EE pulled beside tank in pit, EE attempted to fuel up a pump in the pit. EE noticed cap was tight, used both hands to remove the cap. Used cell phone for light to see if any fuel was in tank, laid phone on tank. Then the diesel fumes ignited, causing flash. Hair on head and face were singed. EE drove out of pit/talked w/an EE. Drove self to hospital, treated for 2nd deg burns.

EE WAS BURNING GRATES OUT OF THE #1 FINISH MILL WITH AN AXY/ACETYLENE TORCH WHEN GAS LEAKED FROM HOSE INTO EE'S BURNING GLOVE. A SPARK SUBSEQUENTLY MADE CONTACT WITH EHE GLOVE CAUSING THE GAS TO BURN HIS LEFT HAND.

EMPLOYEE REPORTED HE WAS ATTEMPTING TO LIGHT THE KILN IGNITION TORCH AT THIS POINT UNBURNED NATURAL GASES IN HOOD IGNITED COMING THRU THE KILN OPENINGS STRIKING HIM ON THE FACE & NECK

INJURED WAS LIG2TING COAL STOKER FURNACE IN PREPARATION TO START RAW MILLS AN APPROX 2IN BED OF COAL HAD BEEN RUN INTO FURNACE (COAL WAS DRY & DUSTY) THE WITNESS TO THE ACCIDENT THE MILLER HEL PER HAD SPRAYED3THE BED OF COAL WITH FUEL OIL AS THE INJURED TOSSED A LIGHTED ROLLED-UP PAPER TOWEL THROUGH THE SOUTHWEST STOKER DOOR A PREMATURE IGNITION OCCURRED VENTING THRU OPENED DOOR
