An employee was using a shovel to clean mud from the tracks of a dozer, when the shovel broke and the employee fell into the blade, cutting employee's ear.
An employee's hand slipped off a haul truck shifting handle and EE struck the trailer brake module, causing a fracture to EE's hand.

Nelson Brothers detonated an explosives blast in the R3 Coal of the 6-West Pit of Coal Creek Mine. Burning coal dust was carried by north winds and ignited a grass fire south of the pit perimeter, at the fence line between Coal Creek Mine and Dennis L. Edwards. The fire burned 12.1 acres of prairie in the northeast corner of the Edwards property.

Three plant technicians were removing grout from between the railroad tracks when a large section broke and fell from a skid steer bucket, causing a pry bar to flip up and hit an employee in the jaw. This resulted in multiple fractures to the left lower jaw.

Employee had stepped over the tongue of a skid steer trailer carrying 3 rolls of cord. As the employee's second foot contacted the ground after step over, his foot slipped and he lost balance causing him to fall backwards onto the tongue of the skid steer trailer fracturing three ribs on his right side. Walking surface was melting snow and ice.

Employee used his hand to help guide a drill bit that had become stuck. When the drill started, the employee's glove became entangled in the bit causing a fingertip amputation of the left hand, fourth fingertip.

The technician walked approximately 20 feet from his equipment to look the work area over. While walking back to the equipment, he experienced pain on his right knee cap from repetitive motion.
