Employee was trimming a piece of plastic when knife slipped resulting in a laceration to left index finger requiring five stitches.

While securing a load to the bed of a service truck with a ratchet strap the anchor point failed. the buckle from the strap struck the employee in the mouth damaging three teeth.

Employee was prepping a splice for vulcanization when the olfa knife employee was using slipped and contacted employee's arm. Employee was transported to the hospital and received stitches resulting in a recordable incident. Employee was released back to work full duty.

An employee was using a shovel to clean mud from the tracks of a dozer, when the shovel broke and the employee fell into the blade, cutting employee's ear.

IE was tapping on a rear strut with a hammer when EE's hand contacted a pulling rod that was in the strut pin causing a laceration to EE's left middle finger that required 5 sutures to close.

A Technician was using a bar to hold the bolt into place so they could attach the nut to the end of a bolt. While screwing the nut to the bolt, the bar slipped. This caused the bolt and attached nut to drop, pinching the Technicians left 5th finger in between the drag case and the nut. This split the Technicians skin on their finger requiring 4 stitches.

Removing fittings with a crescent wrench. While pulling on the wrench the employee felt a pop in left elbow. Initial diagnosis is muscle strain.

An equipment operator slipped on the dozer tracks while pulling on a pry-bar causing the bar to hit EE in the mouth, chipping EE's upper left front tooth.

Employee was swinging a sledge hammer against the outside of a bulk storage bin to dislodge the material inside. The end of the hammer became entangled in a stainless steel lifeline cable, causing a strain to the employee's shoulder. The employee received first aid treatment initially, but was later placed on restricted duty by the medical provider on January 13, 2017.

Was changing a bearing and got a piece of metal stuck in hand. Had it removed and required 3 stitches.

Changing ties at the mine - setting spikes and a shard split off and hit arm of employee, small particle removed, no lost time went back to work put wrap on arm.

Employee was removing a swing motor from a PC2000 Komatsu Excavator, when the fitting became loose employee's hand slipped down the wrench crushing it between the wrench and a hardline hose.

While trying to loosen a nut on a steering wheel of a 793 haul truck, the employee was pulling the wrench towards himself the wrench slipped and struck the employee in the mouth causing a chipped tooth.

A pit millwright was using a come-a-long to lift a shovel track pad when the hook slipped off the pad striking him on the forehead above the right eye. The result was a laceration requiring stitches.

An electrician was in the process of cutting the tape off a blower motor connection when the knife slipped and cut through his leather glove resulting in a laceration to his left hand that required stitches.

While loosening a plug with a ratchet the injured employee felt a pull in his shoulder/neck area which was diagnosed as a strained muscle and resulted in restricted duty.

A pit millwright was struck on the head/neck by the handle of a sledge hammer when it was improperly being used as a spacer between a hydraulic ram and an idler bearing block to adjust the tension of a shovel track.

Three plant technicians were removing grout from between the railroad tracks when a large section broke and fell from a skid steer bucket, causing a pry bar to flip up and hit an employee in the jaw. This resulted in multiple fractures to the left lower jaw.

Individual pinched hand between steering arm and jack, causing laceration requiring sutures.

The technician was struck in the mouth with a hammer by another technician while straightening an engine compartment ladder; resulting in chipped teeth and sutures.

EE was setting jack under right front side of haul truck. When putting it in position got hand caught between jack handle and haul truck.

The technician's right thumb was pinched between the hammer handle and the feeder chain flight when he struck the chain with the hammer. The result was a fractured thumb and stitches.

While starting a pin in the apron roller of a scraper, the mechanic struck his right middle finger with a hammer causing a laceration requiring sutures.

The mechanic was tightening a bolt on a hydraulic steering line when the wrench slipped off striking his left front tooth and chipped it.

A wrench slipped off while tightening a hose fitting and the employee lacerated his first and second knuckles on a metal edge. It required stitches to close the wound.

Was scraping mud from stairs, scraper got caught and cut thumb and ring finger.

The technician's palm and knuckle on the left hand were lacerated/pinched when the swing style pin driver he was using glanced off of the wedge in the dump rope socket.

Employee was installing propel motor on P&H 2800 shovel. Employee tightened feet bolts with impact wrench. Injured worker was verifying that he had all the bolts tightened by using 2-1/4" slap wrench and sledge hammer. As employee attempted the bottom foot bolt, the slap wrench came off the nut, striking the employee in the mouth, chipping the top right central incisor.

While driving a bearing out, a chip broke off and struck right thigh. Individual received stitches to close wound.

Technician was loosening fitting on engine when it suddenly came loose, pinching and cutting his hand between the wrench and another fitting. Cut required stitches to close.

The mechanic was prying the top of the LR suspension out of its pinned position. The suspension was supported by a come-a-long near the bottom of the barrel. As the weight began to shift, it caused the prybar to recoil rapidly, pinching/severing the tip of his right pinky finger.

A mechanic slipped while using a knife to cut a wire-tie and cut his left hand. It required stitches to repair.

While working on removing an injector, the pry bar slipped and hit the individual in the upper lip, requiring stitches.

Employee was jacking a clutch on the propel using a port-a-power, the ram kicked out and hit him in the R.H. leg.

At the time of the incident the service technician was removing damaged hydraulic hoses from a haul truck. As the the tech was using a knife to cut the plastic ties that held the hoses in place, the knife slipped and cut his left wrist.

At the time of the incident the service technician was removing fuel lines from the fuel transfer pump of a CAT 793 truck. As the technician was applying pressure to a wrench, the ladder he was standing on tipped causing him to fall to the ground and land on his right wrist.

Employee was tightening bolts on primary steel; wrench slipped, arm was torn open at bicep muscle and required stitches.

At the time of incident the technician was replacing a damaged hydraulic hose. As the technician was applying considerable pressure to the wrench, the wrench slipped off the bolt and struck him in the mouth, chipping a tooth.

Employee was changing teeth on a loader bucket. One wouldn't come loose so he hit it with a hammer. A chip of steel came off the tooth and embedded in his left thigh. There was no initial treatment as the emergency room doctor recommended he leave it in place. A second opinion by a surgeon recommended removing it. He had it removed on 9/20/07.

Employee was using a hammer to uncrate fan, over extended arm causing partial thickness tear to tendon.

While chipping slag, got a particle in his eye. He was wearing safety glasses and a welding helmet. Antibiotics were administered.

The technician was struck in the left shin with a piece of metal when striking a ripper tooth with a hammer. The resulting laceration required sutures.

A snap-ring came off the snap-ring pliers just as it cleared the housing and struck the employee, causing a 1 1/2" laceration to his chin. The injury required stitches to close it.

Employee was swinging a sledge hammer to loosen a tooth on a dragline bucket. He changed his swing from right to left and his backed popped. He worked the rest of his rotation, 2 1/2 days and on 1/24/06, his days off, he saw a chiropractor and was manipulated.

Employee struck steering pin (haul truck) with sledge and two pieces broke loose, penetrating his right thigh. He pulled one piece out himself and the emergency doctor decided to leave the other in place. The wound was sutured.

Employee was using a hammer to drive a master link in a feeder chain. One of the blows glanced off resulting in him smashing his finger between the hammer handle and the door frame to the feeder. The finger was fractured and lacerated and required sutures to close.

Employee was cutting electrical tape with a pocket knife when the knife slipped and cut his left wrist. The laceration required sutures to close.

The employee had experienced several years of chronic shoulder pain. While pulling a chain wrench, it broke and he experienced more pain. He continued to work until October 28 when he underwent surgery. The doctor diagnosed a rotator cuff tear with the underlying cause being a large acromioclavicular spur.
