EE was repairing a tear on T1 conveyor belt. As EE was removing the bad belt EE's locking blade knife slipped and cut left forearm. Injury resulted in 5 stitches. EE was wearing cut resistant gloves

While removing material from the loading face, the pit loader came to a sudden stop, "jolting" the machine. Later in the shift, employee reported some discomfort between the shoulder blades. First aid/self care measures were employed. A clinic visit was required on 9/23. At that time, the employee's work duties were restricted from operating equipment due to medical precautions.

Employee loading trucks in pit with 990H stated a 6'x2' rock slid appr 13' down muckpile and struck left front corner of the bucket and felt a pain in neck. Clinic placed employee on restricted duty on 8/14/19, currently assigned to operate smaller loader

EE pulled beside tank in pit, EE attempted to fuel up a pump in the pit. EE noticed cap was tight, used both hands to remove the cap. Used cell phone for light to see if any fuel was in tank, laid phone on tank. Then the diesel fumes ignited, causing flash. Hair on head and face were singed. EE drove out of pit/talked w/an EE. Drove self to hospital, treated for 2nd deg burns.

IE was pouring hydraulic fluid into rock breaker at the jaw when some fluid spilled/splashed up and into IE's face. IE was wearing safety glasses but fluid ran down IE's face and into IE's R eye. Another EE was in the area but not a witness to the accident & helped IE flush out eye at eye wash station. IE was taken to local ER and treated & released to go back to work that night.

A mining foreman was operating a Cat D6K2 LGP dozer blading a haul road. While blading he struck a rock, buried in the subgrade. This stopped the machine suddenly, causing the operator to lunge forward and strike his head against a steel grab bar in the machine. Injury resulting was a left forehead laceration above the left eye, requiring 10 stitches.

Employee was tightening bolts on a jaw crusher with a knock wrench. The wrench slipped from the bolt and struck him in the arm. Initial treatment was first aid. Follow-up visit on 06/02/2014 resulted in the application of a removable wrist splint. Employee was also transferred to another operator's tower not requiring the use of a joystick controller.

EE was carrying an inmpact roller w/co-worker. Tripped over a rock that he did not see, fell and dropped roller on left foot/ankle.

Employee donned gloves to grease his loader, and shortly thereafter felt his left hand itching. He removed his glove and saw his hand was beginning to swell. He was apparently bitten by some type of insect. He was examined by the company physician and administered medications to control inflammation.

The employee was removing rocks from a jaw crusher by hand that had been shut down. Employee felt pain in lower back, and was later diagnosed with a muscle strain. He returned to work on restricted duty.

EMPLOYEE WAS SENT TO ROLLUP WELDING LEAD ON WELDING TRUCK. THE REEL IS MANUEL. HE ROLLED & HIT HIS RT. THUMB ON EDGE OF REEL FRAME, CUTTING HIS THUMB. AT THE DR.'S OFFICE THE THUMB RECEIVED (3)STITCHES & WAS BANDAGED UP. EE RETURNED BACK TO WORK.

Employee was installing a return roller on a conveyor. While using a pry bar to seat the roller, the bar slipped, striking the employee in the mouth. He suffered a laceration to his lip. The injury required stitches. The employee returned to work.

A CUSTOMER truck driver employed by Autry Grading was removing a rock hung in the tailgate of the truck before being loaded. The tailgate slammed on her left index finger, causing a crushing avulsion of the tip of the finger.

EE WAS LIFTING THE HOOD ON THE PC-300 TRACKHOE IN ORDER TO CHECK FLUIDS, WHEN HE FELT A PAIN IN HIS BACK. HAVING HAD BACK PAIN FROM TIME TO TIME HE DID NOT CONSIDER IT A PROBLEM & WORKED THE R EMAINDER OF THE DAY.

1560 CRUSHER JAMMED UP WITH ROCK. WHILE USING A METAL BAR TO DIG MATERIAL OUT, BAR SLIPPED CAUSING EMPLOYEE TO HIT HIS HAND UP AGAINST CRUSHER BOX, WHILE WEARING WORK GLOVES STILL CAUSED LACER ATION TO EMPLOYEE'S FINGER.
