While EE was working on C/M EE received a laceration to the finger, EE did not file an incident report until 7/15/19 after seeking medical attention on 7/12/19 for an infection. Cause of this incident was lack of concentration. PPE was not a factor/mining equipment was not a factor/compliance with rules/regs was not a factor, miner proficiency may have been a factor

While breaking a rock with a sledge hammer, employee smashed right index finger between rock and hammer handle, causing a laceration requiring stitches.

Employee was lowering a track jack when EE's finger was caught between the cap of the jack and the body of the jack resulting in a laceration. Cause of this incident was employee concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.

Employee was installing water manifold on head gate water tree, while doing so EE was struck in the finger with a hammer resulting in a laceration requiring glue. Contract employee was helping another contractor to change a roll of felt material, cardboard tube got caught on roller and while using a knife, contract employee accidentally cut finger. Contract employee was cutting the belt for making a mechanical splice, as EE was cutting the belt, the knife contacted upper leg when EE reached the edge of the belt resulting in a laceration. EE was using 4lb hammer to make belt splice on 12/4/16,during work activities EE states they strained wrist. EE worked w/o restriction until seeking med attn on 2/28/17 becoming a RLT on 3/1/17.Cause of the incident was lack of EE concentration. Employee was using a knife to cut a nylon winch rope when the knife slipped resulting in a laceration to the thumb. Employee's lack of awareness was the cause of this incident. Employee was lowering jack, in the process of re-railing a mantrip. Employee failed to keep firm grip on jack bar as instructed. Resulting in jack bar coming out of jack striking employee behind right ear resulting in laceration. Employee was attempting to retract a tape measure when it slipped resulting in his finger being lacerated. Cause of the incident was the employee not paying attention. Injured was cutting a bad section of rockdust hose out with a razor knife, the blade broke. This cause the knife to slip off the hose cutting left wrist. Cause not using the right tool for the job. Employee was using a pick hammer to pound on his drill chuck to free up stuck drill steel on his roof bolter when a piece of metal broke off striking him in the forehead. Using track jack to raise critter went to lower the jack when the top part of the jack fell smashing finger between top of the jack and bottom part of the extension. Employee finished pinning slope belt together and was cutting trailing edge of the belt and the belt knife slipped stabbing him in the leg. PPE was not a factor, mining equipment/system was not a factor, compliance of rules/regulations could have been a factor, miner proficiency was a factor.

Employee over exerted his shoulder. Injured worker sought medical care, and on 12/19/2013 underwent surgical procedure.

During part 50 audit, it was determined this injury was reportable. Employee was removing a hose from inside shield, used hammer to break loose fitting, hit thumb instead of fitting. When he got ready to strike the pin the hammer slipped out of his hand striking the victim in face breaking his two front teeth. Cutting hole in mine belt for winch cable, the knife blade broke. When the blade broke my left hand went forward cutting it on the broken blade left in the mine belt. During a part 50 audit, it was determined that this injury was reportable due to embedded object. Employee was changing bits and a piece of the hammer (sliver) flew off embedding into his forehead above his eye. Cause of this injury was using pick hammer as a tool. Employee was using a chisel to cut weld off an area light on miner and chisel popped off the weld, it hit him in the chin causing a laceration.

He was jacking up the tripper drive base, the jack kicked out hitting him on the jaw and chin.

Helping to remove drive sprocket at tail gate drive along with assistance from scoop car. Scoop car had chain holding sprocket when positioning and he assisted with stone bar to put in place, chain broke allowing the pressure of the bar to be pushed into tail gate drive housing, cutting his hand. Was grooving a fire suppression pipe, had hand on ratchet which turns groover, the groover handle came out of the main body taking right hand under the machine, breaking thumb. Cutting shuttle car cable with utility knife. Holding cable approx waist high, cutting away from himself and cable twisted causing knife to slip off and he cut his left leg.Was approx 1 1/2 to 2"cut. Employee and another co-worker were making a belt splice and as the two worked toward each other he was struck above the eye with a hammer. Cause of this injury was lack of communication and safety awareness. Employee was lowering a jack and he did not notice that the jack did not latch and the handle snapped up and struck him on his wrist. Cause of this injury was safety awareness. Employee was using a hammer to pounding against a metal drill chuck and a piece of metal flew off sticking him in his lower leg. Cause of this injury was safety awareness and knowledge.
