer and stepped on a rock on the push arm step. IE received first aid and transported to Hospital. X-Rays were negative, but MRI results indicate a ruptured ACL.
IE claims that the tractor of IE's truck began to bounce causing IE to bounce in the seat. As a result IE suffered from back pain. Incident became reportable on 10/22/2021 when the IE was placed on light duty.
Employee was dismounting the dragline and stepped on uneven ground resulting in a sprained right ankle. IE was transported to hospital for evaluation, x-rays were negative and IE was released to work. On 8/12/2021 IE requested that Dr. take them off of work until fully released.
IE fell on rocks landing on the right side of body. No fractures, but too sore to return to scheduled shift.
27X49 tire wheel lock failed and struck miner in the hard hat/left forehead. Laceration required stitches.
IE assisting in changing dump ropes at the dragline. Coworker operating the tool carrier was hoisting the attached dump rope to reposition when a loop in the drag rope flipped over causing the cable/rope to strike the IE in the right ankle (who was walking away at the time of the incident) which caused the ankle to roll resulting in an ankle sprain.
EE jumped to ground from bottom step of dragline shoe (approx. 3 feet). Upon right foot contact with ground, EE experienced pain in foot. Heal fracture diagnosis - medical treatment.

Attempting to mount the chain case of a motor grader while lifting a starter into place. Slipped in mud, fell forward and pinched right little finger distal between the starter and chain case.
Truck operator traveled over an area that had been spot watered on a 3% grade. The truck slid sideways and then contacted a dry area of road. The right front tire deflated and the truck rolled over onto it's right side.
IE claims got dust in eye on 04/09 at 5:30pm. Thought it would go away, so EE did not report it to the supervisor. Irritation was present the next morning, so EE reported to the supervisor. Treating doctor prescribed an antibiotic ointment after removing 2 small pieces of dirt/rock.
IE stood up to exit cab of rubber tire dozer. As IE turned to open the door IE heard a pop and felt pain in knee. An MRI was ordered and IE placed on Light Duty until results of MRI are known
Installed engine and removed turnbuckles from lifting jig. Lifting jig got hung up on horse collar. Wiggled jig with both hands to try to get jig free. Jig started to move but got hung up again. Wiggled it and jig moved sideways, as I was moving out of the way of the jig it popped loose moving very fast and caught my right hand pinky finger.
Unloading a tightly packed shipping container filled with machining equipment. Stored energy due to the amount of equipment in container and movement equipment inside during travel of container. Mashed fingers between two metal boxes.
IE was attempting to put a dragline tooth in place. IE dropped the tooth and it struck an adapter on the ground. The energy from the two pieces of metal striking each other resulted in a sliver of metal striking the IE in the right shin. Metal was removed by a doctor.
IE was attempting to connect the discharge hose to a pump when the pump capsized. IE was struck by the pump resulting in fractured ribs.
IE placed a brake pod assembly on the back of his service truck. The assembly began to roll and the IE attempted to catch it. His hand was caught between the brake pod and bumper of truck resulting in a fracture to the right hand first metacarpal.
IE was walking around a light plant after setting it up at the shovel dump. IE stated the next thing he knew he was on the ground. IE was evaluated by ERT and transported to Hospital for X-Rays. X-Rays were negative and ER Dr. released IE to normal duty and scheduled an appointment with an orthopedic specialist. IE had orthoscopic procedures to repair a torn meniscus.
Employee was priming bore holes using cord and boosters. Conditions were muddy and as employee cut through the cord his foot slipped causing a body position change and the knife cutting his left thumb. Employee required 4 stitches. Employee was wearing leather gloves and using a self retracting safety knife.
Employee fell from a draw bridge style platform at the Pit #7 East silos when the welds on the platform failed. Employee landed on tail bone causing a fractured back and bruised tail bone area.
IE positioned outside bed of coworker's truck in employee parking lot, on driver's side. IE lifted tool box right side slid off bed rail and fell in bed of truck. Movement of box allowed IE's left thumb to be caught between tool box and bed rail of truck. IE told coworker to pull tool box because his thumb was caught. Coworker pulled box, caused avulsion required sutures.
IE was tying in a ramp on the spoil side. IE was moving from N-S on spoil side. IE felt the ground give away dozer started to slide. IE immediately lowered the blade, dozer continued to slide and eventually rolled over coming to a rest on the tracks. IE removed seatbelt and dismounted dozer Emergency Response Team members arrived on scene and transported to Hospital for examination
IE was washing crusher roller area when he attempted to reposition himself; as he was moving, he bumped into the open inspection door causing him to lose his balance and fall into the bearing block.
IE was inspecting #1 drag generator on BE-1570W dragline when the generator brushes flashed. The flash resulted in eye exposure to the electrical arc. IE was seen by ER doctor. No blisters or foreign material were detected under ultraviolet light, but the ER doctor prescribed an antibiotic ointment as a precautionary measure until the IE could be seen by an optometrist.
IE was assisting in positioning a loader tooth, when IE pulled IE felt pain in right shoulder. IE underwent surgery on May 8, 2014.
Team was replacing power cable at shovel. 30' of cable was spooled off cable reel. Cable reel operator backed up and spooled more cable off reel. Unknown to operator cable had backlashed on reel. Cable was being reeled back onto reel. Cable tight-lined, movement of cable struck IE on left leg. 4/9/2014 a brace was prescribed to support leg, IE was placed on Light Duty.
Initial audiometric test indicated a reportable STS shift. Retest was conducted and confirmed the condition. STS shift is being reported at this time and baseline will be revised. Employee no longer works for Peabody Energy, so no test will be conducted next year. Results will be sent to the individual by certified mail.

Employee was removing a belly pan on a D11, the pan was resting on a jack. A passing mine employee asked if our employee needed help pulling the jack out from under the machine. While moving the jack the pan slid off of the jack falling on our employee's foot.
Tower locking cyl. rod on IR DM-M2 drill had stripped from the locking pin. Contract tech and IE were in the process of positioning the cyl. rod in preparation to weld the rod to the pin. As the contractor was controlling the movement of the cyl. the IE was aligning the rod and pin. IE had his right thumb in a pinch point resulting in an amputation.
Driver was done unloading ammonium nitrate emulsion. Shut off pump unhooked hose and hung it up put cap on after pump side as he was standing up cap blew off and hit him on the right inside lower leg. He received seven stitches. We are still in the investigation.
IE and co-worker were replacing a sway bar on a haul truck. a broken portion of the sway bar was attached to the pin. As the pin started out of the frame bushing the IE reached to steady the pin, it rotated allowing a sharp edge to contact the IE's arm. IE was transported to hospital. IE received 9 sutures to repair the laceration then returned to work.
IE was boarding P&H 4100 Shovel, IE tripped on step causing IE to strike knee on upper step. IE was transported to Cibola General Hospital. X-rays were negative and the IE was released to work. He was seen by Orthopedic specialist and scheduled for an MRI. IE had surgery on 07/31/2012 to repair a torn meniscus.
IE WAS REMOVING A CHOKER FROM AN ELEVATED AREA, HE WAS UNAWARE THERE WAS A CLEVIS ON THE OTHER END OF THE SLING, WHEN HE PULLED DOWN ON THE SLING THE CLEVIS SWUNG AND HIT IE IN THE MOUTH. FACIAL PORTION OF THE #8 TOOTH WAS SHEERED REQUIRING A CROWN TO REPAIR THE TOOTH.
