Employee was trimming a splice and caught his knife on the buckle. He twisted the edge of the belt. The knife glanced off of buckle striking employee on his left thumb. This resulted in a laceration which required two (2) stitches to close the wound.

Employee was pulling a Head Drive over with a Chain Ratchet. The chain broke and the chain hit him in the knee. Employee did not go to the doctor until the following day.

Employee was loading roof bolter and when he cut the strap on the pallet of roof pans, he cut his left hand with a utility knife employee was wearing gloves. Resulting in (5) stitches

Employee was making a cable splice, while using a 4lb hammer to hit the knocker. The side of the anvil broke. A piece of metal splintered and cut the skin and the metal embedded just behind the left thumb. The doctor removed the piece of metal and put one stitch in to close the wound.

EE was holding up a highline cable while another employee cut rope hangers down. When the other employee cut one of the rope hangers down, the highline cable came down causing his knife to hit employee's right elbow. (4 stitches)

EE was plastering on overcast wall outby section of Rt. Mains. EE had to adjust safety glasses and slid them down on the nose to un-fog. EE was putting plaster on underside when the B-bond plaster splattered into the employee's eyes. The employee was brought to the surface, eyes flushed out. Taken to the hospital, eyes examined, Lt. eye scratched, dialated eyes & medication.

EE was cleaning out material from under chain lid on coal feeder. While replacing the lid, the slate bar slipped, causing the lid to slide and mashed and cut the EE's right hand between the lid and the coal feeder. This resulted in having 4 stitches to the top of the EE's right hand.

Employee was cutting on a piece of cable when his belt knife slipped cutting his upper right leg. Required gluing cut back together. Employee was on 003 section when injured.

While tightening top of drill pot, the wrench slipped, causing EE to strike pot with his right hand.

EE was taping out drilled hole when the tap pin broke. EE's right hand fell on broke tap, resulting in laceration.

Employee was cutting mesh with razor knife. He was pulling knife down, knife slipped and he cut into his left forearm.

EE was knocking a lock off with a hammer when a piece of metal from the hammer went into his thumb.

EE was trying to extract a miner bit when his hammer splintered causing a piece of the hammer to became lodged in his right hand index finger.

Employee was using a hammer & chisel to remove right rear drive axle on roof bolter when dirt & rock fell into his right eye.

Employee was hitting the feeder conveyor chain with a hammer when a piece of metal flew off strucking him in his right hand.

Employee was cutting belt with belt knife when the knife slipped cutting his right thigh. Employee received 5 stitches - on restricted duty.

EE was changing tailpiece bearing on belt and was hitting the tailroller with a sledge hammer when he injured his lower back.

The mantrip was stuck on a rock and ee was trying to free it up with a slate bar. The bar slipped and ee crushed his finger between the bar and mantrip.

EE WAS CUTTING A 18" ROPE HANGER WITH A BELT KNIFE WHEN HE CUT THROUGH THE ROPE AND INTO HIS LEFT HAND. HE RECEIVED 4 STITCHES TO HIS HAND.
